OVERVIEW GUIDELINES NOTICE IS HEREBY GIVEN that the Board of Trustees of the Chabot-Las Positas Community College District, State of California, hereby calls for qualified persons, firms, partnerships, corporations, associations, or professional organizations to provide full design-build services, Preconstruction and Construction Services, for the New Baseball Field Project, Chabot College. (“Project”). Respondent to this Request for Qualifications (“RFQ”) shall deliver one (1) original and three (3) bound copies and one (1) electronic copy, to:

REQUEST FOR QUALIFICATIONS (“RFQ”)

FOR NEW BASEBALL FIELD PROJECT, CHABOT COLLEGE

NOTICE IS HEREBY GIVEN that the Chabot-Las Positas Community College District (“District”) Board of Trustees invites Design-Build Entities (hereinafter “DBE” or “Respondents”) to submit the Pre-Qualification Questionnaire and other information required by this RFQ for the design and construction of the New Baseball Field Project, Chabot College (“Project”) on the District’s Chabot College Campus located at 25555 Hesperian Boulevard Hayward, CA 94545.

1. Request for Qualifications.

1.1. General.

1.1.1. Purpose of RFQ. The purpose of this RFQ is for prospective DBEs to submit materials establishing qualifications to provide the work, labor, materials and services necessary to complete design and construction of the Project. This RFQ is the first phase of a two-phase competitive process for the District’s selection of a DBE to complete design and construction of the Project. The RFQ phase consists of a screening and ranking process to establish a “short-list” of pre-qualified DBEs who will be invited to participate in the Request for Proposals (“RFP”) phase of the DBE selection process. Timely submitted and responsive RFQ Responses will be reviewed and scored by a Selection Committee based on the scoring criteria established in this RFQ. Following the Selection Committee’s determination of which Respondents meet or exceed the qualifications standards set forth in this RFQ, in the sole and exclusive discretion of the Selection Committee, the three (3) DBEs with the highest scored RFQ responses will be invited by the District to the RFP Phase.

1.1.2. Design-Build Approach. For this Project, the District is utilizing a Best Value, Guaranteed Maximum Not to Exceed, design-build procurement method under the California Education Code §81703. Pursuant to Education Code §81700 et seq., the District intends to contract with a DBE to complete the Design Documents, obtain all necessary permits/approvals for construction of the Project, and construct the Project.

1.2. Obtaining RFQ. This RFQ will be made available on the Chabot-Las Positas Community College Facilities website at http://www.clpccd.org/bond/BiddingProjectBusinessOpportunitiesRFB.php, labeled as - RFQ No. K-19, New Baseball Field Project, Chabot College.

1.3. District RFQ Contact. Questions or other communications relating to this RFQ shall be directed in writing to the District at:

The District shall not be obligated to answer any questions received after the specified deadline or any questions submitted in a manner other than instructed above.

1.4. District Modifications to RFQ. The District expressly reserves the right to modify any portion

of this RFQ prior to the latest date/time for submission of RFQ Responses, including without limitation, the cancellation of this RFQ. Modifications, if any, made by the District to the RFQ will be in writing; potential Respondents who have obtained this RFQ from the District prior to any such modifications will be issued modifications to the RFQ by written addenda. All addenda issued by or on behalf of the District shall become part of the RFQ. Addenda will be distributed only to the Respondents who have obtained the RFQ from the District’s RFQ Contract pursuant to Paragraph 1.2 above.

1.5. No Oral Clarifications/Modifications. The District will not provide any oral clarifications or modifications to the RFQ or the requirements hereof; no employee, officer, agent or representative of the District is authorized to provide oral clarifications or modifications to the RFQ. No Respondent shall rely on any oral clarification or modification to the RFQ.

1.6. Unauthorized Communications. Unless and except when requested to do so in writing by the District or as otherwise permitted by this RFQ, Respondents (including, without limitation, members of the DBE, Sub-Consultants and Subcontractors) shall not, prior to completion of the RFQ, communicate, either verbally or in writing, with: (i) any member of the RFQ Selection Committee; (ii) any consultant or professional retained by the District for the purpose of providing the District advice or professional services in respect to the Project or the RFQ; or (iii) any employee or representative of the Chabot-Las Positas Community College District.

1.7. Project References: The District reserves the right, but assumes no obligation, to contact and interview any persons or organizations identified in Applicant’s Pre-Qualification Submittal, including, without limitation, Project References. Applicant assumes sole responsibility for any inaccuracies in the contact information provided by it. Incorrect contact Information for a Project Reference provided in response to a request in the Pre-Qualification Questionnaire may, in the sole and absolute discretion of District, result in the Project Reference being stricken.

