WHC is a Federally-funded breast and cervical cancer screening program serving low-income, uninsured Idaho women since 1997.

Approximately 4,000 women are served per year– in 2016, the Program expects to exceed this number after adding the 21 – 39 year old age group to the cervical cancer screening eligibility range.

When diagnosed with breast or cervical cancer through WHC enrollment, patients are eligible for Breast and Cervical Cancer (BCC) Medicaid for treatment coverage.

WHC diagnosed 36 women for breast cancer, and 41 women for cervical pre-cancer requiring treatment in 2015.

Since 1997 WHC has provided breast and cervical screening and diagnostic services and patient navigation to more than 61,000 Idaho women. Yet, Idaho remains last or near last in the nation for breast and cervical screening rates.

WHC provides professional development to assure quality screening and patient navigation services.

WHC works with partners and stakeholders to leverage opportunities for collaboration to increase screening rates in Idaho through implementation of evidence-based education and outreach efforts, evaluation and sharing of ideas for improvements in system, policy, and environmental change.

LLS will be hosting a Blood Cancer Treatment Update in late spring. This will be focused on a review from ASH as it pertains to blood cancer treatment findings. LLS is working with Triage Cancer and the Comprehensive Cancer Control Program to bring an in-service training for oncology health care professionals on employment, disability insurance, and health insurance issues faced by cancer survivors and caregivers. We are also hoping to incorporate A patient/caregiver focused educational event on employment, disability insurance, and health insurance issues faced by cancer survivors and caregivers. As always, we have free patient and professional web education program, offering free CE’s to HCP’s. Our information resource center continues to:

The statewide Health Matters Wellness Program includes a website, blog, FB page, and two state employee wellness e-newsletters a month. The program focus is on state employee wellness education, motivation to create healthy behaviors at home and at work, providing access to credible health resources, and promotion of statewide wellness opportunities/events/screenings. The website is accessible to the general public; it is not password protected. It averages 8,000 visitors per month and over 220,000 web hits per month. The website resources change monthly to reflect timely topics…e.g., skin cancer, prostate cancer, breast cancer, colon cancer, etc. The e-newsletters further flesh-out those topics and bring the reader back to the resources on the website or blog. In October there is an entire web page devoted to breast cancer resources, in March there is an entire web page devoted to colon cancer, and there is a permanent web page called Sun Safety devoted to skin cancer prevention, treatment, and screening. Outreach Example: In March 2016 the “Your Colon” web page will once again be live. It will promote colorectal cancer prevention, screening, and treatment. Events include an employee poetry contest, a noon hour seminar on colon cancer and genetics, and a display of the walk-through Super Colon, including employee photo opps which can be shared on FB.

Susan G. Komen Idaho Montana provides breast health education and services to all those living in Idaho and Montana. We work in all areas of the continuum of care including prevention, early detection and screening, treatment and quality of life. Our signature event, Race for the Cure, is held in Boise, Helena, Pocatello (new for 2016), and Coeur d’Alene each year. Join us at Race for the Cure or any of our other community events and get involved with Komen Idaho Montana.

Prevention: Living a healthy lifestyle and getting routine annual screenings are the best way to prevent, or at least detect at an early stage, breast cancer. Komen Idaho Montana encourages everyone to talk to their health care providers about annual mammography and encourages insurances companies to cover annual screenings regardless of age, when a women is working with her provider to determine the best screening schedule for her.

Early Detection and Screening: Seventy-five percent of all money raised by Komen Idaho Montana stays in Idaho and Montana to help fund our mission to end breast cancer forever. From that money Komen Idaho Montana provides community grants throughout Idaho and Montana. These grants focus on increasing access to screening mammograms, increasing access to diagnostic procedures, and providing financial assistance to those in treatment. To date Komen Idaho Montana has granted more than $6.9 million in community grant programs.

Treatment: Twenty-five percent of all money raised by Komen Idaho Montana goes to the Komen National Research Grant Portfolio to fund ground-breaking and life-saving research. Research grants are focused on treatment programs, stage IV/metastatic breast cancer, and general breast cancer research. Since 2000, Komen Idaho Montana has donated over $2.2 million to breast cancer research.

