In 2016, RVNA opened its Center for Exceptional Care in Ridge�eld,

Connecticut. It encompasses a Nutritional Education Center, Caregivers Support Center,

Competency Lab/Skills Training Room, Rehab by RVNA and HomeCare by RVNA, our

non-medical home care agency. It also houses of�ces for clinical and administrative staff

and meeting rooms for community health and wellness meetings and programs.

In addition to continuing to provide healthcare in one’s home, and public health in the community,we can now offer care under our very own roof.

We’re proud to say that despite the transformative changes taking place in the health care industry and the challenges in government reimbursement, RVNA’s continued focus is on enhancing the quality of life along a continuum of care. We offer multi-generational health and wellness services and work alongside physicians and other healthcare professionals for the good of all residents in our communities, while still providing charitable care and community benefit.

During the past year, RVNA moved to its new headquarters at 27 Governor

Street in Ridge�eld. The building offers space for a great variety of new programs

and services. Launched in the past year were Rehab by RVNA, an outpatient

physical therapy center; dietician services and consultations in the Nutritional Education Center, containing a state-of-the art teaching kitchen; a

Competency Lab/Skills Training Room to review practices and teach

the latest nursing techniques; a Caregivers Support Center to provide

resources and programs to support those caring for loved ones; and dedicated space

for HomeCare by RVNA, a non-medical home care agency that offers services

for those needing assistance at home.

... healthcare at home RVNA’s extensive clinical services offered care to all ages and conditions from birth

to end of life. Patients underwent comprehensive physical assessments at

the start of care resulting in careful care planning and case management. In-home medical technology including INR, glucose testing and pulse oximetry moni-

tored patient conditions and informed care plans. Speci�c disease management programs provided optimum care for patients with cardiac, diabetic, dementia,

respiratory, wound care, orthopedic/balance and end of life conditions. RVNA’s “Tell-Us-Your-Health” personal telemonitoring program was an

integral component of patient care and was managed by a nurse coordinator who con-

nected with patients via regular telephone communications. A Clinical Education Coordinator instructed RVNA nurses on the most advanced techniques and skills.

Physical, occupational and speech therapies were provided for joint

replacements, orthopedic care, balance and vestibular rehabilitation, cognitive and

language restoration, falls prevention and maintenance programs for MS, Parkinson’s

Disease and ALS.

RVNA’s medical social worker assessed needs, connected patients with

RVNA services and other resources, provided care coordination/oversight, communicated

with family, and assisted with Medicaid forms, insurance and budgeting. The

Paraprofessional Staff provided non-medical assistance with bathing, dressing

and toileting, mobility, light housekeeping and medication reminders.

CLINICAL STAFF ACCOMPLISHMENTS:

Ashley Snelwar, RN Case Manager was certi�ed as an infusion nurse and recognized as a Hearst Media Top 10 Nurse in Fair�eld County.

Crystal Madyda, Physical Therapist was certi�ed in kinesiotaping.

Clint Layog, Physical Therapist earned a Doctorate in Physical Therapy.

Natalie Farrar, RVNA Chief Clinical Of�cer and certi�ed trainer, trained all professional staff to be Certi�ed Dementia Practitioners.

RVNA received Visiting Nurse Associations of New England Innovator Award for its Dementia Care Program.

With expanded space in the new building, RVNA opened an outpatient rehabilitative therapy center, Rehab by RVNA, in 2016. It provides a full

range of physical therapy and is expanding to also offer occupational and speech

therapy. For RVNA clients who are no longer homebound but require additional therapy

in an outpatient setting, they can continue their care in our state-of-the-art facility.

HomeCare by RVNA continued to provide quality non-medical hourly and live-in caregivers in the home to promote the health, safety and comfort of individuals. Assistance ranges from companionship to hands-on

care, depending on the need. HomeCare by RVNA specializes in the care of all levels

of dementia, degenerative diseases and terminal illnesses. Great effort is made to match

the caregiver to the individual to ensure the highest quality of care possible.

RVNA’s Community Health and Wellness services were offered throughout the year

in a variety of formats. 107 educational programs were held in 12 different

communities. Flu and pneumonia clinics were conducted during the fall and

winter at locations in and beyond Ridge�eld. Blood pressure screenings took

place at nine area locations. Nurses provided immunizations, medication administration and blood draws in RVNA’s Center for Exceptional Care

clinic rooms. 139 travel consultations were provided for people heading

abroad. The annual Health Fair had 575 attendees and offered 1,227 individual

screenings. RNVA’s dietician provided private consultations and programs in our new

Nutritional Education Center. RVNA staff offered 5 CPR training classes to

clinical staff and outside groups. The Quality Living at Home newsletter was distributed to 1,029 people aging-in-place in area communities. RVNA’s public safety arm was responsible for tuberculosis treatment surveillance.

ACCOMPLISHMENTS:

Barbara Newland, Director of Community Health and Wellness earned Aging-in- Place Specialist certi�cation, became a certi�ed CHOICES counselor and was credentialed to offer a program entitled “Powerful Tools for Caregivers”.

April Rodriguez, RN was cer-ti�ed in travel health and with Terry Chikezie, LPN became a credentialed CPR trainer.

RVNA provided Lifeline medical alert devices to help keep 159 clients safe at home.

Volunteers are the backbone of our organization. In the past year 2,742 volunteer hours were contributed by community members, registered

nurses and physicians, business professionals, high school and college interns

and civic groups.

Volunteers served on our Board of Directors and various committees,

provided of�ce and clerical support and worked at our �u, blood pressure and well child clinics. More than 100 volunteers contributed to the success

of our annual Community Health and Wellness Fair; others chaired

fundraising events, offered educational seminars and volunteered as

Friendly Visitors. Volunteers conducted research on caregiver resources and

helped restock RVNA’s food pantry for patients in need.

Volunteers assembled 150 holiday gift bags for distribution to RVNA patients.

Lion’s Heart service organization stocked RVNA’s food pantry.

Friendly Visitors offered companionship and support for the homebound.

Ira Joe Fisher For seven years, Ira Joe volunteered as auctioneer at our annual

Spring Breakfast, helping to raise critical funds to support our mission. His keen ability

to captivate a room full of supporters with his charm, humility and great humor made

this annual morning event lively, entertaining and enjoyable for all, while bene�tting

RVNA. We are grateful to Ira Joe for his many years of kindness and support.

Pam Gugliotta Pam donated countless hours of her time and design expertise to

the décor of our new facility. From furnishings to �xtures, and keeping budget in mind,

Pam created an environment both aesthetically and functionally pleasing to perfectly

suit RVNA’s needs. She has been decorating businesses and residences in Ridge�eld and

beyond for 40 years. RVNA is deeply appreciative of Pam’s time, talent and dedication.

Carl Lecher Carl was RVNA’s Owner’s Representative during the construction of

our Center for Exceptional Care, ensuring the completion of our project would be

on-time and in-budget. His many hours of pro-bono involvement, advice and support

were invaluable to our agency, as he ensured our best interests were at the heart of

every decision made. Carl has been a trusted advisor and friend, and we are proud

to honor him.

RVNA and Meals on Wheels have worked in cooperation to serve the elderly, homebound members of the Ridge�eld community for more than forty years. Together, we provide home healthcare and encourage a diet of nutritious meals so that Ridge�eld senior citizens can remain independent in their comfortable surroundings.