1.8. Public Records. All materials submitted in response to the RFQ are deemed property of the District upon submission to the District. Responses to the RFQ and other materials submitted in connection therewith shall be deemed “public records” except for those marked or noted by a Respondent as “trade secrets” (as that term is defined and used in Civil Code §3426.1), “confidential” or “proprietary.” Financial statements are not subject to disclosure under the Public Records Act. The foregoing notwithstanding, the District may reject for non-responsiveness a Response to the RFQ which indiscriminately notes that the Response or portions thereof are “Trade Secret,” “Confidential,” or “Proprietary” and exempt from disclosure as a public record. Notwithstanding the designation of materials submitted in response to the RFQ as “public records” such materials shall not be subject to disclosure under the Public Records Act until after: (i) conclusion of negotiations between the District and Respondent(s) concerning RFQ Responses and/or the terms, conditions and covenants of the DBE Contract; and (ii) a decision is made as to the Respondent to whom District staff will recommend to the District’s Board of Trustees for award of the DBE Contract. The District is not liable or responsible for the disclosure of RFQ Responses, or portions thereof, deemed to be public records, including those exempt from disclosure if disclosure is required by law, by an order of a court of competent jurisdiction, or which occurs through inadvertence, mistake or negligence on the part of the District or its agents or representatives. If the District is required to defend or otherwise respond to any action or proceeding wherein request is made for the disclosure of the contents of any portion of a RFQ Response deemed exempt from disclosure hereunder, by submitting a RFQ Response, each Respondent agrees to defend, indemnify and hold harmless the District in any action or proceeding from and against any liability, including without limitation attorneys’ fees arising therefrom. The party submitting materials sought by any other party shall be solely responsible for the cost and defense in any action or proceeding seeking to compel disclosure of such materials; the District’s sole

involvement in any such action shall be that of a disinterested stakeholder, retaining the requested materials until otherwise ordered by a court of competent jurisdiction.

1.9. Errors/Discrepancies/Clarifications to RFQ. If a Respondent encounters errors or discrepancies in this RFQ or portions hereof, the Respondent shall immediately notify the District RFQ Contact of such error or discrepancy. Any Respondent seeking clarification of any portion of this RFQ shall submit the requested clarification in writing to the District RFQ Contact. Responses of the District to any requested clarification will be in writing; if in the sole judgment of the District, any clarification response affects the RFQ or other Respondents, the District will issue the clarification response by a written addendum distributed to all potential Respondents who have theretofore obtained this RFQ from the District. All requests for clarification of this RFQ must be submitted and actually received by the District no later than 4:00 PM ON TUESDAY, FEBRUARY 19, 2019; the District will not respond to clarification requests submitted thereafter. All communications to the District shall be as set forth in Paragraph 1.3 above.

1.10. Continuing Obligation. Each Respondent is under a continuing obligation, commencing upon submission of its RFQ Response, to immediately notify the District, in writing, if it learns that any of the following have occurred: (i) any statement made in any portion of its RFQ Response was false, misleading or omits material facts rendering any statement in its RFQ Response to be false or misleading; (ii) circumstances have occurred since the Respondent submitted its RFQ Response that, if they had occurred prior to the date that the Respondent submitted its RFQ Response, would have required the disclosure of such circumstances; or (iii) the Respondent has undergone a change in ownership in which ownership of fifty percent (50%) of more of its stock, ownership equity or assets has changed.

1.11. No Commitment to Award DBE Contract; District Reservation of Rights. Issuance of this RFQ and the District’s receipt or review of RFQ Responses does not commit the District to award of the DBE Contract. The District expressly reserves the right to modify the due date for RFQ Responses, RFQ requirements or action to award the DBE Contract in the sole and exclusive discretion of the District. Responses to the Design-Build Services Pre-Qualification Application (“Pre-Qualification Questionnaire”), Attachment A to this RFQ, including information about current bonding capacity, notarized statement from surety, and the most recent reviewed or audited financial statements, with accompanying notes and supplemental information, are required. The District will use these materials as the basis of rating the Respondents. The District reserves the right to check other sources available. The District’s decision will be based on objective evaluation criteria. The District reserves the right to waive minor irregularities in the RFQ Responses.

1.12. Notice of Appeal and Hearing. A Respondent who timely submits an RFQ Response that is not one (1) of the three (3) highest scored RFQ Responses may appeal its RFQ Response score (“RFQ Appeal”) by strict compliance with the following. RFQ Appeals must be in writing and must be submitted to the District RFQ Contact identified in Paragraph 1.2 above by 4:00 PM on the fifth (5th) calendar day after the date of the District’s notice to such Respondent of its failure to achieve the “short-listed” status. The RFQ Appeal must include: (i) all factual basis for the RFQ Appeal; (ii) all evidentiary materials supporting the factual basis for the RFQ Appeal; and (iii) all legal authority supporting the RFQ Appeal. If a Respondent timely submits a RFQ Appeal strictly conforming to the foregoing, the District will schedule a RFQ Appeal Hearing within ten (10) days of the date of submittal of a RFQ Appeal; the District will endeavor to schedule the RFQ Appeal Hearing upon mutual agreement with the Respondent submitting the RFQ Appeal, but the District expressly reserves the right to unilaterally schedule the RFQ Appeal Hearing. The RFQ Appeal Hearing is an informal process conducted by a panel of District employees or District consultants authorized by the District to conduct the RFQ Appeal Hearing and to render a decision on behalf of the District to a RFQ Appeal. The Respondent submitting a RFQ Appeal will be afforded the opportunity to present

