General Information
The Boise Interagency Dispatch Center Aviation Mishap Response Plan provides direction to
dispatchers, aviation managers, and fire managers as to how to respond to various types of aviation
mishaps. Use the flow charts and checklists provided in this plan to assist with any and all aviation
incidents.
Types of Aviation Mishaps and Definitions
Aircraft is late for a scheduled check in - Aircraft misses a scheduled position check, which could
either be from their signal disappearing on Automated Flight Following (AFF) or missing a radio
check-in.
Overdue Aircraft - Aircraft fails to arrive at the planned destination within 30 minutes past the
estimated time of arrival (ETA) as shown on the flight plan or 30 minutes past the designated radio
check-in time.
Missing Aircraft - Aircraft is considered missing when it has been reported to the Federal Aviation
Administration (FAA) as being overdue and administrative search (radio/phone) has been conducted
without success. An aircraft is officially missing when the fuel duration, as reported on takeoff or the
flight request, has been exceeded and the location of the aircraft is unknown.
Aircraft Accident - An occurrence associated with the operation of an aircraft, which takes place
between the time any person boards the aircraft with the intention of flight and all such persons have
disembarked, and in which any person suffers death or serious injury, or in which the aircraft
receives substantial damage.
Substantial damage - Damage or failure which adversely affects the structural strength,
performance, or flight characteristics of the aircraft, and which would normally require major repair
or replacement of the affected component. Engine failure or damage limited to an engine if only one
engine fails or is damaged, bent fairings or cowlings, dented skin, small punctured holes in the skin
or fabric, ground damage to rotor or propeller blades, and damage to landing gear, wheels, tires,
flaps, engine accessories, brakes, or wingtips are not considered “substantial damage” for the
purpose of this part.
Aircraft Incident - An occurrence other than an accident, associated with the operation of an
aircraft, which affects or could affect the safety of operations.
Incident with Potential (IWP) - An incident that narrowly misses being an accident and in which
the circumstances indicate significant potential for substantial damage or serious injury. The USDA-
FS National Aviation Safety and Training Manager or the OAS Aviation Safety Manager, as
appropriate, will determine final classification.
Serious Injury - an injury which:
1. Requires hospitalization for more than 48 hours, commencing within 7 days of the mishap;
2. Results in a fracture of any bone (except simple fractures of fingers, toes, or nose);
3. Causes severe hemorrhages, nerve, muscle, or tendon damage;
4. Involves any internal organ; or
5. Involves second or third degree burns, or any burns affecting more than 5% of the body.
Non-Agency Aircraft Events If Boise Dispatch Center receives a report of an aircraft mishap that involves commercial, military or
civilian aircraft, notify the FAA Western Regions Operations Center at (425)227- 1999 and the
Idaho State Emergency Communication Center at (208)846-7600.
Search and Rescue The County Sheriff is responsible for all search and rescue operations, and recovery. Coordination
with the Sheriff’s office is essential for SAR and site security.
Priority of Actions As soon as you are aware of a mishap or a potential mishap START A LOG OF ALL ACTIONS
AND CALLS.
1. Protect People - Life saving operations take first priority.
2. Protect Property - Property should be protected from unnecessary additional damage.
3. Preserve Evidence - Treat the area as if it were a crime scene and provide 24-hour security
until the investigation team arrives. Identify witnesses, get their addresses and phone
numbers.
4. Notify and investigate - Report the accident using your organization’s chain of command and
policies. Do not delay reporting if detailed information is not immediately available.
5. Recovery Operations - Everything at the site is under the control of the NTSB until released.
Anyone who has ever been involved in the immediate response to an aircraft accident will agree that
the first few minutes (and hours) are chaotic. Developing and practicing your Aviation Mishap
Response Checklist today is your best defense against the chaos of tomorrow. Time is an
extremely critical factor and immediate positive action is necessary; delay may affect someone’s
survival
Aircraft Late for Scheduled Check-In / Lost Contact or AFF Signal
* Frequencies to try are Local Flight Follow, National Flight Follow, Air Guard, and the command channel,
if on an incident.

Attempt to contact aircraft on all radio frequencies until positive contact is
established or an accident has been confirmed (Try National FF, Air Guard,

Notify Floor Supervisor that you have lost contact and have tried all
frequencies
Contact other aircraft to see if they can see or contact the aircraft on A/A
Contact ground units, lookouts, helibases, to see if they have a visual or can
contact the aircraft on A/G
Contact all available phone numbers for personnel on the aircraft
Check with the aircraft’s starting location/dispatch to see if they have
contact with the aircraft
Check with the aircraft’s destination location/dispatch to see if they can see
or have heard from the aircraft
Start to gather information for the Overdue Aircraft Information Sheet and
the Aircraft Accident Report in case it is needed
Aircraft is 15 Minutes Past Check-In

If a search with agency aircraft is initiated, inform the county Sheriff Office
for the affected counties
Contact the vendor home base
Contact FAA/Lockheed-Martin Flight Service Station 1-800-992-7433
and request an Alert Notice (ALNOT). Select “1” to speak to a briefer, give
the briefer the info from the Overdue Aircraft Information Form and your
contact info
Notify the Great Basin Coordination Center
*If an overdue aircraft is located and there is no emergency situation re-notify all parties contacted
during the initial search and cancel the overdue aircraft alert. Once all information about what
happened is collected complete and submit a SAFECOM.

