receive follow-up care and support within their community.
A person suffering from addiction can directly contact a Team Member, or can enter a health care facility;
The Team will then coordinate care and a treatment plan; and the person will receive all available supports, coordinated by the Team;
A Team Member will follow the person’s progress and offer continued guidance and support.
