CFO Solutions has created a streamlined hospital-specific solution for budgeting and planning using Infor’s EPM suite. The Core Healthcare Planning Content Package for Lawson Clients module is the foundation for this planning solution. Hospitals may purchase additional modules to meet their specific unique needs at fixed costs with pre-defined services and assumptions leveraging CFO Solutions’ vast healthcare experience. Hospitals with planning needs not described within this scope of services or Hospitals whose needs are not addressed within the modules may request a custom estimate of services designed around their specific needs. An implementation scoping call would be required in such a scenario. Note, all prices are estimates based on CFO Solutions’ extensive experience with the products and within the space. Price estimates do not include travel expenses. 1. CFO Solutions’ Core Healthcare Planning Content Package for Lawson Clients - $49,211 Healthcare organizations who are seeking a planning solution utilizing Infor’s EPM suite may select this module as the basis for their application. This component contains the basic features to enable core planning functionality, be it in annual budgets, re-forecasts, and/or long term plans. Features:

• High-level design to aid in future use of the model • Application installation and core configuration • Facility Department/Account Expense and Revenue Planning • Approval Workflow • Integration with the Lawson G/L

o Account and Department Dimensions based on the Lawson structure o P&L and Balance sheet structures built for both budgeting and reporting purposes

Budget Input Templates:

• EPM utilizes the concepts of Planning Cycles (i.e. Annual Budget), Financial Planning Steps (i.e. Departmental Operating Budget Entry) and Workflow (i.e. Jon Doe submits department Y's budget to Jane Smith for approval) to enable administrators to easily create or reuse planning processes as the need arises. This enables the functional administrator at a client to configure a nearly unlimited number of budgeting templates.

o The consultant will provide up to 4 total planning steps with 10 total users in each. The client can add to this as need be

CFO Solutions' Healthcare Planning Content Package for Physicians' Groups - $39,440 Hospitals and Physicians’ Groups that plan at an individual Physician-level may choose to add the Physician Revenue Planning Module, which includes planning by physician for inpatient and outpatient revenue such that Net Patient Service Revenue is quickly and easily calculated.

The following is included within the Physician Revenue Planning Module:

• Creation of a custom Physician Hierarchy • Collection by Detail Charge level with Detail Charges and Time as members • Collection by location level with the hospital’s locations as members • Driver-based Price/Volume, RVU, work RVU, Payer Mix and Collection Rate elements • Unique security to limit access • Mapping/Data Transfer to Core CPM Module • Option of 4 different options for end user screens • Data import process from a single source system (typically not the G/L)

Configuration of this module includes security configuration, mapping detailed HR data to GL accounts, and 5 user-defined employee attributes that drive costs. Purely driver-based employee expense planning (i.e. productivity optimization) is not included within the scope of this module. See the Advanced Workforce Planning module for this functionality. The solution works with multiple data sources, including Infor Lawson Enterprise Financial Management and Infor Lawson Supply Chain Management, so it can grow along with you, while also allowing you to leverage the latest Infor innovations. You get a faster time to value, long-term return on your investment, and the ability to turn information into a competitive advantage. Includes the implementation of the out-of-the-box Financial Management, Supply Chain Analytics and Project Accounting modules and assumes that Lawson G/L, Infor Lawson Enterprise Financial Management and Infor Lawson Supply Chain Management are fully functioning at the time if its implementation. Administrator Training and "Train the Trainer" End User Training - $8,640 CFO Solutions offers a 4 day training course designed to prepare the core team to manage their application and deploy trainers throughout the organization. This is separated from the core implementation, as clients may alternatively choose to outsource their administration of the system, or utilize a different funding source (i.e. operating vs capital funding) and therefore need to separate it from the implementation pieces.

7. CFO Solutions' d/EPM Core Drill-Through to Lawson - $22,240 Enables users to drill directly into Lawson’s G/L Journal-level details from within d/EPM as provided by the PMVIEWs integration. The following fields are included in the drill:

Drilling into sub-ledgers is possible, but not included within the scope of this particular module. Drills occur directly into the source G/L data and no staging area is used to cache the data. 8. Report Development 10 pack - $8,020 Build or modify 10 reports such that the formatting and end user experience is consistent with the client’s expectations. Includes formatting for both on-screen and print use, so for this reason the customization of pre-existing reports counts towards this number. 9. Additional Data Source Configuration - $11,700 per additional data source A Hospital with additional data requirements outside of the Lawson G/L system may choose to purchase additional data source load configuration services. Data source configuration can be based on table to table load or text file to table loads. Creation of supporting accounts in the account dimension is included in these services. Where source systems allow, data load scheduling can be enabled. Hospital is responsible for providing source data to Consultant in a pre-defined format as defined by Consultant. CFO Solutions' d/EPM Lawson Sub-Ledger Drill-Through - varies based on the Sub-Ledger This module requires the Core drill-through and enables users to drill from the G/L Journal level into a single sub-ledger. Please contact CFO Solutions for pricing regarding the particular Lawson G/L sub-ledger and the other Lawson products installed. 11. Advanced Workforce Planning Module - $48,750


This module introduces driver-based methodology for the planning of salaries with a high-level staffing functionality that would facilitate the requirements in Canadian healthcare organizations, and any U.S.-based organization that looks at this similarly. Actuals can be introduced to look at job-code-level productivity.

12. Automated Forecasting via the R Statistics add-on - $55,200 This add-on enables clients to leverage the R Statistical modelling package to automatically forecast out their data on-demand. This is functional within the Core module, the CFO Solutions’ Healthcare Planning Content Package for Physicians’ Groups, the CFO Solutions’ Healthcare Planning Content Package for Facilities, and the Advanced Workforce Planning Module. 13. Cash Flow Configuration - $11,700 This add-on includes the resources required to configure cash flow functionality.
