There are steps each one of us can take — some big, some small — to improve our health, both as individuals and as a company. Reading this guide is an important step.

In this guide, you will find a detailed listing of many of the health and wellness programs and tools that compose the Health Rewards program and the health improvement portions of the JLL Health platform. These programs and tools are available at no cost as part of the UnitedHealthcare medical plan to employees of JLL and their enrolled dependents. All of the programs are completely voluntary and 100 percent confidential in accordance with the law. These programs are designed to help you improve your health and reduce your health care costs.

Please review this guide as often as needed. It is helpful to familiarize yourself with these resources, so you can quickly access any of the tools when you need them most. Just click on the At-a-Glance table (pages 3–4) to quickly find the information you’re looking for.

If UnitedHealthcare finds that you or one of your enrolled dependents might benefit from participating in an available support program that you haven’t already self-enrolled in, a nurse or engagement specialist may call you directly. In order to protect your privacy, you may receive a message similar to the one below:

Neonatal Resource Services (NRS)

of stay and avoid readmission.

The Network Provider must notify NRS if the newborn’s NICU stay is longer than the mother’s hospital stay.

START YOUR BABY ON A HEALTHY TRACKIf you plan on adding to your family, myBLUEPRINT4HEALTH is here to support you.NEW! Cancer Support Program (CSP)If you or a covered dependent have cancer, you can receive benefits for oncology services provided by Designated Facilities participating in the Cancer Support Program (CSP) program. For oncology services and supplies to be considered Covered Health Services, they must be provided to treat a condition that has a primary or suspected diagnosis relating to cancer.

Cancer clinical trials and related treatment and services are covered by the Plan. Such treatment and services must be recommended and provided by a Physician in a cancer center. The cancer center must be a participating center in the Cancer SupportProgram at the time the treatment or service is given.

Congenital Heart Disease (CHD) Resource ServicesManaging Congenital Heart Disease just got easier thanks to the Congenital Heart Disease Resource Services program. The program guides patients and their families to the nation’s top Congenital Heart Disease centers, pays benefits for congenital heart disease surgeries ordered by a Physician and information that helps in selecting a care center and making informed decisions.

A team of specialized Congenital Heart Disease nurses provides a comprehensive set of services, including:

• Condition awareness• Disease and treatment education• Treatment Decision Support• Access to Centers of Excellence network• Patient empowerment and support• Intensive case management, when appropriate

Support is provided through all stages of treatment and recovery, to help patients make informed treatment decisions and improve their health care experiences

Improve your health and earn money. Serious health conditions may be delayed or prevented when you know your numbers, know your risks and make healthy lifestyle changes.

Meeting target biometric values not only improves your health, but also helps you earn a Health Bonus under the Health Rewards program. You earn dollars for the biometric values that you meet. Employees, and their spouse/domestic partner, can each earn up to $725, with the potential for additional award opportunities based on age, gender and health status.*

HOW TO GET YOUR HEALTH REWARDS IN 2016Know the numbers that matter. Complete a screening for four biometric numbers—BMI (body mass index), LDL cholesterol, fasting blood sugar or hemoglobin A1c, and blood pressure—at a JLL onsite screening event, if available. You can also complete your biometric screening during your preventive visit at your doctor’s office, at a convenience care center located in many retail stores or at a participating LabCorp facility. Make sure you request the screening ahead of time, since you will need to fast.

Complete a Fitness Challenge. The American College of Sports Medicine advises adults to complete at least 150 minutes of physical activity per week, consisting of both cardiovascular and strength- training activities.

Earn a free Fitbit by completing the recommended 150 minutes of physical activity per week on average over the course of 6 weeks. You can use your Fitbit whenever you’d like (and are encouraged to as well!), but you need to wear it during the dates of one of the four challenge periods to earn your incentive. If you already own a Fitbit, you can earn $100 for completing these Health Rewards opportunities.