witnesses and evidentiary materials supporting the RFQ Appeal. Witnesses presented by the Respondent submitting a RFQ Response will be required to testify under oath and are subject to cross-examination by the Appeal Panel. The Appeal Panel will issue rulings on RFQ Appeals in writing (“RFQ Appeal Decision”) within three (3) business days of the conclusion of a RFQ Appeals Hearing. RFQ Appeal Decisions constitute the District’s final administrative action a RFQ Appeal and not subject to further administrative review.

1.13. No Warranty by District. Respondents are solely responsible to satisfy themselves as to the suitability of any information provided by the District that is in the nature of estimates of costs, statements of needs or requirements, projections, budgets, or other information describing the proposed DBE Contract or Project contemplated by this RFQ. Nothing stated in this RFQ or any portion hereof shall be construed as implying the creation or existence of any warranty, express or implied, on the part of the District with respect to the accuracy, sufficiency or completeness of such information.

1.14. Informational Summaries. Portions of these RFQ Instructions include summaries of the process that the District currently contemplates for completion of the RFQ process. Such summaries are provided for the convenience of the Respondents and cannot be interpreted as a complete or definitive statement of all procedures, conditions, requirements or standards that may apply to any of the aforementioned processes. The District reserves the right, at any time and in the exercise of its sole and absolute discretion, to modify such procedures, conditions, requirements or standards, by changes, additions or deletions thereto.

1.15. RFQ and RFP Response Costs. All costs and expenses incurred by a Respondent to prepare and submit a response to this RFQ shall be borne solely and exclusively by the Respondent.

1.15.1. A Stipend will be paid to the short listed forms per the following:

First Place – Award of Contract for Design/Build Services

Second and Third Place - $10,000, that may be awarded by the District, in its sole discretion, to each responsive Design Build team that places second or third.

3. The District and the Project.

3.1. The District. The Chabot-Las Positas Community College District was founded in 1961 and serves the San Francisco East Bay Area, particularly southern Alameda County, through its two colleges: Chabot College in Hayward and Las Positas College in Livermore. The colleges specialize in university transfer, technical training, continuing education, workforce development, contract education with local businesses and cultural enrichment. The district serves 24,500 students and employs more than 2,000 administrators, faculty and classified staff.

The Chabot College Athletic Department offers 16 intercollegiate sports for men and women, and Chabot Athletics prides itself in developing student-athletes to achieve skills in the pursuit of athletic competition and to excel in their academic pursuits. By participating in athletics, students can grow mentally, physically and to develop teamwork skills. Through quality

coaching and dedication, student-athletes achieve a successful transfer rate to four-year institutions where they can continue their academic and athletic careers.

A seven-member elected Board of Trustees sets policy for the District. The registered voters of the nine communities elect them from the following trustee areas: Castro Valley, Dublin, Hayward, Livermore, Pleasanton, San Leandro, San Lorenzo, Sunol, and Union City. The District has undertaken a major capital building program to implement the $950 million Measure A Bond Program approved by the voters in June 2016. The overriding goal of this program will be for construction, repair, modernization and expansion efforts are to expand the existing college campuses to meet the changing needs of students and the community.

3.2. Project Description. This Design Build Project constructs a new permanent state-of-the-art, baseball field in the present location of the existing baseball field. It is anticipated that the current baseball field orientation will be retained for an optimum playing experience. This is anticipated to be a year-round training and playing field and will promote the Chabot College Gladiator identity and strength.

The Project will consist of approximately:

Demolition of the existing Press box building, Three (3) Conex containers, existing tiered seating adjacent to the home and visitor infields, backstop netting with poles and foundations, existing storage rooms and restrooms, existing home and visitor dugouts, irrigation and site utilities, scoreboard signage and foundations, bull-pens, batting cages and foundations, hardscapes, and poles and fences in the backfield.

Baseball Field Site:

• Total site area is approximately 197,110 GSF

• 134,128 GSF of baseball playing field, dugouts, bull pen and outfield.

• 4,950 GSF Three (3) lane batting cage

Playing Field Structures:

It is anticipated that any permanent structures built would closely follow the material, color and texture precedent set by the structures at the Chabot College Women’s Softball Field.

• 900 GSF single story fully conditioned Press box building with the following program –

o Press box

o Tiered stadium style seating

o (2) public restrooms

o Custodial

o Electrical

o Telecom

• 500 GSF Single-story Storage building.

• (4) lane Home Bullpen and (2) lane Visitor Bullpen, both sized to NCAA dimensions

• (3) lane Batting Cage.

• (2) at grade Home and Visitor Dugouts.

