Dear Opstein,

Please find enclosed a complete Individual Sewage Disposal System (ISDS) design for proposed church located on Highway 13, north of Rifle, Colorado, This design is based on the Garfield County Regulations and the Colorado Department of Public Health and Environment's Guidelines on Individual Sewage Disposal Systems, This letter and the attached documents combined constitute the design of the proposed system,

Bullseye Engineering, Inc, lUlderstands that the proposed project consists of a new residence and a new church building, The new residence is understood to be a 5 bedroom house, The church, for the purpose of this design, will have a congregation of up to 200 people, Also included in the church is a kitchen, intended only to be a preparation kitchen, The planned ISDS includes the following components:

• House septic tank sized for minimum 30 hour detention time: 1,500 gallon- 2 compartment tank. Install effluent filter on outlet_

• Church septic tank sized for minimum 30 hour detention time: 3,000 gallon - 2 compartment tank. Install effluent filter on outlet_

• Pump Vault, Pump, and Control Panel: o Pump Vault: 2,000 gallon single compartment tank o Pump: Pump shall have intended use for effluent pumping with a flow

rate of 21.2 gpm at 9.6 TDH (Total Dynamic Head). • Bullseye Engineering, Inc. should be notified when pump is

selected. o Control Panel: Aquaworx Control Panel or engineered approved equal.

• The absorption field shall be a trench configuration utilizing gravelless chambers, 111is design utilizes Infiltrator Systems Standard Quick 4 Chamber, however, an approved equal is acceptable. The pressure dosing lateral shall be Schedule 40 PVC pipe with orifices at the designated size and spacing. This design specifies 1.50" lateral with 3/32" diameter orifices with 28 evenly spaced orifices per lateral or 3.5 ft. spacing.

GENERAL CONSTRUCTION NOTES: I. ALL CONSTRUCTION SHALL BE IN ACCORDANCE WIllI THE STATE AND COUNTY

REGULATIONS OF INDIVIDUAL SEWAGE DISPOSAL SYSTEMS, EVEN THOUGH ALL SUCH REQUIREMENTS ARE NOT SPECIFICALLY NOTED ON THE ORA WINGS. THE CONTRACTOR SHALL BE RESPONSIBLE FOR SUCH SPECIFIC DETAILS AS ARE REFERRED TO IN THE ABOVE MENTIONED REGULATIONS.

2. INSTALL CLEAN OUTS AT ALL BENDS AND AT LEAST EVERY 100 FT. 3. INSTALL GRAVITY SEWER PIPE AT 2% MINIMUM SLOPE. 4. INSTALL RISERS ON SEPTIC TANK, CLEAN OUTS, INSPECTION WELLS, ETC. TO

PROVIDE APPROPRIATE ACCESS AT GROUND LEVEL. 5. LOCATION OF SPECIFIC COMPONENTS CAN BE ADmSTED AS NECESSARY SO

LONG AS ALL REQUIRED SETBACKS ARE MET ACCORDING TO THE GOVERNING REGULATIONS. CONTACT BULLSEYE ENGINEERING, INC. IF ADmSTMENTS ARE SIGNIFICANT.

6. PROVIDE DRAINAGE SWALES AROUND ABSORPTION AREA TO PREVENT SURFACE RUNOFF FROM FLOWING OVER ABSORPTION AREA.

7. THIS DESIGN AND PLAN DOES NOT CONSTITUTE A SEPTIC PERMIT. THE CONTRACTOR OR OWNER IS RESPONSIBLE FOR OBTAINING THE SEPTIC PERMIT FROM APPROPRIATE GOVERNING OFFICIALS.

8. BULLSEYE ENGINEERING INC. MUST COMPLETE AN INSPECTION OF THE INSTALLATION FOR ACCEPTANCE PRIOR TO INSPECTION BY COUNTY OFFICIALS OR BACKFILLING. MULTIPLE INSPECTIONS MAY BE NECESSARY TO MINIMIZE SAFETY ISSUES OR PROCEED WITH OTHER ASPECTS OF THE CONSTRUCTION. BULLSEYE ENGINEERING REQUESTS A MINIMUM OF 24 HOURS NOTIFICATION OF REQUEST FOR INSPECTION. INSPECTIONS MAY ALSO BE REQUIRED OR REQUESTED BY COUNTY OFFICIALS.

9. THIS SYSTEM IS SIZED FOR TYPICAL DOMESTIC USES APPROPRIATE TO THE DESIGN. BACKWASH OR FLUSHING FLOWS FROM REVERSE OSMOSIS UNITS OR WATER SOFTENERS SHALL NOT BE INTRODUCED INTO THE SYSTEM. OWNER IS RESPONSIBLE FOR TIGHT PLUMBING FIXTURES TO AVOID OVERLOADING THE SYSTEM.