Missing Aircraft
If the aircraft exceeds its fuel duration given at takeoff or if an accident is known or suspected the
following actions need to be taken.
If the accident occurred under supervision of an Incident Management Team, the Air Operations Branch
Director (AOBD) will notify BDC in order to coordinate the notification and accident response process.

Other Agencies Involved:
Boise Smokejumpers Boise smokejumpers utilize Boise Dispatch Center for flight following services when they perform
practice jumps, spotter training, or transition from Boise to another base. These activities can occur
between the months of March to October. There is a Project Aviation Safety Plan (PASP) in the
2019 PASP binder. For more information, see the Smokejumper Users Guide.
Aircraft Incident: If there is an aircraft incident, utilize the steps in this guide for that situation
(Overdue Missing, Aircraft Accident). Use the notification procedures for the agency whose land is
involved with the incident. For example, if the aircraft goes missing over the Boise National Forest,
you would use the FS notifications or if the aircraft had a hard landing on BLM land, you would use
the BLM notification procedures. For accidents that occur on private land follow the BLM
notification procedures and work with the Center Manager for additional notifications that need to be
made. In addition to the agency notifications, you must notify the Boise Smokejumpers
Operations/Duty Officer at 208-387-5426 / SMJ DO cell 208-850-5144
Medical Incident: If there is a medical incident involving a smokejumpers utilize the notification
procedures for the agency who’s land the medical incident occurred on. In addition to the agency
notifications, you must notify the Boise Smokejumpers Operations/Duty Officer at 208-387-
5426 / SMJ DO cell 208-850-5144
Refer all media calls for incidents involving jumpers (who are not on a local incident) to NIFC
External Affairs at 208-387-5895.
OAS Aircraft Boise Dispatch Center will occasionally flight follow Office of Aviation Services (OAS) aircraft.
When this occurs, the pilot should fax in a WRO Flight Following form (Appendix I in the DOI
Office of Aviation Services Western Region Office 2019 Aviation Plan). Their Aviation Plan is
located in the 2020 PASP binder in dispatch.
If there is an incident involving an OAS aircraft utilize the steps in this guide for that situation
(Overdue, Missing, or Aircraft Accident). Contact the supervisors that are listed on the WRO
Flight Following form and notify them of the incident. Next use the notification procedures for
the agency whose land is involved with the incident. For example, if the aircraft goes missing over
the Boise National Forest you would use the FS notifications or if the aircraft had a hard landing on
BLM land you would use the BLM notification procedures. For accidents that occur on private land
notify the county dispatch center and the supervisor listed on the form.
Transitioning Aircraft There is a large number of aircraft that transfer in and out of the Boise area and utilize Boise
Dispatch Center for flight following. If there is an incident involving a transition aircraft, consider
the following information to determine the appropriate agency notification procedures. Which
agency’s land is involved, which agency owns/procured the aircraft, and what was the aircrafts
mission. Notify the Center Manager and the appropriate agency’s Unit Aviation Officer to
determine what other notifications need to take place. For accidents that occur on private land notify
the county dispatch center and follow the notifications procedures for whichever agency the aircraft
belongs to. Private aircraft on private land should be handed over to the county dispatch.
ALL MEDICAL COMMUNICATIONS AND NOTIFICATIONS ARE THE
PRIORITY REGARDLESS OF WHAT KIND OF INCIDENT HAS OCCURED
Mid-Air Emergencies Mid-air emergencies can include fire on board, mechanical failure or malfunction, shortage of fuel,
worsening weather, aircraft damage, and more. Emergency situations can lead to loss of altitude,
diversion to a nearby airport, or a forced landing. During most mid-air emergency situations, the
aircraft will contact either the closest FAA tower or the closest dispatch center. Due to the many
variables that can occur during a mid-air emergency there is no step by step guide of what to do.
Below are some general guidelines to follow.
When an aircraft calls in distress they may use the word “Mayday” or call in with their call sign and
tell you they are encountering an emergency situation. If this occurs, clear that frequency for
emergency traffic by saying,
“Attention all units clear this channel for emergency traffic.”
1. Get as much information as possible from the aircraft but REMEMBER the pilot’s priority is
to handle the situation and land safely, so they may not respond right away.

• Type of aircraft
• Position or estimated position (stating which)
• Heading (true or magnetic)
• True or indicated air speed
• Altitude
• Fuel remaining
2. Repeat the information back to the pilot to reduce the chances of errors in the
message.
3. Plot the last known location of the aircraft.
4. Notify the closest FAA tower of the emergency situation and any actions the pilot needs
taken by the tower. (Boise Tower: 208-364-5823)
5. Notify the floor lead, the unit aviation officer, and the center manager. Consider these
questions when doing notifications, which agency’s land is involved, which agency
owns/procured the aircraft, and what was the aircrafts mission?
6. Continue to provide as much assistance as you can to the aircraft.

• Contact Idaho State Communications with location (Lat/Long) and details of
incident. They will determine closest air ambulance to send with an estimated
time frame.
• Inform the Incident Commander (IC) of air ambulance’s time frame and IC will
determine whether the use of air ambulance is the most expeditious method to
use for transport.
• IC may choose to use another method of transport, e.g. incident helicopter at the
incident, upon receiving the air ambulance’s time frame.