• The existing berms adjacent to the home and visitor infields will be regraded and the existing cast in place concrete seating with aluminum benches will be demolished.

• Backstop, netting, and poles positioned to optimize spectator experience.

• New LED Scoreboard with static display.

• Fencing and gates to secure the baseball field facility.

• Outdoor storage areas for large movable equipment.

• Landscaping, walkways, and decorative concrete entry plaza with ornamental fencing

The District has identified the following Additive Alternates to the New Baseball Field Project, Chabot College:

• Add Alternate # 1

Addition of a fourth (4th) batting cage lane to the new Batting Cage facility

• Add Alternate # 2 –

Addition of a paved flat seating area with good spectator views at the berm adjacent to the Home infield. Include stairs and accessible ramp as required.

• Add Alternate # 3A –

Addition of approximately 1,767 GSF consisting of Team room and Coaches office to the Storage Building.

• Add Alternate # 3 B –

Addition of approximately 430 GSF consisting of Training room, Concessions, and Gender-neutral restroom to the Press Box Building.

The above square footages are current estimates and subject to change.

3.2.1. Project Budget. The District established an initial “Project Budget,” for the DBE’s costs for completing the Project Design Documents and construction of the project, at Five Million Eight Hundred Thousand Dollars ($5,800,000). The Project Budget is not inclusive of Owner’s Contingencies. See Attachment F for the Project Budget detail.

Project Schedule. The District anticipates awarding the Project to the successful Respondent no later than June 2019. The District anticipates the release and construction of the new baseball field and facilities beginning May 2020. The District understands the facilities will be unavailable for a baseball season which they can accommodate the athletic teams for a limited duration. The District plans to have a new baseball field and facility by March 2021. See attachment E for more details.

3.2.2. Design Build Criteria Documents Team. The District has retained HMC Architects (“the Consulting Architect”) to prepare the Performance (Criteria) Documents and to assist the District during the DBEs completion of Project design and construction. Information regarding Design Consultants is required to be submitted in Attachment A of the RFQ. Please note that the Consulting Architect preparing the Criteria documents, including the design sub-consultants listed below, are not eligible to be included on any design build entity teams for this Project:

(Electrical Engineer) • Charles Salters and Associates, Inc. (Audio-Visual and IT) • Kate Keating Associates, Inc. (Wayfinding and Signage) • Marcene Taylor, Inc. (Cost Estimation)

3.2.3. Bridging Documents. The Consulting Architect has prepared performance criteria documents and programming that includes the District’s standards for materials and equipment for this Project. These documents will be provided to the selected three (3) qualified DBEs as part of the RFP process. These documents will provide guidelines as to Space Programming with Adjacencies, which may be a “test fit” schematic floor and site plan drawing format. A geotechnical study has been completed for the site, and an existing site utilities plan will be a part of the Bridging Documents.

4. Entity Qualifications. The DBE will be an integral member of the Project Team, consisting of the DBE, District representatives, and other consultants as required. It will be the responsibility of the DBE to integrate the design and construction phases, utilizing skills and knowledge of design and general contracting to provide design, code analysis, value engineering and constructability reviews, develop schedules, prepare detailed project construction estimates, and, in any way deemed necessary, contribute to the development of the Project during the pre-construction/design phase. The DBE will have the following characteristics:

• Include a team member with a California General Building Contractor License, “B” classification, and a Public Works Contractor Registration through the Department of Industrial Relations.

• Have successful experience completing projects for Community College Districts and Higher Education Districts.

• Have successful experiences completing collegiate athletic facilities and fields where the fields were constructed with synthetic turf.

• Provide adequate staffing and resources to Project(s). Maintain key staff through the duration of Project(s).

• Have reasonable proximity to the City of Hayward in order to provide: (1) team working/review sessions during design; (2) ease of team coordination meetings; (3) minimal cost to attend District meetings; and (4) quick response to field issues.

• Have an established Quality Assurance/Quality Control program.

• Work collaboratively with District and District’s Authorized Representative.

• Be flexible in nature, with positive, can-do attitudes. Be solution-oriented; deadline driven.

• Be organized, effective and efficient. Exercise professional prioritization skills. Work easily with modern amenities like scanners and email.

• Be proactive. Be prepared, working in advance of deadlines. Be comfortable making in-field decisions, problem solving with engineers or experts via phone during critical times of construction.

• Be mindful and tenacious about maintaining project budgets and schedules, especially during construction. Make decisions that support District goals and objectives, relative to scope creep and especially contractor-initiated change orders.

• Articulately communicate potential issues early and often. Provide thoughtful recommendations where applicable.

