Couchtops & OverlaysThe Importance

As technology advances it becomes increasingly important to have radiation-friendly treatment devices. Indexable carbon fiber couchtops and overlays provide you with a radiation-friendly base creating fewer image artifacts and more treatment plan options. CIVCO’s couchtops & overlays also provide an indexing system for CIVCO’s wide variety of patient positioning & fixation accessories. This allows a patient positioning device to transfer accurately between couchtops and overlays and quickly lock into the same location daily on the treatment table.

Differences Between a Couchtop & Overlay

An overlay lies on top of the cradle, existing support frame and/or spine of a CT, simulator or LINAC. For LINACS, an overlay will still result in the need to rotate the table in order to avoid the beam passing through the underlying steel support. A couchtop replaces the steel support system of the LINAC with a carbon fiber support system, eliminating the need for table rotation. An indexed overlay is necessary for a CT to ensure that your positioning devices are in the correct location from room to room. These overlays are lightweight and can easily be removed between uses on a non-dedicated CT.

How Indexing Works

Every Indexed Patient Positioning System™ begins with an indexed couchtop or overlay. These tops feature an array of indexing points running down the side of the couchtop, each spaced an identical 14cm apart or 7cm apart on the Universal Couchtop. A Lok-Bar™ is also provided with every couchtop or overlay. Every Lok-Bar features two pins that interface with the two holes on CIVCO’s line of patient positioning and fixation devices. Lok-Bars remain with the couchtop.

To index a product, attach the Lok-Bar to any of the indexing points. Then set the patient positioning device on the two-pins on the Lok-Bar. Indexing patient positioning and fixation devices to the same indexing notches for every treatment ensures that your device is in the same location and is “square” with the couchtop for every treatment. This definitive repositioning of the device relative to the treatment table allows clinicians to confidently tighten the tolerance window in the record-and-verify system – an important enhancement. The result: increased target accuracy with increased patient throughput.

Universal Couchtop™

The new Universal Couchtop is a strong, lightweight couchtop with low attenuation, ideal for IGRT tracking. The Universal Couchtop is also available in a dosimetrically matching CT overlay to assist in dose accuracy between planning and treatment. Contact your account representative to confirm DoseMatch technology is available for your CT Simulator.

The Universal Couchtop features a metal free extension seam with five extension choices: rectangular extension, Palliative Extension, Type-S™ extension, Posifix® extension and Varian end plate. The unique profile allows CIVCO’s SBRT Body Pro-Lok™ bridges to attach directly to the Universal Couchtop. The Universal Couchtop is capable of being installed on linac models from Elekta, Siemens and Varian. The CT overlay is compatible with nine different models from GE, Philips, Siemens and Toshiba.

Carbon fiber couchtops and overlays for linear accelerators and simulators provide both a radiation-friendly treatment surface and indexing capabilities. Some of these couchtops and overlays have exchangeable panels, allowing the user to choose between a solid (foam core with a carbon fiber layer), tangential or a grid surface for your treatment.

Carbon fiber overlays securely lock onto a cradle, providing a surface consistent with treatment couchtops. They are lightweight and durable, allowing them to be easily removed or reinserted for use on non-dedicated imaging machines. The attachment mechanics ensure that once the top is reinserted on the cradle, it will be level and stable throughout imaging.

CT overlays are made of carbon fiber with a foam core. They have indexing options and come with a two-pin Lok-Bar™ providing a system for indexing positioning devices to the couchtop.

Overlay Stand

The Overlay Stand is ideal for shared CT rooms, where space is limited. The overlay stand creates a fast and effective way to store and protect your CT overlay between uses. The base and strap plate easily mount to the wall, allowing for quick access and out of the way storage. Upper straps secure the overlay between uses.

Foot Extension

SBRT/SRS ImmobilizationThe Importance

The use of SBRT is quickly expanding due to significant advances in radiation therapy along with a history of success. According to Dr. Martin Fuss, “Outcomes of early clinical experience in medically inoperable patients are very encouraging and thus the use of SBRT is expanding.”1 With higher doses and fewer treatments patient positioning for SBRT has become even more important. “Accurate patient positioning and tumor relocation are essential for SBRT use in the liver and other abdominal and retroperitoneal sites, as at other tumor sites,”2 says Brian Kavanagh.

