RE: ISSUANCE OF CERTIF ICATE AUTHORIZ ING REGISTRATION AND TAX CLEARANCE

This refers to our application for a Certificate Authorizing Registration (CAR) and Tax Clearance for the donation executed by _________________________.

We received from your office an ONETT Computation Sheet for Donor’s Tax (Annex “A” hereof) and was assessed Donor’s Tax in the amount of THREE MILLION FOUR HUNDRED FORTY SIX THOUSAND THREE HUNDRED SIXTY FOUR PESOS AND 11/100 cents (PhP 3,446,364.11). We paid the Donor’s Tax by depositing such amount with the Bank of the Philippine Islands, as evidenced by the BTR-BPI Deposit/Payment Slip (Annex “B-1” hereof), on ___________________and filed the corresponding Donor’s Tax Return (Annex “B”) on the same date. We were then issued a CAR Claim Slip (Annex “C”) by your good office.

After repeated follow ups, we have yet to receive the CAR and the Tax Clearance from your office. We are now, by way of this letter, requesting for your prompt action in issuing the CAR and Tax Clearance so that our client, Ms. Leticia Martinez, may cause the issuance of a new Transfer Certificate of Title in favor of her Donee, Maria Jolina Martinez, the soonest.