Quality of Life: Komen Idaho Montana has been a longtime supporter of survivorship programs to help transition women from treatment to life after breast cancer. Komen Idaho Montana helps facilitate support groups and funds many survivorship programs throughout Idaho and Montana.

Komen Grant: PHD saw a reduction in Komen funding from the previous year which means we are currently out resources for those who are 45-49 and are in need of routine screening.

Women’s Health Check: As of December 2015, PHD has enrolled 352 women into Women’s Health Check. Of the 30% of enrollments which are allotted for women ages 21-39, we are 60% through that funding.

Mammography Screenings: Through partnership with Heritage Health and Panhandle Health’s focus on women’s health and increasing mammography screenings, Providence Women’s Health Center Mobile Mammography Coach will now visit outlying clinics in Shoshone County for the first time in an areas where no mammography services currently exist.

PHD/CARE Foundation Cancer Prevention Project (coordinated by Erin Whitehead, HES): In December and January, Cancer Advisory Board Members conducted one-on-one interviews with area oncologists, radiologists, treatment providers, primary physicians, dermatologists, dietitians, social workers, patient navigators, current cancer patients, and cancer survivors to determine and assess what current resources exist for cancer care in North Idaho and identify barriers to access and preventative care/screenings. In January, PHD brought a B.S. student intern on board for the Cancer Prevention Project. The intern began creating a research design and compiling a list of all of the cancer resources in the five northern counties. We hope to put this information in one source, such as a website, that can be easily accessible to all providers and the community, at large. Interns are compiling cancer prevalence and risk factor data for all five northern counties. In February, plan to meet with Northwest Specialty Hospital and Cancer Care Northwest to discuss voucher program to help their low-income patients pay for their medical bills. Plan to bring on MPH student from Eastern Washington University to work with B.S. student in research design as well as in conducting more one-on-one interviews with people in or previously in treatment. Plan focus group discussions to be conducted before March 25th.

Colorectal Cancer: PHD working on taking the 80% by 2018 pledge with its employees with goal to promote and track screening of eligible employees (aged 50-75) for CRC. Natalie Colla (Comprehensive Cancer Control Contract Coordinator) to facilitate. PHD conducting provider outreach with various clinics to implement QI processes for improving CRC screening rates in primary care.

Southwest District Health (SWDH) has provided healthcare to women, including breast health care, for over 40 years. Women with marginal resources are primary customers of services, with nearly 50% being Hispanic. This has provided a foundation for the success in SWDH breast and cervical cancer program. Over the years funding from the Women’s Health Check program has helped bridged the gap in the districts efforts to provide community based breast and cervical cancer screening for women who meet the eligibility guidelines. SWDH has developed strong partnerships with providers within the districts, including mammogram facilities. Additionally we network with mobile units to improve access to screening throughout the district. Since 2013, Sixteen uninsured low income women have been diagnosed with breast cancer through the Women’s Health Check program. Without this vital program, these women would have not received services which would have put them at great risk for mortality. Many of our women have verbalized a great appreciation for the benefits this program affords.

The ICCCP is Idaho’s CDC funded program for cancer control efforts. The ICCCP is funded to work on population based initiatives under the Comprehensive Cancer Control cooperative agreement and sustainable health systems change to increase colorectal cancer screening under the Organized Approaches to Increase Colorectal Cancer Screening cooperative agreement. Comprehensive Cancer Control

Implementation of policy, systems and environmental changes to allow for sustainable/standardized cancer prevention and early detection activities. Conducted through local cancer coordinators at Idaho’s Public Health Districts. Specific activities in 2015-2016 include implementation of breast cancer reminder systems within a primary care clinic and assessment of current sun safety polices at childcare facilities.

Collaboration and engagement from cancer control professionals statewide. The CCAI is one of these collaboration avenues along with the local cancer coalitions occurring in each of Idaho’s Public Health Districts.

Implementation of evidence-based interventions to increase cancer screening. In 2015-2016, the Public Health Districts worked with a primary care clinic in their area to implement the FluFIT program. Through this effort patients who presented for a flu shot and were within the screening guidelines also received an at home colon cancer screening kit (FIT or FOBT)

Organized Approaches to Increase Colorectal Cancer Screening

Implement priority evidence-based interventions within Idaho’s Community Health Centers.