Why CIVCO SBRT Immobilization

CIVCO’s Body Pro-Lok has become an essential immobilization tool at numerous clinics, both for SBRT and other treatment procedures. The Body Pro-Lok system offers a wide variety of accessories and provides the ability to utilize your existing CIVCO positioning devices. The decision to purchase an SBRT immobilization system is an important choice for any cancer center. When Ball Memorial Hospital was testing different SBRT immobilization devices, they chose to demo three different systems before making a decision. “We had one therapist act as a patient in all three demos, so we could get a detailed impression from the patient’s perspective,” Harmon explained. “She felt most comfortably and securely immobilized in Body Pro-Lok, and the respiratory belt felt like it constrained breathing better than others while being much more comfortable.”

Facilities have also found the Body Pro-Lok to provide effective respiratory restriction. “Body Pro-Lok with compression plate is considered the most effective for SBRT respiration immobilization and allows for setup reproducibility while providing patient comfort within the short treatment period.”3

Immobilization is crucial for head and neck radiotherapy because of the many critical anatomical structures in the area. Whether you are providing conventional radiotherapy, 3D Conformal, or IMRT treatments, accurate, reproducible patient positioning and fixation are important factors for precise dose delivery to the head and neck. Thermoplastic based systems offer a reliable method for accurate positioning and immobilization, they are comfortable, easy to use and cost effective.

There are three essential components to a head and neck system. First, the baseplate provides the foundation for the head and neck system, various baseplates provide different options including different thermoplastic and headrest options and supine, tilting, prone or lateral setup choices. Next the headrest provides enhanced repositioning, accuracy and patient comfort, available in standard-formed or custom-molded supports. Finally the thermoplastic mask provides fixed positioning from imaging through treatment, available in various materials, sizes and shapes providing a custom solution for each patient.

Posicast® Thermoplastics

Microperfo Multiperfo

Posicast thermoplastic material has been specifically developed for radiotherapy applications and provides excellent positioning and reproducibility during treatment. It does not stick to the skin or hair. If needed, Posicast adheres to itself by applying local pressure. The patented composition of this product results in minimal shrinkage after cooling.

Posicast thermoplastic materials are available in Multiperfo and Microperfo versions depending on the selected material. Microperfo masks are preferred when small sized treatment fields are to be marked onto the mask.

PosicastPosicast has a soft polyurethane coating on both sides guaranteeing ultimate comfort in handling. It is available in Multiperfo and Microperfo.

Posicast-PlusPosicast-Plus has two different coatings: a soft polyurethane coating on the side which is in direct contact with the patient’s skin and a smooth spray-coating on the outside where treatment fields are to be marked directly onto the mask. Posicast-Plus material is comfortable to the skin and offers the possibility of precise field marking. Minimal shrinkage of the material ensures rigid and reproducible fixation. Posicast-Plus is only available in Microperfo.

Posicast-Lite With a smooth spray coating on both sides, Posicast-Lite feels like a traditional thermoplastic. Its polycaprolactone core ensures rigid and reliable fixation. Posicast-Lite is only available in Microperfo.

PrecutsCIVCO offers a wide variety of thermoplastic precuts. Rectangular plates are available allowing technologists to cut the material in any desired shape. All types are sold in sets of 10 pieces. The materials have a thickness of 2.3mm. Precuts are available with reusable or disposable L-profile mask attachment handles.

L-profiles for Mask AttachmentBy using radiotranslucent L-profiles fitted with durable quick fasteners, a rigid mask can be molded around the contours of the patient. With this method air gaps between the inside of the mask and the head of the patient are prevented, resulting in a very reliable, stable mask.

Reusable Carbon Fiber L-profiles are attached to the desired precut before making a mask and remain on for the duration of treatment. Disposable L-profiles come attached to a mask and are a quick, cost effective alternative to reusable carbon fiber L-profiles.

Uni -frame® BaseplatesThe Uni-frame head and neck fixation system provides rigid immobilization for enhanced image quality, as well as consistent patient posture through imaging, simulation and treatment procedures. Every Uni-frame baseplate is indexable, acrylic versions are three-pin indexable and MRI compatible.

Available in carbon fiber or acrylic, the baseplates feature a cutout area under the headrest for visualization and treatment. They utilize easy-to-use swivel clamps for locking down a variety of thermoplastic mask options.

Tilting Baseplates

Tilting baseplates adjust for angles from 0 to 45° and are ideal for pituitary treatments. The product line includes a carbon fiber baseplate with a square base cutout for posterior treatment fields and three acrylic baseplates: one with an “I” shaped base for posterior oblique treatment angles and two with a cutout base.

