    COMMUNITY HEALTH CENTRE: SERVICES OFFERED

    CHC Basna is 30 bedded facility. It is well connected with roads & easily approachable from bus

    stand. It is located within residential area. The health centre provides basic primary health care

    services & diagnostic facilities of routine Lab tests & X-ray, family planning services TT &

    NSVT , services of normal delivery. The health centre also offers its services under various

    national programmes like RNTCP, AIDS Control, NLEP, NBCP, JSY, JSSK etc. The basic

    The health centre is well connected with all weather road & is near to bus stand.

    No signage in front.

    Boundary wall is there.

    No designated parking space.

    02 separate building one for OPD & another for IPD.

    SUGGESTIONS:

    Signagerequired in front.

    Concretization of floor required.(either from JDS or from RSBY CMHDF)

    Ramp can be constructed (with support from PWD).

    WAITING AREA

    Very congested area.

    No adequate space for sitting of patients .

    No facility of drinking water (water cooler supposed to be provided by RSBY fund).

    REGISTRATION COUNTER& MITANIN HELP DESK

    Separate(open)area forregistration counter & mitanin help desk located close to entrance

    in front of OPD.

    Manual entry being done.

    User fees displayed. Preprinted OPD slips is used .

    SUGGESTIONS:

    Registration counter can be closed with Aluminium section partition.

    CONSULTATION ROOM

    There is single consultation room.

    No examination table at place.

    Doctors duty chart displayed there.

    Essential equipments & instruments were available in room.

    INJECTION ROOM

    Open area being used.

    Screen with curtains required.

    Infection control practices were found dissatisfactory.

    SUGGESTIONS:

    Screen with curtain required for privacy.

    Puncture proof container for needle disposal required.

    Emergency drug tray with list needed.

    Disinfection with 1% Sodium Chloride solution required.

    Uses of PPEs need to be encouraged.

    DRESSING ROOM

    There is separate room for dressing.

    Adequate space is there.

    All essential drugs found at place.

    SUGGESTIONS:

    Emergency drug tray with list required.

    DRUG DISPENSING COUNTER

    Single window drug dispensing counter with qualified pharmacist present.

    Proper timing not displayed.

    Near expiry drug list not displayed.

    It provides facility of basic routine tests.

    Insufficient space, untidy room.

    No demarcated area for sample collection, testing & reporting.

    Seepage from roof &sidewall. Infection control practices not being followed.

    SUGGESTIONS:

    Seepage to be constructed with support of PWD.

    X-RAY

    X-ray machine in working condition.

    Lead apron available.

    No caution signage is there.

    IN PATIENT WARD

    No patient was there.

    Beds were properly arranged.

    Mosquito net required.

    Toilets attached to ward found uncleaned.

    There was no IV stand, stool & bedside locker .

    Water supply from Nagar Panchayat , motor pump not working since long.

    SUGGESTIONS: Mosquito net in all windows required ( with support from PWD )

    Regular cleaning of toilets with phenyl required.

    For supply of IV stand,bedside lockers & stools coordination with district store required.

    LABOUR ROOM

    Services of normal deliveries available.

    02 labour tables with screen at place.

    24 hour running tap water available.

    Provision of attached toilet is there.

    Adequate power backup present.

    NBCC at one corner.

    Improper illumination.

    Radiant warmer not available (must required). Suction machine unavailable (must required).

    No display of essential drug list/

    Seepage from sidewall(prone to infections).

    Shadowless lamp not working, not reported yet.

    SUGGESTIONS:

    For Radiant warmer & Suction machine coordination with district required.

    Essential drug list need to be displayed.

    Staff training & oroentation required.

    Seepage can be constructed with support of PWD.

    Curtains in windows required.

    OPERATION THEATRE

    OT is there with proper zoning.

    Slipper stand not found at place.

    Fumigation done on previous OT days.

    Running tap water facility available.

    Use of Signallac tape was suggested(during sterilization to ensure proper sterilization).

    Adequate power back up present.

    Essential equipments found at place.

    OT culture not dent since last year.

    Insufficient cooling. One more AC required.

    Examination table is being used despite availability of OT table.

    SUGGESTIONS;

    Signallac tape can be purchased locally.

    OT table must be used.

    Infection control practices need to be encouraged.

    BIOMEDICAL WASTE MANAGEMENT & INFECTION CONTROL PRACTICES

    Color coded bins not found at all designated places.

    Disinfection with 1% sodium chloride solution is not being practiced.

    PPEs used by staff is not satisfactory.

    SUGGESTIONS:

    Color coded bins for all patient care areas can be purchsed from JDS.

    Disinfection with 1% sodium chloride solution need to be ensured.

    PPEs used by staff is to be encouraged.

    Regular cleaning & mopping of premises with disinfectant must required.

    Reshuffling of different rooms can be done in OPD area which will be convenient for both

    hospital staff & patients. It can be done as given below:

    CONCLUSION

    Seepage problems in OPD area & Labour room must repaired (with support from PWD).

    Reshuffling of different rooms can be done in OPD area.

    Whitewash of OPD building required. Fall ceiling to be given at some places in OPD area.

    Training of class 3 & class 4 employee for Biomedical waste management & infection

    control practices required.