5. RFQ Response.

5.1. RFQ Activities; Timeline. The following is a description of the principal activities to be completed under this RFQ and the timeline for completion of each activity. The following notwithstanding, the District expressly reserves the right to modify the extent, nature or scope of RFQ activities and/or the timeline for commencing/completing RFQ activities by issuance of

5.2. RFQ Responses.

5.2.1. Latest Date/Time for Submission of RFQ Response. The latest date/time for submission of RFQ Responses is set forth in Paragraph 5.1. RFQ Responses which are not actually received by the office of the Bond Buyer, Facilities/Bond Programs and Operations Department at or prior to the latest date/time for submission of RFQ Responses may be rejected by the District for non-responsiveness. 5.2.2. Submission of RFQ Response. RFQ Responses shall be enclosed and sealed in an envelope or container bearing the title of the Project, RFQ No. K-19, and the name of the Respondent.

EMAIL OR FAXED REQUEST FOR QUALIFICATIONS WILL NOT BE ACCEPTED. It is the Respondent’s responsibility to verify that the Respondent’s RFQ Response has been received by the District prior to the latest date/time for submissions. Verification of receipt can be made emailing [email protected].

5.3. Respondent Questions; Clarification Requests. Respondents’ questions or clarification requests regarding this RFQ must be in writing to the Project Manager, Facilities/Bond Program, and must be received by the District RFQ Contact no later than 4:00 P.M. (PST) February 19, 2019. Written questions may be emailed to the District RFQ Contact [email protected] . The project title, RFQ K-19, must be referenced in the subject line of emailed questions. The District shall not be obligated to answer any questions received after the above-specified deadline or any questions submitted in a manner other than as set forth

above. The District will respond to timely submitted questions in writing. The District’s response(s) to a Respondent’s question(s) will be in the form of an addendum. The District will issue an addendum which will be emailed to all participants and posted on the District website under the applicable project name and number.

5.4. RFQ Response Contents/Format.

5.4.1. RFQ Response. All materials submitted in response to this RFQ shall be on 8 ½” x 11” paper, preferably in portrait orientation, 12 point (or larger) Arial, Calibri or Times New Roman font with 1” page margins on all sides. Mistakes may be crossed out and corrections made adjacent, however, each correction must be initialed by the person signing the Design-Build Services Pre-Qualification Application. All submitted materials must be bound in either a three-ring binder or spiral bound notebook. Tabbed dividers should be used to identify and separate discrete sections of the RFQ Response which correspond to the RFQ Response Format described in Paragraph 5.5 below.

The total submittal length shall not exceed thirty-five (35), one-sided pages. 5.4.2. Additional Materials. Respondents are not prohibited, but are discouraged from

submitting materials in addition to those specifically responding to the matters noted in Paragraph 5.5 below. If a Respondent elects to submit materials with its RFQ Response which are in addition to the matters described in Paragraph 5.5 below, the Respondent shall separately bind all such additional materials from the RFQ Response addressing the matters set forth in Paragraph 5.5.

5.4.3. Copies of RFQ Response. Each Respondent shall submit one (1) complete electronic version on a flash drive, one (1) original and three (3) copies of their respective RFQ Responses. The electronic copy shall be in PDF or read-only Word document.

5.5. RFQ Response Format/Contents. Each RFQ Response must conform to the following described format and must include the content described below. Failure of a Respondent to submit its RFQ Response in a format and with content conforming to the following requirements will be a basis for the District’s rejection of such RFQ Response for non-responsiveness.

5.5.1. Cover Sheet. Identify the submittal utilizing a Cover Sheet stating the following, “Statement of Qualifications for [FIRM NAME] in Response to Chabot-Las Positas RFQ #K-19” and an identification of the DBE submitting the RFQ Response along with the DBE’s address, telephone/fax numbers, and email addresses of the DBE’s principal contacts in connection with this RFQ or the RFQ Response.

• Acknowledgement of Addendum: The Bidder confirms that this Bid Proposal incorporates and is inclusive of, all items or other matters contained in Bid Addenda issued by or on behalf of the District. Locate as the final statement on the Cover Sheet the acknowledgement of addendum using the following language:

Provide contact information, including the telephone number, fax number and email address for the personnel of the Respondent who will be receiving notices and other communications from the District regarding the RFQ. 5.5.3. Table of Contents. Include a Table of Contents reflecting each of the following sections of the RFQ Response:

• Tab 1: Pre-Qualification Application Questionnaire. Each Respondent must complete the Design-Build Services Pre-Qualification Application (“Pre-Qualification Questionnaire”), Attachment A to this RFQ. The District’s Selection Committee will evaluate and score each Pre-Qualification Questionnaire for completeness; conformance to project requirements; history of violations, claims, arbitration and/or litigation; and other requirements. Pre-Qualification Applications must be signed under penalty of perjury in the manner designated at the end of the Pre-Qualification Application, by an individual who has the legal authority to bind the Respondent on whose behalf that person is signing. If any information provided by a Respondent becomes inaccurate, the Respondent must immediately notify the District and provide updated accurate information in writing, under penalty of perjury. (This section does not affect page limit of the RFQ Response.)