Posifix® Supine Headrests

Posifix headrests are made of low density polyethylene foam, in five colors. The red cushion positions the head with the neck straight to prevent anterior curving of the spinal cord in the treatment field. The black cushion is used for neck tumors and has the same curvature but is extended in the caudal direction. The yellow and white cushions enable the chin to be more or less extended to move the oral cavity and mandible out of the radiation field, or to obtain a cranial field border which avoids the orbit (ear and parotid treatment). The blue cushion allows the chin to be fixed towards the breast.

Cranial Stop Headrest - Coming Soon!

CIVCO’s new Cranial Stop Headrest offers improved superior-inferior repositioning of the patient for head and neck setups. The enhanced cranial form provides a more-contoured fit of the patient, resulting in a more efficient, longitudinally- accurate setup of the patient. This wedge shaped cushion supports the shoulders and upper body of the patient. Custom Headrests

AccuForm™ Cushions

AccuForm Cushions are ideal for high-precision head and neck treatments when enhanced comfort, stability and repositioning accuracy are key. This soft, cloth-covered pillow is filled with tiny beads coated in water-curable resin. When sprayed with water, the cushion becomes pliable, allowing it to be formed to the patient’s specific anatomical contours, including custom pitch and rise. After a few minutes the cushion forms a rigid, yet comfortable custom mold. The 15 x 20cm cushion is ideal for filling in gaps under a patient’s head and neck on a standard headrest such as a Silverman while the 20 x 25cm can be used as a stand alone cushion. Head & Neck Vac-Lok™ Cushions

Vac-Lok custom-formed vacuum cushions provide comfortable support of the head and neck as well as the upper shoulders. Filled with tiny polystyrene beads, the cushion forms a rigid mold when a vacuum is pulled. The versatile system can be used to create a patient specific cradle, and is reusable after the patient treatment cycle is complete. Vac-Lok cushions are available in a variety of shapes and sizes in both polyurethane or nylon finishes. The nylon Type-S cushion is indexable.

Precise Bite™ Patient Re-Positioner

The Precise Bite attaches easily to a thermoplastic cast and conforms to the teeth and/or maxilla, decreasing head and neck rotation, flexion and extension. The Precise Bite with tongue depressor assists in tongue displacement and provides an airway opening for easy breathing. Precise Bites are sold in packages of 5.

The positioner softens in warm water, then comfortably conforms to the patient’s dental contours. After the dental positioner is set, a thermoplastic cast is pulled over the patient, and the Precise Bite docking plate simply snaps on top to secure the Precise Bite to the mask. Shoulder Retractor

The Shoulder Retractor is often used with thermoplastic-based fixation systems. Retraction is especially beneficial for shoulder clearance when opposing lateral fields are used to treat the neck. The MT-350 carbon fiber breastboard features carbon fiber grid treatment panels for rigid support for the bi-lateral treatment windows. The breastboard’s carbon fiber construction makes it lightweight and “treatment field friendly” with low attenuation coefficients. The breastboard features a positive locking angulation system offering angles in 2.5˚ increments from 0 to 20˚ and an angle of 25˚. The hip stop is indexable to ten locations and prevents patients from sliding down the board. The MT-350 Breastboard is CT compatible and accepts a wide variety of arm supports and headrests providing you versatile setup options.

How do breast thermoplastic systems compare to a treatment brassiere?

Breast thermoplastics help lift and custom-shape the affected breast up off the chest wall, reducing skin reaction and unwanted radiation dose to the patient’s lung. The treatment brassiere positions the breast similarly, but is not a custom fit and is not affixed in a reproducible point to the breastboard.

Step -by -Step1. Soften thermoplastic in the

waterbath as instructed in the instructions for use.

2. Starting with the affected side, attach one handle of the softened thermoplastic to the breastboard as your assistant pulls the opposite handle to the other side.

3. Stretch the thermoplastic over the patient, using both hands to cup the breast, shaping it into the desired treatment position.

4. Attach the second thermoplastic handle to the breastboard. Continue to mold the thermoplastic around the affected breast, taking care to form the sheet to the sternum.

5. Use the thermoplastic to compress the contralateral breast out of the treatment field.

6. Allow the thermoplastic to cool completely. For additional repositioning, outline the areola mammae of the affected breast onto the thermoplastic sheet.

Tips for Daily Breast Repositioning with Thermoplastics

Method A With the patient in treatment position on the breastboard, tape the breast into the original treatment position. Attach the thermoplastic handle to the breastboard on the affected side, then bring the sheet up and around the breast, taking time to carefully shape the breast back into position. Attach the opposite handle to the breastboard.

