anonymised dataset showing the tests performed for
each suspected cancer referral in 2014/15
o This was to inform our modelling of the future
demand for diagnostic tests
o Our fantasy was ….patient level data of each event

appointment, CT scan etc

• Upper GI cancer 2WW cancer predicted referrals drop of -40% to -80%
• Assumed primary care will refer to direct access endoscopy so not technically cancer 2WW but urgent 2WW instead.
• Will identify patients unlikely to have cancer.
• A much smaller proportion of patients will therefore require a 2 week wait cancer referral to the hospital Trust.
• The overall number of referrals will not be impacted by this change only whether they are referred as a 2 week wait cancer pathway referral to the hospital Trust or an urgent 2 week wait direct access referral to an endoscopy service.
• The benefit is a significant reduction in the number of outpatient appointments required in the hospital Trust and associated costs.
• Issues around governance and pathway linkage with the independent endoscopy providers and associated pathology laboratories have been raised.

annum to approx. 9,500 per annum in TVSCN. This is a 61% increase since

• Upper GI cancer 2WW cancer predicted referrals drop of -40% to -80%
• Assumed primary care will refer to direct access endoscopy so not technically cancer 2 week wait but urgent 2 week wait instead.
• Will identify patients unlikely to have cancer.
• A much smaller proportion of patients will therefore require a 2 week wait cancer referral to the hospital Trust.
• The overall number of referrals will not be impacted by this change only whether they are referred as a 2 week wait cancer pathway referral to the hospital Trust or an urgent 2 week wait direct access referral to an endoscopy service.
• The benefit is a significant reduction in the number of outpatient appointments required in the hospital Trust and associated costs.
• Issues around governance and pathway linkage with the independent endoscopy providers and associated pathology laboratories have been raised.

• Upper GI – complex work up, tertiary centre involved
and late stage presentation
• Urology – Primary care don’t always undertake
baseline investigations, single sex clinics, thinking
time for treatment options and complex range of tests.

• Diagnostics are becoming more numerous, complex and time consuming per referral
• Currently no one meeting 28 day referral to confirmed diagnosis - range 0-60%
• Overall radiology and pathology services are unsustainable in the long term
o workforce,
o year on year cut to budgets,
o diagnostics not included in service planning,
o equipment falling over.