• Tab 2: Relevant Project Experience. Provide additional information on the design and construction profiles of the projects identified in the Pre-Qualification Application. Clearly identify the relevance of each project and address relevance to the proposed Project.

The projects identified should demonstrate the DBE’s design and construction experience in projects of similar size and scope, and complexity as the proposed Project, should have been completed in the education construction sector preferably in higher education however K-12 will be acceptable, completed within the past (10) ten years, and had success with the Division of State Architects.

Include specific experiences in delivering projects under a design-build project delivery method. Include in the discussion of the experience of the Respondent and its proposed Architect of Record (“AOR”) and Design Consultants to the AOR for the following: (i) conformity to schedule and budget requirements; (ii) narrative of design approach and construction experience (iii) if the Respondent and proposed AOR worked on the same project, (iv) utilization of value engineering and cost control techniques, (v) salient features and discuss how the client’s program, mission, and operational objects were satisfied by the overall design and planning solution.

• Tab 3: Proposed Project Team. Identify the key members of the Respondents’ staff that will be assigned to the design and construction phases of the Project. Identify the Respondents’ proposed AOR and General Contractor, as well as a Mechanical Engineer, Structural Engineer, Electrical Engineer, Civil Engineer, Landscape Architect, and Sports Field Design Consultants to the AOR and/or DBE.

For the AOR and General Contractor, and each Design Consultant identified by the Respondent, identify the key members of their staff that will be assigned to the design or construction phases of the Project. Identify each person proposed as part of the Design Team and indicate whether such proposed personnel are employees of the Respondent, an employee of a Sub-Consultant to the Respondent or an independent contractor to the Respondent. For each member of the Respondents’ organization and for each member of AOR and Design

Consultants identified, provide brief descriptions of the relevant qualifications, including DBIA Certified Professional certificates, background, and any project team members that have previously worked as members of an integrated team.

Include a matrix indicating which proposed personnel have worked together before on the indicated Relevant Projects Experience section. Include an organization chart illustrating the relationships among the Respondent’s proposed Project Team.

Do not include resumes for the proposed personnel of the Respondent, AOR and proposed Design Consultants under Tab 3; personnel resumes are to be bound and incorporated into Tab 4 (Personnel Resumes) section of the RFQ Response.

• Tab 4: Project Personnel Resumes. Provide a current resume for each member of the Respondent’s personnel and for each member of the personnel of the AOR and each Design Consultant identified in Tab 3, Proposed Project Team. (This section does not affect page limit of the RFQ Response.)

• Tab 5: Project Approach. Provide a narrative of the Respondent’s approach and methodology to completing tasks necessary to complete the design-construct of the Project set forth in Section 3.2 Project Description and as shown in Attachment D. Include a detailed description and discussion of measures the Respondent utilizes for quality assurance and quality control of Design Documents, for minimizing errors or omissions, and for completing construction of the Project within the District’s schedule and budget. Include any control systems for planning, scheduling, and cost management.

Discuss the Respondent’s philosophy and design intent as related to the project. Include topics as the parameters that may apply in creating a high-performing and best value baseball field and associated athletics facilities. Discuss the Respondent’s management process that will be applied to this Project and the approach to design build. Include how the Respondent will manage communication, decision-making, interaction with sub-consultants and District’s representatives, and means to integrate client and community input.

• Tab 6: Financial Statement. Respondent to include a current financial statement for 2018 CY or 2017/2018 FY (This section does not affect page limit of the RFQ Response.)

• Tab 7: Insurance Certificates. Provide copies of Certificates of Insurance for the Respondent and its proposed AOR and Design Consultants. Required Certificates of Insurance and minimum coverage amounts for each policy of insurance are as set forth below. The District reserves the right to modify the minimum coverage limits for each policy of insurance on the Project. (This section does not affect page limit of the RFQ Response.) B. Automobile Liability: One million dollars ($1,000,000) per accident for bodily injury and property damage.

Professional Liability: Architect/Engineer shall provide insurance covering the prime design professional and his/her subconsultant(s) for one million dollars ($1,000,000) aggregate limit subject to no more than seventy-five thousand dollars ($75,000) per claim deductible, coverage to continue through substantial completion of construction plus two years thereafter.

E. Excess/Umbrella Liability: Three million dollars ($3,000,000) per occurrence.

Other Insurance Provisions: The general liability and automobile liability policies are to contain, or be endorsed to contain, the following provisions: 1. The District, the Construction Manager, their representatives, consultants, trustees, officers, officials, employees, agents, and volunteers (“Additional Insureds”) are to be covered as additional insureds as respects liability arising out of activities performed by or on behalf of the Architect/Engineer; instruments of service and completed operations of the Architect/Engineer; premises owned, occupied or used by the Architect/Engineer; or automobiles owned, leased, hired or borrowed by the Architect/Engineer. The coverage shall contain no special limitations on the scope of protection afforded to the Additional Insureds.