Method B Ask the patient to stand beside the table and lean forward. In this position, use the thermoplastic cast to “capture” the breasts. Lay the patient onto the breastboard, holding the cast in place around the chest. Attach the thermoplastic handles to the breastboard, repositioning breasts as needed.

Hip & Pelvic PositioningThe Importance

Patient positioning for the hip and pelvic region can be difficult due to organ motion and patient shape. Before choosing a pelvic or abdominal system, it is important to first determine which emphasis - re-positioning or immobilization - is most paramount to your setup needs. For instance, if your prostate treatments are performed under a daily image-guidance system, you may determine that immobilization is your primary goal, since real-time image acquisition and subsequent isocenter adjustments will account for much of the re-positioning. However, if daily imaging is not being performed, a repositioning system will be more important.

Carbon Fiber BellyboardThe Carbon Fiber Bellyboard has been developed in order to reduce the irradiated small bowel volume of patients undergoing treatment in the pelvic region. For treatment fields including the pubic bone towards the third lumbar vertebra, reduction of the irradiated small bowel volume up to 64% (four field box technique) can be obtained. The Bellyboard is radiotranslucent, lightweight, rigid and indexable.

For patient comfort the lower part of the Bellyboard has been adapted to support the upper legs. A soft polyethylene padding on the cranial side ensures a comfortable position for the head.

The Bellyboard has two apertures:

The caudal aperture (1) is for the positioning of the pubic bone

The central part (2) of the device is for the compression of the small bowel in order to push the small bowel cranially towards the major aperture (3).

Positioning guidelines for patient set -up on the Bellyboard:

As a patient positions themselves on the Bellyboard they should start cranially and slide in the caudal direction. The pubic bone must be positioned in the caudal aperture for an optimal reduction of the small bowel volume. Fiducial markers are integrated in the bottom part of the device for patient setup verification. Clear scales on both sides of the Bellyboard allow easy verification of the longitudinal position of the patient.

Prone Thorax SupportThe Prone Thorax Support is designed to relieve chest compression during treatments in the prone position. It can be used as a stand alone patient support or as an option on the Bellyboard to allow large patients to be treated in the prone position in order to decrease the volume of irradiated small bowel.

CombifixThe Combifix is a baseplate that indexes to the couchtop and combines the Feetfix and Kneefix. It is recommended that both cushions are used in combination, as each cushion enhances the performance of the other, especially when treating the pelvic region.

Shoulder RetractorsThis shoulder retractor system consists of straps in two different lengths with hand grips that can be attached in five positions on the Feetfix Couch Support. This allows the shoulder retractor to be adjusted at ten positions. With the shoulder retractor the shoulders can be pulled down during radiotherapy simulation and treatment sessions where opposed lateral fields are used in the neck area. The shoulder retractor can be used with the Feetfix Couch Support alone or the whole Combifix system.

Feetfix™ Couch SupportThe Feetfix Couch Support positions the Feetfix at indexed positions and allows the Feetfix to be tilted at four different angles. The Feetfix Couch Support can be purchased separately from the Combifix and is two-pin indexable.

HipFix®

The HipFix hip and pelvis immobilization system utilizes a single sheet of thermoplastic over the entire abdomen and pelvis region. The versatile system accommodates both prone and supine patient setups. The HipFix system provides immobilization to enhance fixed fiducial targeting.

The HipFix baseplate features a cutout treatment window. The baseplate can be indexed to the couchtop. A Vac-Lok or RediFoam cushion can be integrated to the HipFix for added comfort, support and repositioning accuracy.

Multifix™

The Multifix is a flexible system allowing arms and legs to be fixed in straight or bent positions, using small Posicast® precuts. These precuts are easily shaped to anatomical contours and are then fixed onto the baseplate on clearly indicated positions. The baseplate is indexable to ensure a reproducible setup. It features four sets of indexing holes to offer an isocentric position of the treated extremity wherever it is placed on the Multifix.

Vac -Lok™ CushionsEasy-to-use, reusable Vac-Lok cushions are designed to meet the challenges of today’s radiotherapy and diagnostic imaging departments. Filled with tiny polystyrene beads, Vac-Lok cushions create a rigid, comfortable cradle around the patient when a vacuum is drawn through the quick-release valve. The quick-release valve is self-sealing.

