ORGANIZATION

Easterseals Blake Foundation reaches over 20,000 individuals

and their families annually throughout Arizona, providing

infants, children, adults, and families with a broad continuum

of services including early intervention and family support &

preservation, child care and training, behavioral health services,

vocational and independent living training, and community

residential services. We are the largest social services non-profit

employer in Southern Arizona.

MISSION

ESBF has a significant impact on our community, with an annual

budget of 46M we are the largest nonprofit, human services

provider in Southern Arizona, employing more than 1,100

full-time staff.

VISION

We will provide opportunities, support and services to

children, adults and families, with compassion and respect for

them as valued members of their communities, involved and

participating based on their own choices.

INTAKE & COORDINATION OF CARE SERVICES

Intake and Coordination of Care Services provides children, teens and adults with customized eligibility, enrollment and referral services for covered behavioral health needs including therapy, family support and in-home intervention. Comprehensive assessments, coordination of treatment plans, as well as, psychiatric and medication services are also available.

CHILDREN & FAMILY SERVICES

Children & Family Services programs provide support to children and families in the areas of family support, reunification, parenting, child development and early education, behavioral health and developmental disabilities.

COMMUNITY LIVING SERVICES

Community Living Services provides residential and rehabilitative care for adults with cognitive and physical disabilities. Some of the residents are dually-diagnosed with physical and cognitive needs. They need full-time habitation care in order to perform daily living activities.

SAGE EMPLOYMENT & COMMUNITY SERVICES

SAGE provides individualized residential supports and job development services to individuals in their late teens through adulthood. SAGE operates with a philosophy on the belief that all individuals must be full and active participants in school, work and community, occupying socially-valued roles. SAGE also offers behavioral health services, including equine-assisted health services and camps to youth.

SUPPORTIVE LIVING SERVICES

Supportive Living Services provides therapeutic homes for children aged 5-18 with behavioral and/or mental health issues, as well as group homes for adults 18 and older with serious mental illness, traumatic brain injury, and/or substance abuse issues, plus outreach services to ensure medication compliance and daily living supports in community based settings.

WALK RUN ROLL

Saturday, May 6, 2017 at Reid Park (22nd and Country Club)

The Walk Run Roll event is an opportunity for the community to come together to raise funds for Easterseals Blake Foundation. The walk will be a family friendly event that includes a 2.5K leisurely walk, followed by an expo, food and snacks and entertainment. We are expecting in excess of 1,000 walkers that day.

The Spring Golf Classic is an annual event to benefit Easterseals Blake Foundation. The event includes the golf tournament, followed by a buffet dinner with a raffle and prizes.

Meet Zig.

Zig is 5 years old. He’s a happy, bright child that loves to play and socialize. But he wasn’t always this

way. His mother Erikka had a difficult labor ending with an emergency C-section because Zig’s heart

rate began to drop. He seemed healthy, with high Apgar scores, but was not able to breathe well.

Erikka recalls that it appeared as if he was panting like a dog.

During his first year, he was often sick and continued to breathe oddly. Erikka was exhausted

trying to help her baby and she feared that he was in pain. He cried continuously and his head was

continually at a tilt, as if he was unable to hold it up.

Doctors repeatedly told her not to, yet Erikka couldn’t help but worry. Was Zig autistic? Or was there

something bigger going on? He just didn’t seem to be developing as a child should. When Zig was

5 months, Erikka was determined to find the help her child needed and contacted The Arizona Early

Intervention Program (AzEIP). AzEIP is Arizona’s statewide, interagency system of supports and

services for infants and toddlers with developmental delays or disabilities and their families. AzEIP

partners with Easterseals Blake Foundation (ESBF) in Tucson.

The ESBF therapist agreed immediately that something was not typical with Zig and for the first

time Erikka felt confident that Zig was going to receive the help he needed. He began working

with a speech and physical therapist and when progress was slow, Zig’s case worker recommended

an evaluation by a neurologist. The results astounded everyone. During Erikka’s difficult labor, it

was likely Zig had had a stroke. At 18 months, Zig was diagnosed with hemiplegic Cerebral Palsy,

dysarthria, and motor apraxia.

With a clear diagnosis, Erikka was able to ensure that Zig received appropriate treatment!

“I’m really thankful,” explained Erikka. “he has gown leaps and bounds and is a completely different

kid than he was a year ago. “

Through it all, ESBF helped Erikka navigate uncertainty and become an advocate for her child.

“I’m happy he got help so soon, the early intervention services have given Zig the tools necessary to

overcome what used to be significant weaknesses.”

These days Zig is thriving. He used to need assistance going up and down steps but now he’s

jumping and climbing and running like the other children.

“Easterseals Blake Foundation has changed Zig’s life. They were able to identify his issues and help

validate my concerns. We were able to intervene early enough so that here we are, we have a five-

year-old that is able to communicate his needs, self-regulate, and who can ride a bike!”
