Chapter Awards Application This application is designed to showcase those chapters who are contributing to the fraternity experience in Phi Sigma Kappa through creative, informative, and relevant programming using “The Basics” guidelines. The application will be used to determine awards for individual categories as described in “The Basics” program (New Member Education, Scholarship, Community/Campus Involvement, Website Development, Rush/Recruitment, Risk Management, Philanthropy, Public Relations/Community Service), as well as the Most Improved Chapter and Outstanding Chapter Awards. All chapters and colonies are encouraged to apply. The winners will receive their prestigious awards and Grand Chapter recognition at this summer’s annual meeting. The awards will be one of the meeting’s highlights, and as a winner, your chapter will be in the spotlight! Your chapter’s accomplishments will also be featured in the Signet. Chapter Awards Application

This application is designed to showcase those chapters who are contributing to the fraternity experience in Phi Sigma Kappa through creative, informative, and relevant programming using “The Basics” guidelines. Entry Rules(PLEASE READ BEFORE COMPLETING APPLICATION)

1. The Chapter Overview section is to be completed by everyone, regardless ifyou are applying for Outstanding Chapter, Most Improved or a category award.

2. All chapters must complete applications for the specific awards category they are applying for.

3. Any chapter wanting consideration for Outstanding Chapter or Most Improved Chapter must apply for ALL awards.

4. All forms of the application must be completed in full.5. Send supporting materials with the entry forms.6. At least one letter of recommendation from a fourth degree member is required for the

application to be complete. The letter may address a specific category or summarize thechapter as a whole and must be received with the application. Letters from PSK Headquarters staff cannot be accepted.

7. Entries will not be returned and some may be displayed at this summer’s annual meeting. The entries will be displayed on the website as well.

8. All entries are to be sent as Microsoft Word or PDF files to [email protected] no later than 12 midnight May 15. To obtain free software that converts documents to PDFs, go to www.primopdf.com

Judging

Criteria for selection include the creativity, implementation and effectiveness of chapter programming. Entries will also be evaluated on the quality and relevance of supporting material submitted and responses to questions listed on the application. Recipients will be selected objectively by the Awards Committee and will be announced during this summer’s annual conference.

Note: If your chapter sends in an awards application, it is your responsibility to ensure its receipt at the International Headquarters. Be sure to call the IHQ to confirm receipt.
