AGENDA

How we got here…o Federal Criteria requires that OPWDD move Service

Coordination to a Conflict Free model by 7/1/2018.

oAs with other states, NY is moving to a Medicaid Managed Care model within the next few years. People with IDD are the last group to move to managed care.

oThe Care Coordination model will allow providers of developmental disability services to maintain control of the services and ensure the funding will be preserved for this population.

BACKGROUND

LIFEPlan CCO was founded by The Center for Disability Services in Albany & Upstate Cerebral Palsy in Utica

Have been working together for 5 years, first on the DISCO/FIDA Managed Care Initiative, and second on the Care Coordination Organization (CCO).Upstate CP is an experienced NYS Health Home in 7 Counties in

Central NY & North Country for Adults and 10 Counties for Children and has an established network of healthcare providers.

LIFEPlan’s Geographic coverage is all of OPWDD Regions 2 & 3, which includes 38 counties

Approximately 10,000 members

in Region 2

o We currently have commitments with MSC provider agencies totaling over 17,000 lives.

o This includes partnering with agencies in Central, Northern, Capital Region, Southern Tier and the Hudson Valley.

o We anticipate establishing Local Hubs and Offices to help manage the CCO and account for regional differences.

o Utica, Syracuse, Albany, North Country, Hudson Valley and the Southern Tier are planned Regional Offices.

OUR PROGRESS

o MSC Agencies--LIFEPlan is offering a choice of options to your home agencies who have aligned with us:

Transition MSC Services into LIFEPlan CCO on July 1, 2018Subcontract Services for 6 or 12 month options, the limit set by OPWDD

o People receiving MSC services—MSC ends 6/30/18. Individuals will be auto-enrolled into CCOs by OPWDD based on MSC provider agency alignment; Individuals will then be given choices of CCOs, as well as to opt out of enhanced care management services.

o MSCs--LIFEPlan CCO anticipates needing all qualified MSC staff to help create our CCO.

o It is LIFEPlan CCO’s hope that individuals/families and their MSCs will choose to transition together to the new CCO-HH enhanced care management model.

o Family Advisory Roleso Special Populations---focus on Sub-populations such as

Dementia/ Alzheimer's, Geriatrics, Willowbrook.
