By signing this application, I certify under penalty of law that the statements made in this application and its required attachments are true and correct to the best of my knowledge and information following reasonable investigation.

I further certify that Charter School (“Charter School”) is in compliance with the Charter School Law, 24 P.S. § 17-1701-A et seq., including the requirements that: (1) a person who serves as administrator for the Charter School is not employed by, receiving compensation from, or providing services to another charter school or cyber charter school or a company that provides management or other services to another charter school or cyber charter school; and (2) the Charter School receives and disburses funds for charter school purposes only.

I understand that false statements herein may subject me to criminal prosecution under 18 Pa.C.S. § 4904, relating to unsworn falsification to authorities.