• Tab 8: Safety Record. Include submission of evidence that establishes the DBE has an acceptable safety record. For purposes of this RFQ, an “acceptable safety record” is a Workers Compensation Insurance experience (EMR) for the most recent three (3) year period that is an average of 1.0 or less, and an average total recordable injury or illness rate and average lost work rate for the most three (3) year period does not exceed the applicable statistical standards for the construction industry. If the DBE’s EMR for the most recent three (3) year period exceeds an average of 1.0, the DBEs acceptable to the District will be scored “0”.

• Tab 9: Geographic Location of Home Office. Include submission of evidence that

establishes that DBE has a current home office nearest the Chabot College campus location, ideally in either Contra Costa County, Alameda County, or Santa Clara County although it is not required to have an office in these counties. For Respondents who conduct business from multiple office locations, the “home office” for purposes of this RFQ is the Respondent’s regional office serving the San Francisco/Hayward/San Jose geographic area.

6. RFQ Evaluations.

6.1. General. Timely submitted RFQ Responses will be reviewed for responsiveness and independently evaluated by each member of the Selection Committee. A Response to the RFQ which does not comply with the requirements of this RFQ will be subject to rejection for non-responsiveness.

6.2. District Policy. It is the policy of the District that the selection of firms to provide professional services in connection with construction projects of the District be based on the demonstrated competence and qualifications to complete the required professional services at a fair and reasonable price to the District.

6.3. Initial Evaluation Criteria; Short List. The following sets forth the criteria by which each RFQ Response will be evaluated and the relative weight of each evaluation criteria.

6.3.1. Pre-Qualification Application. DBEs must submit the Pre-Qualification Application as described in Design-Build Services Qualification Application attached as Exhibit A to this RFQ for an Architect and General Contractor, as well as a Mechanical Engineer, Structural Engineer, Electrical Engineer, Civil Engineer, Landscape Architect, and Sports Field Designer. The Selection Committee will evaluate for completeness; conformance to project requirements; history of violations, claim, arbitration and/or litigation and other requirements.

6.3.2. Relevant Experience and Ability. The initial standard is met when the Respondent provides at least (3) projects that address the requested submittal requirements Respondent’s profiles of relevant projects will be evaluated based on how similar in size, scope, use, and complexity the projects are to the proposed Project subject to this RFQ. The relevant experience and ability will be evaluated in accordance with the following criteria

• Completed for a California Community College District,

• Successful experience with DSA,

• Demonstrated Respondent involvement in the planning, design, and construction administration of sports fields and sports facilities within the last ten (10) years,

• Completed on time and within budget,

• Demonstrated high relevance to the proposed Project.

• Successfully utilized value engineering for the benefit of the Owner and implemented construction cost reduction measures,

• Were successfully delivered by design build method,

• Obtained certificate, award, or recognition demonstrating design and/or construction excellence.

6.3.3. Proposed Project Team. The skills, experience, and capabilities of the respondent and its proposed Project Team will be evaluated. The standard is met if the Respondent identified 1) Key members of the Respondents’ staff that will be

assigned to design and construction phases of the Projects identified; and the Respondent identified 2) Architect, Mechanical Engineer, Structural Engineer, Electrical Engineer, Civil Engineer, Landscape Architect, and Sports Field Designer. Favorable consideration may be given if the Respondent included 1) brief descriptions of the relevant qualifications and background 2) a matrix of any like projects team members has previously worked as members of an integrated team; 3) and an organization chart illustrating the relationships among the Respondent’s proposed Project Team. Further favorable considerations may be given if the Proposed Project Team achieves any of the criteria set forth:

• The Design Team has a track record of delivering superior quality and demonstrated consistently high level of exploration, rigor, and personal commitment to an excellent design.

• The Design Team demonstrated a history of dedication to clients.

• Ability to develop effective working relationships with Owners, Owner’s Representative, key management personnel and other consultants.

• Acceptable and verifiable references from clients contacted by the District, including Key members’ firms reputation, satisfaction of previous clients, timeliness of work and adherence to client’s schedule.

6.3.4. Project Personnel Resumes. Current resume for each member of the Respondent’s personnel and for each member of the personnel of the Architect and each Design Consultants provided.

6.3.5. Project Approach. The Respondent provided a narrative discussing their ability and commitment to develop and implement high-quality, functional, cost-effective and aesthetically attractive designs for the Project anticipated to be completed under this RFQ. The standard is met if the Respondent demonstrated their philosophy and design intent is suitable for this project and there is satisfactory probability that the Respondent’s management process will result in a collaborative team effort. Further favorable considerations may be given if the Respondent achieves any of the criteria set forth:

• Clear demonstration and defined compatible values and philosophy,

• Proven and successful implementation of the Respondent’s management process on any of the projects listed under Relevant Experience and Ability,

• A patented process or unique product which the Respondent intends to rely on in performing the project which adds additional value to the project.

6.3.6. Financial Statement. Provide current financial statement.

6.3.7. Insurance Certificates. Copies of Certificates of Insurance and minimum coverage amounts for each policy of insurance as set forth above in Section 5.5.3.7 for the Respondent and its proposed Architect and Design Consultants.