Vac-Lok cushions are available in two durable finishes, clear urethane and blue nylon - which is reinforced for resistance against punctures. Some styles of nylon cushions are also available with indexing for enhanced patient positioning. Both styles of Vac-Lok cushions are washable and reusable for ultimate cost-effectiveness. They are radiotranslucent, almost air equivalent, and available with treatment windows to avoid any possible buildup.

Indexing battens provide an easy method for repositioning Vac-Lok cushions.

Vacuum CompressorsCIVCO’s Dual Function Vacuum Compressor provides both a vacuum and inflate cycle with a convenient hand control or foot control. It provides an audible alarm when your optimum vacuum level is reached. The pump is both Medical Grade and ETL tested and is available in a 115V and 230V version. This pump has been CE Marked.

The Single Function Vacuum Compressor is a lower cost alternative. It provides a vacuum cycle only. To re-inflate a Vac-Lok cushion, simply detach the compressor and depress the end of the cushion valve. With the MTVL1BEX the hoses can be switched to provide an inflate cycle.

Testicle ShieldsTesticle shields are designed to reduce scatter radiation to the testicles. Each unit is cast to provide a lead wall with a thickness of ½” (1.25cm) and features an open sector to allow comfortable attachment to the patient. All surfaces are coated with a polyurethane paint to reduce lead exposure.

The shield’s top and bottom sections are secured with either the two included rubber straps or nylon tape. When using the testicle shield stand, no straps are needed.

BolusBolus is made from a solid, homogeneous, tissue-equivalent gel with a density of

1.03g/cc. It will not sag, deform or run. Bolus is available encased in a tough layer of thin plastic “skin”, or “skinless” or sticky for greater conformity to steep body contours. It is closely controlled for uniformity of thickness, electron density and homogeneity. Bolus sheets can be cut with a scissors to any desired shape and layered to obtain the desired bolus effect.

Bolus is the ideal choice for applications above 1MV where additional tissue-equivalent thickness is required. It can also be used as a phantom material due to its homogeneous composition and uniform thickness.

Return PolicyIf you are dissatisfied with a product, contact a CIVCO customer service specialist for advance return authorization and instructions. Returns should be arranged within 30 days of the invoice date and are subject to a 25 percent restocking fee. Returned goods must be in original packaging and in new condition. CIVCO reserves the right to refuse unauthorized returns.

Claims, Shortages or DamagesPlease open all shipments immediately and inspect for concealed damages, shortages or errors. If goods were damaged in shipping, please hold the package and request a carrier inspection and claim form. Please report any shortages or errors to your CIVCO customer service specialist within 30 days of receipt of shipment to arrange an authorized exchange.

Product WarrantyReusable products. CIVCO warrants products manufactured by CIVCO and intended to be reusable shall be free from defects in materials and workmanship for 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions.

Disposable products. CIVCO warrants products manufactured by CIVCO and intended to be disposable (single-use) shall be free from defects in materials and workmanship for 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions. Sterile disposable products shall be sterile for not less than 12 months from the date of invoice by CIVCO unless otherwise stated on the product labeling and/or instructions.

All products not manufactured by CIVCO are warranted if applicable as defined under the original manufacturer’s warranty as stated on the labeling and/or instructions.

Buyer’s sole remedy for breach of this warranty will be replacement or repair of the product or part, at CIVCO’s option. To the full extent allowed by law, the foregoing warranties and remedies are exclusive and in lieu of all other warranties, terms, or conditions, express or implied, either in fact or by operation of law, statutory or otherwise, including warranties, terms, or conditions of merchantability, fitness for a particular purpose, satisfactory quality, correspondence with description, and noninfringement, all of which are hereby expressly disclaimed.

Notwithstanding the foregoing, CIVCO makes no warranty, nor shall it have any obligation to the customer with respect to any product sold hereunder, if such product has expired according to product labels or has not been used, handled or stored in accordance with written CIVCO guidelines.

Disclaimer. Some countries, states, or provinces do not allow the exclusion or limitation of implied warranties or the limitation of incidental or consequential damages for certain products supplied to consumers, or the limitations of liability for personal injury, so the above limitations and exclusions may be limited in their application to buyer. When the implied warranties are not allowed to be excluded in their entirety, they will be limited to the duration of the applicable written warranty. This warranty gives buyer specific legal rights which may vary depending on local law.

All products may not be licensed in accordance with Canadian law.

Quality StandardsCIVCO is registered to ISO 13485:2003 Quality Standards.
