Direct Support Professional Job Description

A Direct Support Professional (DSP) provide direct care including activities of daily living (ADL), monitoring and maintaining safety, assisting with activity goals according to the Activity Support Plan (ASP). This position answers directly to their Medicaid Staffing Supervisor at Embassy Home Care.

EDUCATION High School Diploma or GED equivalent must be kept in personnel file. This includes a waivered family member or friend providing services

RESPONSIBILTIES 1. Follow established Policies and Procedures of Embassy Home Care.2. Assist with personal care.3. If a family member or friend is waivered into the program, the responsibilities are the same as aDirect Support Professional (DSP)4. Encourage individuals to be active, to participate in recreational therapy and activities of dailyliving.5. May transport the individuals into the community but not during the entire service period.6. Keep environment safe and clean for individuals.7. Observe individuals carefully and report changes in condition or behavior to your MedicaidStaffing Supervisor.8. Complete all necessary paperwork during your hours worked. Document all activitiesaccomplished on Service notes.9. Prepare the individual's meals and assist with eating when necessary.10. Provide the Individual with personal care including but not limited to:• Provide bath and personal hygiene, mouth hygiene as needed especially after meals• Assist with clothing, dressing if needed• Observe and report the presence of any reddened skin areas or breakdown to your Medicaid StaffingSupervisor• Keep individual's clothing clean and dry• Make individual's bed daily• Keep individual common areas in home neat and clean• Prepare meals if required

• A DSP worker is not allowed to perform duties outside their scope of practice. For example, DSPworkers are NOT allowed to give the individual 's medication or provide feedings through a gastrictube or PEG tube. These things MUST be done by the guardian. There are no exceptions.

QUALIFICATIONS 1. Current certificate in standard first aid and CPR.2. Valid driver’s license and reliable transportation3. Ability to work independently.4. Excellent communication and listening skills.5. Compassionate, patient and caring nature.6. Able to work in a stressful environment.7. At Embassy Home Care, a Registered Nurse is a competent, conscientious and dependable member of the professional health care team.

EDUCATION: Certified as a Registered Nurse with the State Board of Nursing Mississippi.

QUALIFICATIONS

• Current license to practice as a registered nurse in the state of

practice.

• Excellent mental, physical and emotional health.

• At least one (1) year of clinical practice as registered nurse.

FUNCTIONS

• Follow established Policies and Procedures of Embassy Home Care

• Willingness to work a flexible schedule and fill in when needed.

• Keep environment safe and clean for individuals

• Complete all necessary paperwork during your hours worked. Document all

activities accomplished on Service notes.

• Assists in providing care according to physician's orders and individual patient

needs. Follows established guidelines in accordance with recognized standards of

practice and operational standards.

Assists in assessing and documenting pertinent information that reflects the

patient' s clinical condition, with respect to meeting all patient needs (physical,

emotional, spiritual). Reports the same to the registered nurse and/or physician.

• Assists in maintaining accurate and complete records of nursing observations and

care given.

• Assists in assigning patients to ancillary personnel based on patient's needs and

personnel 's qualifications.