6.3.8. Safety Record. Evidence of an acceptable safety record of an average of 1.0 or less experience modification rating (EMR) for the most recent three (3) year period is provided.

About the District

The Chabot-Las Positas Community College District was founded in

1961 and serves the San Francisco East Bay Area, particularly southern

Alameda County, through its two colleges: Chabot College in Hayward

and Las Positas College in Livermore. The colleges specialize in

university transfer, technical training, continuing education, workforce

development, contract education with local businesses and cultural

enrichment. The district serves 24,500 students and employs more than

2,000 administrators, faculty and classified staff. The Chabot College

Athletic Department offers 16 intercollegiate sports for men and women,

and Chabot Athletics prides itself in developing student-athletes to

achieve skills in the pursuit of athletic competition and to excel in their

academic pursuits. By participating in athletics, students can grow

mentally, physically and to develop teamwork skills. Through quality

coaching and dedication, student-athletes achieve a successful transfer

rate to four-year institutions where they can continue their academic and

athletic careers.

Project Description

Chabot College intends to build a new permanent state of the art

Baseball Field through a Design Build project delivery process. It

is the intent of Chabot College that this new facility serves as a

reflection of the pride and quality its student athletes and coaches

are known for and which will strengthen the Chabot College Gladiator

identity and brand.

The College’s Vision of the Baseball Field includes the following:

•A Facility that students are proud of and where they can hone

their craft

•A Facility that student athletes desire to be part of

•Recognition as a comfortable venue to view and enjoy quality

baseball games

To which end, the new baseball field will be a good pitching and

batting facility and provide a great fan experience with good sightlines

and comfortable stadium style seats.

In the existing facility, student athletes are unable to play nor practice

year round because of poor field drainage and exposed practice

facilities. Students have to drive several miles to other facilities

that the college rents to practice or play when there is inclement

weather. Therefore in support of the goals and visions stated above, this

new Baseball Field will be an all-weather, year-round facility with improved

connectivity and accessibility with the rest of the campus.

Some of the features the New Baseball Field will have includes:

•New Press box Building with improved sightlines and media

support

•Tiered Stadium style seats

•New Dirt Pitcher’s Mound with padded backstop and improved

netting

•New Three (3) lane All Weather Batting Cages

•New Restrooms

•New Fully Accessible and larger Dugouts

•All New Turf in the Playing Field, Bull Pens, Outfield with

improved drainage

•All New Batters Eye

•New LED Scoreboard with video support

•New Entry Plaza, Walk of Fame and with improved security for the

field

This new Baseball Field represents a commitment on the part of Chabot

College to its student athletes, coaches and the community that supports

it.

This Design Build Project constructs a new permanent state-of-the-art,

Baseball Field in the present location of the existing baseball field. It is

anticipated that the current baseball field orientation will be retained for

an optimum playing experience. This is anticipated to be a year round

training and playing field and will promote the Chabot College Gladiator

identity.

The Project will consist of approximately:

Demolition of the existing Press box building, (3) Conex containers,

existing tiered seating adjacent to the home and visitor infields, existing

concessions building, and poles and fences in the backfield.

The main goals for the New Basebal field master plan are:

The new baseball field should be playable year round

-The district has decided to build a Full synthetic turf field and foul

areas. The pitcher’s mound will be the only playing field element

that will contain clay.

-The field will be fully fenced and secure. While no field lighting is

foreseen in the program, an LED scoreboard able to support video

is part of the scope of the work.

The field and supporting facilities are geared toward athlete support and

development

-The backside of the pitcher’s mound in the field should be

fitted with a quick coupler and power to support technical analysis

and development.

-The home bullpen and batting cages will be full synthetic turf

-New Plyometric wall at the batting cage.

-Dugouts have storage and sanitary facility to accommodate

athlete needs.

-If add alternate 4A is accepted Team room will be located for

athlete comfort and convenience.

- If add alternate 4B is accepted, training room will be located

adjacent to New field to provide immediate

treatment if needed.

New baseball field should create “Fan Experience” that promotes

attendance and support.

- Tiered stadium seating is provided directly adjacent to field

behind the backstop, home and visitor’s dugout.

-Improved views are offered from new viewing berms outfitted

with ramp and stair access for spectator comfort.

-New restroom facilities and concession stand

-Pre-function plazas in the site

Emphasize and promoted the Gladiator Identity

-Integrated turf logo in the outfield

-New Gateway at the entrance to the new facilities

-College branding at Batting cage plyometric wall and other

opportunities the DB Team identifies.

Connect to the campus

-The architecture of the baseball field shall be harmonious with the

campus’s existing conditions and guidelines set forth in the 2012

Campus Masterplan

-New Paths and plazas will be a natural extension of existing

campus circulation

-New landscape will create diverse visuals that merge with the

existing groves