Partnership with 5 Community Health Centers (CHC) in 2015-2016. Goal is to work with all 13 CHC’s in Idaho by 2020.

Public Health – Idaho North Central District’s efforts to address cancer prevention, screening and early detection are mainly within the health promotion department. Current work includes the following:

To prevent lung cancer: - Tobacco Prevention Education for youth through Tar Wars and youth advocacy. - Tobacco Cessation classes are offered to residents throughout our 5-county area. - Policy efforts to protect nonsmokers from secondhand smoke and encourage tobacco-users to quit. - Partnering with health care providers to address tobacco use with their patients.

Colorectal cancer efforts: - PH-INCD partnered with local clinics to implement flu/FIT, which involves providing FIT kits to eligible

adults when they receive their flu shot. - PH-INCD is working with health care providers to improve colorectal cancer screening rates. - Social media messages encourage colorectal cancer screening, especially during March. Also include

messages at community events.

Skin Cancer efforts: - PH-INCD is currently contacting child care providers to complete a sun safety policy assessment. - Educational presentations to youth to discuss the importance of sun safety. - Social media messages to encourage protecting your skin from the sun. Also include messages at

community events.

Breast cancer efforts: - PH-INCD will partner with a clinic to distribute breast cancer screening reminder cards to their patients

Through the Comprehensive Cancer Control Program, South Central Public Health District works in the following cancer-related activities:

Maintains and supports a regional cancer advisory group

Partners with regional healthcare providers to implement interventions to increase screening rates for both breast cancer and colorectal cancer

Partners with regional childcare providers to assess facilities’ sun safety policies, systems and environment (PSE) activities

Provides specific outreach to clinic sites to increase colorectal cancer screening rates Additionally, through Tobacco Prevention and Control, as well as Tobacco Cessation, the District aims to reduce lung cancer incidence through the following activities:

Preventing initiation of tobacco use among youth through programs such as Tar Wars

Eliminating exposure to second-hand smoke through the development of smoke-free/tobacco-free policies

Promoting tobacco cessation and encouraging healthcare providers to increase screening of tobacco use and referrals to cessation services/classes

Conducting tobacco cessation programs tailored to youth, adults and pregnant women

Partnering within regional pharmacies to increase screening for tobacco and referrals to cessation services/classes

Finally, through offering Women’s Health Check, South Central Public Health District provides the following:

Monthly breast and cervical cancer screening clinics to women who qualify for the program, as well as coordinated care with providers for additional diagnostic testing and treatment that qualifies

Coordination for enrollment and database maintenance of all women throughout the district who are enrolled in Women’s Health Check, regardless of their provider.

Program education and updated information to new and/or existing providers

Community outreach to promote the importance of breast and cervical cancer screening

GU Navigator assists newly diagnosed patients with prostate, testicular, bladder, and kidney cancers

NCORP Grant: MSTI is part of consortium with Swedish Cancer Institute and Providence Portland Medical Center to improve access to cancer care and clinical trials

Survivorship

Recently launched a pediatric autologous stem cell transplant program

Fully implemented survivorship care plans for patients from all diagnosis types with a new inclusion of pediatric oncology survivorship visits

The Idaho Oral Health Program (IOHP) works with partners and health champions from across the state to promote the prevention and screening/early detection of oral and oropharyngeal cancer. One of the ways it specifically works towards this is by providing free educational materials to both dental and medical professionals. Some of the free educational materials available include a brochure on oral cancer that was developed in collaboration with Project Filter and the Comprehensive Cancer Program, oral cancer fact sheets, and tobacco cessation materials.

In addition to the free educational materials, the IOHP has included the following goals and strategies around preventing oral and oropharyngeal cancer in the 2015-2020 Idaho Oral Health Action Plan:

Increase the number of oral health professionals conducting oral and oropharyngeal cancer screenings during routine preventive oral healthcare visits.

Develop and disseminate culturally appropriate, targeted health messages about the risks associated with tobacco, alcohol and substance abuse, and the link to poor oral health, including oral and oropharyngeal cancer and rampant tooth decay.

Promote third party payer funding of oral health prevention activities/services by both dental and medical professionals.

The IOHP has also recently joined efforts with the Idaho Immunization Program and other programs to help increase the number of adolescents who receive the HPV vaccination.
