Bid 18CSL0216 Page 1 of 5 EXHIBIT A DESCRIPTION OF GOODS AND SERVICES This bid is for HVAC services at the Connecticut State Library facilities in Middletown and Rocky Hill, Connecticut. HVAC services to include scheduled preventive routine maintenance, seasonal start up and shut down, emergency and unscheduled services as described in sections 1, 2, 3 and 4. This bid is for the two years period April 1, 2018 to March 31, 2020. 1. SCHEDULED PREVENTIVE ROUTINE MAINTENANCE REQUIREMENTS A. Each of the two locations on this Contract shall receive two (2) annual preventive maintenance service calls from the Contractor. B. At any time after the receipt of a purchase order from the Client Agency and prior to the Spring Service, the Contractor shall meet with the Client Agency’s designated representative and determine the exact dates for each of the service calls. i. Spring Service (Cooling start-up): Perform all items on the Task List contained in Section 5 of this Exhibit A including seasonal start-up or shut-down where stated. Ensure proper functioning of cooling applications. ii. Fall Service (Heating start-up): Perform all items on the Task List contained in Section 5 of this Exhibit A including seasonal start-up or shut- down where stated. Ensure proper functioning of heating applications. C. The Contractor’s service technician shall complete a service report after each of the two preventive maintenance service calls. This service report shall be provided to the Client Agency’s designated representative, who shall verify the work performed. A duplicate record shall be maintained at the Contractor’s site to update the history of the work performed. D. The Contractors service technical shall complete a service report after all other service. EXHIBIT A

DESCRIPTION OF GOODS AND SERVICES

This bid is for HVAC services at the Connecticut State Library facilities in Middletown and Rocky Hill, Connecticut. C. Emergency Service response calls placed by the Contractor shall be made only to the Client Agency’s designated representative.

D. Prior to starting any unscheduled or emergency repair work, the Contractor shall provide a time and material estimate to the Client Agency using labor rates as stated on the bid form STO-93.

E. The Contractor’s service technical shall not begin any unscheduled or emergency

repair work until the Client Agency’s designated representative authorizes such work by affixing his/her signature to a properly executed work order.

3. AGENCY INSPECTION

A. The Client Agency’s designated representative reserves the right to inspect all of the Contractor’s work that is performed under the terms and conditions of this Contract. After inspection, all work that is found to be outside the scope of manufacturer’s recommendations, or that has in anyway corrupted or compromised the functional integrity of the temperature control and mechanical systems, shall be promptly corrected at no additional charge to the Agency.

4. SUBCONTRACTORS DAS must approve any and all subcontractors utilized by the Contractor prior to any such subcontractor commencing any work. Contractor acknowledges that any work provided under this Contract is work conducted on behalf of the State and that the Commissioner of DAS or his/her designee may communicate directly with any subcontractor as the State deems to be necessary or appropriate. It is also understood that the Contractor shall be responsible for all payment of fees charged by the subcontractor(s). A performance evaluation of any subcontractor shall be provided promptly by the Contractor to DAS upon request. The Contractor must provide the majority of services described in the specifications. Nothing herein shall be construed to create privity of contract between the State and any subcontractor.