10. INSTALL INSULATION BOARD OVER SEWER PIPE AT ALL LOCATIONS WHERE BURY DEPTH IS LESS THAN 5 FEET.

II. NO KNOWN OFFSITE WELLS ARE WITHIN THE ABSORPTION TRENCHES ZONE OF INFLUENCE. THE ON SITE DOMESTIC WELL IS MORE THAN THE REQUIRED SETBACK DISTANCE BASED ON THE DESIGN FLOW OF THIS SYSTEM.

12. TOPOGRAPHY AND BOUNDARIES PROVIDED BY OWNER. 13. APPROPRIATE BMP's SHALL BE INSTALLED AND MAINTAINED DURING

CONSTRUCTION AND UNTIL FINAL SOIL STABILIZATION OCCURS. 14. CUT AND FILL SLOPES STEEPER THAN 2.5H:IV SHALL BE PROTECTED WITH

EROSION CONTROL FABRIC. 15. IF LOCATION OF SEPTIC TANK IS TO BE FIELD ADmSTED, ADmSTED LOCATION

SHALL BE ACCESSIBLE FOR FUTURE MAINTENANCE (APPROXIMATELY 50').

3. THIS DESIGN IS BASED ON INFILTRATOR UNITS STANDARD QUICK 4 UNIT OR THE EQUN ALENT ADS BIODIFFUSER UNIT. ALTERNATIVE MATERIALS CAN BE USED WITH APPROVAL FROM BULLSEYE ENGINEERING, INC.

4. INSTALL CHAMBER UNITS ACCORDING TO MANUFACTURERS RECOMMENDATIONS. THE MATERIAL SUPPLIER OR BULLSEYE ENGINEERING CAN PROVIDE THESE DOCUMENTS UPON REQUEST.

5. DOSING PIPE SHALL EITHER BE HUNG SECURELY FROM THE TOP OF THE CHAMBER OR UTILIZE A PRESSURE DOSING PIPE SUPPORT PER MANUFACTURES RECOMMENDATIONS.

PUMP AND CONTROL PANEL NOTES: 1. Selected pump shall have intended use of pumping effluent to a pressure dosed absorption field. 2. Pump shall be selected according to design criteria indicated. 3. Bullseye Engineering, Inc. shall be notified of selected pump. Bullseye Engineering, Inc. reserves

the right to reject pump, based on sizing criteria, intended uses, and previous history. 4. Control panel shall be intended for timed dosing. 5. Control panel shall include a high water alarm, high water override start, low water override off.

The controls shall be adjustable to actual flows to include adjustments for number of doses per day, duration of dose, flow rate, etc. Bullseye Engineering, Inc. recommends Aquaworx Control Panel.

RE: Family of Faith Church - ISDS Asbuilt

Dear Mark,

This letter is being prepared to serve as your asbuilt and approval letter for your submittal to Garfield County.

Bullseye Engineering, Inc. has completed inspection(s) of the installation of the ISDS located at yom- new church construction on Hwy 13 just north of Rifle. lnspection(s) were completed on January 28 & 29 and March 10,2010. The design for this ISDS was completed by Bullseye Engineering, Inc. and signed and stamped dated January 18, 20 I O.

At the time of the inspection, all appurtenances were visible for inspection. The septic tank was backfilled to within 1 foot of the top. The septic tank that was installed was a 2,500 gallon tank (design required 2,500 gallon). At the time of inspection, the effluent filter was not installed, but instructions were given to the installers and yom-self insure installation. All sewer lines and effluent lines were installed at a minimum 2% grade. A concrete Distribution box was installed and inspected for level utilizing a level. The infiltration trenches were installed in the design configm-ation of 5 rows of23 Infiltrator Systems Quick 4 units. Based on the observations made, the ISDS meets or exceeds the State of Colorado and Garfield County regulations. Making the assumption that backfill , cleanup, grading and remaining small miscellaneous tasks (as instructed) will be completed properly, Bullseye Engineering, Inc. has completed their inspection and accepts the installation ofthe ISDS discussed here.

Subsurface Investigation and Percolation Test, Family of Faith Church Outreach 2730 Highway 13 Garfield County, Colorado

ALL SERVICE septic, LLC, performed a subsurface investigation and percolation test for the subject property, on May 13th 2009. The test was performed in accordance with Garfield County Regulations.

EXISTING CONDITIONS AND STRUCTURES

The subject property is located in a residential area where Onsite Wastewater Systems (OWS) and wells are necessary. There is an existing residence and two modular homes at the subject site. As we understand a church is being planned. The slope at the area of the percolation holes slopes approximately 5% to the east, and vegetation consists of a native grasses. A well exists in the southwest comer of the property.

SUBSURFACE CONDITIONS AND CONCLUSIONS

The subsurface was investigated by digging one profile pit and three percolation holes, at the locations indicated on Figure 1. The materials in the profile hole consisted of a 6-inch of root zone, underlain by very stiff sandy clay to 2.5-feet, underlain by medium stiff silty, sandy clay to the maximum depth explored of 8 feet. Groundwater was encountered at 7 feet. Percolation rates ranged from 40 minutes per inch (MPI) to 53 MPI, with an average rate of 45 MPI.

Results are based on field investigations and data provided by the client. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified
