ProgramDescription

BlakeHouseLLCoperatesasanon-profitundertheumbrellaofPathwayToHope,a501(c)3corporation.Itprovideslow-cost,affordablehousingforindividualsdiagnosedwithmentalillnessandabletoliveindependently.ResidentsmayliveatBlakeHouseaslongastheywouldliketo,barringanyunforeseencircumstancesthatwouldpreventthemformdoingso. Foronefee,theresidentreceivestheirroom(couldsharewithoneotherperson),food,accesstolaundryfacilitiesandsupplies,allutilitiespaid,cableTV,internetaccess,andabeautifullivingenvironment.Allresidentsparticipateindoingthevariouschores(fromshoppingforfoodtocleaningthebathrooms)andinchoosingthemenuandcookingtheeveningmeals(Monday-Friday).Theyarefreetocomeandgoastheypleaseandweaskthattheysigninandoutsothatweknowwheretheyare,shouldtherebeaneedtocontactthem. AllofourresidentshavecasemanagersthroughJohnsonCountyMentalHealth'sCommunitySupportServices(CSS).Assuch,CSSandtheiremployeesareresponsibleformanagingtheirmedicationsandforprovidingcasemanagementservices,suchascounseling,therapy,andvariousprogramsneededbytheclienttosucceedandmeettheirgoals.NooneassociatedwithBlakeHousedispensesmedicationorofferscounselingorcasemanagementtoourresidents. WedohaveaResidentManagerinplacethatprovidessupporttothehousefortheresidents.Theyareavailabletotakethemgroceryshopping,checkontimelinessofchorework,andencouragehealthyactivitiesinthecommunity(suchasbowling,goingtothemall,etc.),andjustlistenshouldaneedarise.Ifsomethingshouldhappenandoneofourresidentsisinneedofpsychiatricservices,wehaveaprocedure/protocolforhertofollow,suchascallingthePathwayToHopeExecutiveDirectorortheJohnsonCountyMentalHealthMobileCrisisResponseTeam(JCMHMCRT).Resident’sareexpectedtoabletofunctionreasonablyindependently.Theymustbeagreeabletolivinginacommunityenvironmentwheretheywillhaveregularinteractionwithotherresidentsandpeopleinthecommunity,asappropriate.
