Lamotrigine is thought to act primarily at voltage-sensitive sodium channels to stabilise neuronal membranes and inhibit neurotransmitter release, principally the excitatory amino acids glutamate and aspartate. In this respect it is more similar to antiepileptic drugs such as carbamazapine and phenytoin (as compared to drugs which work primarily through gabaergic mechanisms such as valproate). A number of other possible cellular and mechanisms have been investigated, including actions at calcium channels, monoamine reuptake sites and intracellular signal transduction pathways (e.g., protein kinase C - a possible site of action of lithium). Despite these intensive investigations, the precise mechanism(s) by which lamotrigine exerts efficacy in bipolar disorder remains unknown.
