BRUCE VICKERS, CFC - OSCEOLA COUNTY TAX COLLECTOR DOCUMENTATION REQUIREMENTS FOR DRIVER LICENSE AND ID CARD Effective January 1, 2010, when you visit any Tax Collector’s Office, you must bring original documents that prove your: 1) Identity 2) Social Security Number 3) Residential Address This checklist provides some of the acceptable documents. For more document options and information, www.gathergoget.com 1. Proof of Identity Bring one of the following original documents as proof of your identity: A. US Citizen Your government issued birth certificate or birth registration including territories and District of Columbia (vitalchek.com) U.S. passport Certificate of naturalization Consular report of birth abroad Note: Birth certificates from Puerto Rico must be issued after July 1, 2010. B. Immigrant I-551 (Permanent resident card), immigrant visa, temporary permanent resident stamp on passport or I-94 I-797 for I-90, I-571 or I-751 Immigration Judge’s Order (IJO), order granting asylum, refugee or EOIR42 cancellation of removal proceedings C. Non-Immigrant Proof of non-immigrant classification from the U.S. Citizenship and Immigration Services with supporting documents pertaining to your current status. (ie: I-766, I-688, I-94, INS 797, ESTA, Immigration Judge’s orders, etc.) D. Legal Name Change (if applicable) If you are coming in to change your name, you must first update with the SS Administration Office. An original or certified copy of all marriage certificates or court orders linking name change(s) from birth to current name. (photo copies are not acceptable). A U.S. passport in your current legal name may also be presented. *Please allow 24 to 48 hours after visiting the SSA for your name to be updated in the system. 2. Social Security Number Bring one of the following original documents: Social Security Card W-2 form Pay stub showing name and complete SSN Any 1099 form Note: If no social security number has been issued an additional secondary form of identification will be required. 3. Proof of Residential Address Two proofs required.
