Microambiente tumoral:- Células T, células B, células
natural killer (NK), células dendríticas, macrófagos, neutrófilos e mastócitos
- A maioria na transição do tumor/tecido normal e estruturas linfoides terciárias (TLSs)

Signaling pathway
PD-L1 is a type 1 transmembrane
protein (B7-H1) that belongs to
the B7 ligands family
PD1 binding with PD-L1 or PD-L2Activation of inhibitory kinases involved in T-cell proliferation, adhesion, and cytokine production/secretion
PD-1–PD-L interaction has been shown to play an important role in limiting the initial response of T cells upon antigen exposure and inducing Tcell tolerance.
Expressed on dendriticcells, macrophages, mast cells, T-cells andB-lymphocytes andnonhematopoieticcells, includingendothelial, epithelialand tumor cells

Biomarkers: perspectives
- Mismatch repair deficiency and microsatellite instability1) Mismatch repair deficiency (dMMR) leads to the accumulation of
mutation as well as production of potential neoantigen2) Enhanced treatment effect resulting from MSI-H/dMMR is
attributed to increased density of TIL, elevated TMB, upregulated PD-L1 expression, and more potent tumor-specific immune response
Le DT et al. Science. 2017;357:409-413.Yi M et al. Mol Cancer. 2018;17:129.
- Tumor mutation burden1) Accumulated mutations with increased potentiality of neoantigen
results in elevated immunogenicity2) Clonal mutations (shared by all tumor cells) and subclonal mutations
(expressing on a fraction of tumor cells) affect tumor specific immunity differently
3) Single-site biopsy might overestimate level of clonal mutation due to the interference from subclonal mutation which might explain the poor response of some patients with high TMB

• Quais alterações considerar no cálculo?• Somáticas? Não sinônimas + indels + sinônimas?
• Qual tamanho do exoma tem que sersequenciado? • WES – TAT, R$, tamanho do tecido
• Paineis menores• Entre 1.5-3Mbp pode dar informação adequada
Teste commercial* (F1CDX)
• TMB alto >=20mut/mb• TMB intermediário 6-19 mut/mb• TBM baixo <6 mut/mb
estimativa do TMB)- Padronização do escore- Interpretação

- Gut microbiota1) The alteration of microbiota composition might change the tumor immune microenvironment by
the homing and recirculation of lymphocytes2) analyzing gut microbiota composition would be favorable to predict treatment effect of anti-PD-
1/PD-L1 therapy3) Though the exact modulatory mechanism is unclear, many factors are proposed to enhanced
tumor control
Sivan A et al. observed that fecal microbiome transplantation could d restore the sensitivity to anti-PD-L1 treatment and improve anti-tumor activity in non-responding mice.
Gopalakrishnan V et al. noticed the relationship between high abundance of Faecalibacterium genus and elevated response rate in patients receiving anti-PD-1 treatment.
Dysbacteriosis caused by utilization of antibiotics was proved to influence the efficacy of anti-PD-1/PD-L1 therapy. And the poor response to agents could be reversed by recolonization of Akkermansia muciniphila(Routy B et al, 2018).

- Combining PD-L1 immunohistochemistry with T cell activation measuresmight boost the ability to predict patient response to checkpoint inhibitors
- Multiplex gene expression (NanoString Technologies)

However, whether these newer paradigms will be superior to currently available immunohistochemistry assays is unclear.
Biomarkers: perspectives
Mechanisms of main biomarkerspredicting efficacy of PD-1/PD-L1 inhibitors1) PD-L1 status reflects adaptive
immune resistance which istherapeutic target of PD-1/PD-L1 inhibitors
2) dMMR and MSI-H correlates strongly with high TMB. TMB enhances theimmunogenicity
3) tumor infiltrating lymphocyte(TIL) represents potentialimmune surveillance whichcould be reactivated by agentes. Specific gutmicrobiota promotesdifferentiation of T cell, as well as lymphocyte homingand recirculation

Despite these reservations, PD-L1 IHC still has the ability to enrich for benefit, and it is currently the only approved and widely applied biomarker in NSCLC for the immunotherapy.
PD-L1 expression on TCs or ICs is heterogeneous, meaning that sampling error during tissue acquisition may be a significant factor that is more likely to lead to underestimation rather
than overestimation of PD-L1 expression.
PD-L1 IHC is not a perfect biomarker. Although PD-L1 is effectively the target of the drugs in question, PD1–PD-L1 interaction is only one of many factors that may determine the eventual
outcome of a potential tumor-directed immune response.
