Approximately 78 million adults in the United States have hypertension (high blood pressure) and about 17 million of those adults are unaware or undiagnosed. High blood pressure is a universal risk factor for heart disease and stroke, the leading # 1 and # 4 causes of death and # 1 cause disability in the UNITED STATES respectively. The American Heart Association wants people to understand how they can monitor and improve their heart health by using our web portal and engaging in community‐based health programs like Check. Change. Control. to maintain a heart healthy life

The Program

Check. Change Control. which is a four month programthat utilizes both online and offline components. Online, participants join Heart 360, our web based health management tool. This tool allows an individual to track their numbers (blood pressure, glucose, cholesterol) as well as track their risk reduction efforts (physical activity, weight). Offline, Check. Change. Control. Offers participants education about hypertension, stroke, blood pressure management, nutrition and physical activity to maintain a healthy lifestyle.
