In Case you missed it in the ODA Newsletter The Oral Health Program in the Center for Prevention and Health Promotion in the Public Health Division of the Oregon Health Authority, seeks public comment on an application to the Dental Pilot Project Program.

Project Title: Dental Pilot Project: Application #200 Training, Dental, Hygienists to Place Interim Therapeutic Restorations

Public Comment Period: The Dental Pilot Project Program at the Oregon Health Authority, will open a public comment period on the proposed application beginning February 22, 2016. The public comment period will close on March 4, 2016.

The purpose of the public comment period is to allow individuals to provide information to the Dental Pilot Project Program that will help inform its decision making. All public comments will be reviewed to assist the Dental Pilot Project Program in the approval process.

CENTRAL OREGON REGIONAL HEALTH IMPROVEMENT PLANNING PROCESS: The Central Oregon Regional Health Improvement Plan (RHIP) 2016-19 has just been released. Oral Health improvement has been identified as a major health priority in our region. You will also be interested in the other health priorities identified for regional focus. Dear Dr. Rupert,

Sometimes, the background chit-chat and giggling of my Assistants just outside the room, drowns out my voice and distracts me as I am trying to talk with my patient. I have on numerous occasions and Staff Meetings gone over the need to hold our voices down, but I can’t make the problem go away.

Any ideas on how to eliminate the background chit-chat & distractions once and for all?

Dear Too Much Office Chit-Chat,

It’s important that you keep this as our little secret, because I’m sure the Bureau of Labor & Industries and OSHA would frown on my personal Staff Training technique. However, as a Practice Management Guru and Expert, my primary responsibility is to the Practitioner. I have to be honest with you and tell you what is going to help improve your practice in the most time efficient and least costly method.

A few years ago, I bought a Fitbit watch and activity tracker for myself, to track my daily movements and activities. I made a little discovery with my Fitbit, when I noticed that the Control Switch of the Canine Training Collar that I had bought for my new puppy was actually emitting a moderate shock to my wrist and Fitbit, every time I was pushing the shock button for my canine training collar! Any time I was training my new puppy and trying to give her a shock to correct an unruly behavior, I was also getting a shock on my wrist from the Fitbit!

That accidental discovery served as the most effective Staff Training tool that I have ever utilized! I took my Fitbit watch and made my Assistant who chit-chatted the most in the background to wear it during the course of the day. I told her that I wanted to track how much distance my Assistants were covering during the course of the day. I did not tell her that it would also provide a ‘gentle reminder’ whenever her chit-chat or voice was distracting me as I was working on a patient.

I kept the Control Switch of my dog Training Collar in my pocket and every time that this Assistant started chit-chatting in the other room; I would push the button which was quickly followed by a quick ‘yelp’ in the other room and then immediate silence.

I only had to buzz her three times and just like my puppy, she figured it out and no longer chit-chats to the point that we can hear her down the hall or in the other rooms. I’m telling you, it works great on puppies and staff … stop by my office and I’ll let you borrow my Fitbit Training Watch … and you can try it for yourself.

I have a heck of a time improving the periodontal status of my patients who smoke. No matter what therapy I prescribe or provide, the periodontal disease seems to progress with time and we’re losing teeth over time. What do you do to get your tobacco smokers to quit their habit and save their teeth?

Dear Would Like to Help Tobacco Smokers Quit,

There have been many stop smoking remedies throughout the years. However, unless the patient is willing & committed to quitting the habit, most remedies have a high failure rate. I don’t ask my patients if they are willing to quit or not. I simply make them quit.

I use a product that does not have FDA clearance yet, despite the fact that it’s nearly 100% effective and used routinely in North Korean Smoking Cessation Programs. The product is a chewing gum that I order from China, since they don’t have a Trade Embargo with North Korea. The gum leaves the patient’s mouth feeling refreshed, minty and clean. I tell the patient that they need to chew a stick of this gum every 3-4 hours to stimulate their saliva flow and help with their smoker’s breath. 100% of the patients who chew this gum every 3-4 hours for two to three days will quit smoking.

What I don’t tell the patient about the gum is that it is highly flammable and leaves a flammable residue in the mouth for up to 6-8 hours. Anytime that the patient brings a cigarette, e-cigarette, joint or other object that is attached to a flame or source of ignition near the mouth, it results in a mild to moderate exothermic reaction or burn of the mouth and surrounding soft tissues! I did have one patient who lost some nostril hair and eyelashes in an ignition, but his nose hairs were too long and nasty to begin with and he actually looked better after things were singed a little.

I’m not kidding when I say that nearly 100% of the patients will either quit their smoking habit within a week or stop coming to your practice. You do have to prescribe some lotions and creams to go with the 1st degree burns of the mouth during the first couple of days, but those all heal within a week … whereas the effects of smoking on their mouth and body will last for a lifetime (as well as affect those around them through second hand smoke). I tell my patients to make sure to read the package insert before using the product, but since the package insert is in Korean, nobody can understand the warnings; and the fact that there is a warning label and you made them aware of it gets you legally off the hook for any potential damages.
