IMPORTANT  PLEASE READ THESE INSTRUCTIONS CAREFULLY BEFORECOMPLETING THE FORM.

APPLICATION  The applicant must: -CRITERIA 

1. demonstrate interest, dedication, commitment and ethos of service tthe community and who wish to work in the helping and caringprofessions, in areas that will assist the needy, the disadvantaged, tunder-privileged, or with the physical or social rehabilitation of the lefortunate members of thecommunity;

2. submit an essay (at least 300 words) on how he/she has demonstrainterest, commitment and record of involvement in areas specified inno. (1) above;

3. submit documentary proof of current and past two years participatioin any form of community service; and

4. has applied for Financial Aid through the NUS Office of Financial Aid(i.e. exhibit financial hardship)

SUBMISSION OF Application form must be submitted with supporting documents

APPLICATION FORM arranged in the following order:

1. Salary slips of all working family members2. Essay3. Documentary proof of participation in community-related service

All attachments to the application form must bear applicant’s name andstudent no.

RETURN ADDRESS  Please return duly completed application form to:

CLOSING DATE  Completed form must be returned by 16 April 2012.Late and incomplete applications will not be considered.
