MayCaring Awards 2016This 5th edition of our annual Caring Awards embraces and recognises the commitment, courage, compassion and grace of staff, caregivers and patients from our foundation partners. More than 600 awards were given out this year to the heroes in our regional health system.

2016

JulyCharity Art Exhibition in Aid of HomeCare AssistDr Andrew Peh, Senior Consultant and Chief of Psychological Medicine held a charity art exhibition at CGH to support HomeCare Assist. Sixteen paintings were sold over the 3-day exhibition, raising a total of $52,100.

NovemberEHA Scientific Meeting Focuses on “Transitions of Care”The meeting addressed the need for better integration of care across various care and life settings. Workshops were also conducted to build capability and capacity in community nursing and programme evaluation.

Expansion of Eye Services at SNEC Eye Clinic @ CGHCGH and SNEC signed an MOU to provide patients with a wider range of specialised eye care services at the SNEC Eye Clinic @ CGH, with particular focus on the ageing population in eastern Singapore. Patients will also be able to receive new treatment possibilities through multi-disciplinary programmes developed in collaboration with other clinical departments in CGH.

SeptemberB-PREP to Build Psychological Resilience for EmployeesThe Stay Prepared – Business Psychological Resilience Programme (B-PREP) was jointly launched by CGH and the Singapore Business Federation, in partnership with Temasek Foundation Cares, to help businesses and employees cope with the psychological and emotional impact of emergencies, threats, and crises.

First Ready-to-Eat Texture-Modified Asian Meals CGH announced the launch of its ready-to-eat texture-modified meals for people with dysphagia and signed a licensing agreement with local specialist nutrition company Health Food Matters to produce and distribute the meals. People with dysphagia can now enjoy a variety of dishes instead of bland porridge for their regular diet.

January EHA, CGH and SUTD Renew MOUEHA, CGH and the Singapore University of Technology and Design (SUTD) renewed their strategic alliance to develop solutions for better healthcare delivery, nurture future generations of healthcare engineers, and cultivate healthcare innovation in Singapore.

New Eastern Community Health Centre at Our Tampines HubEHA, SACH and SingHealth Polyclinics extended their collaboration to provide services such as diabetic foot and eye screening, and physiotherapy at the new Eastern Community Health Centre (CHC) at Our Tampines Hub. This enables Tampines residents to access these services at a convenient location, with a GP referral.

February

First Human Clinical Trials of ELO WaterCGH and Hyflux announced the first human clinical trials of Hyflux’s ELO Water for diabetics. The trials seek to determine the effectiveness of Hyflux’s ELO Water and ELO Gel on diabetes control and diabetic foot ulcer.

MarchExpansion of Neighbours for Active Living to Entire South East DistrictNeighbours for Active Living expanded to the entire South East District with the inclusion of Mountbatten. The programme has been strengthened with funding support from Standard Chartered Bank.

Health Peers Programme to Prevent and Manage DiabetesEHA and South East Community Development Council jointly developed and launched the Health Peers Programme, supported by Dr Maliki Osman, Mayor of South East District. The programme equips community leaders and volunteers, known as Health Peers, to reach out to fellow residents who are at-risk or diagnosed with diabetes, to lead healthier lives.

The past six years have been extremely fruitful as we endeavored to keep our community healthy and provide our patients and their caregivers a seamless journey across the continuum of care.

With the population continuing to age and chronic diseases becoming more prevalent, there is a pressing need to establish an effective continuity of care experience that looks beyond simply delivering health care to helping our population to keep well, get well

and age well. We have also worked on enabling and empowering our population to gain knowledge and skills to take charge of their health.

Since its inception, EHA has been at the forefront of community care, providing a suite of services to support residents in the community in the east and strengthen care for them. These programmes have steadily gained increasing traction among residents. Signature programmes such as the Eastern Community Health Outreach (ECHO) and Neighbours for Active Living Programme (Neighbours) have grown by leaps and bounds and now cover all constituencies in the east and beyond.

Screening services by the ECHO team help our residents to better understand their health status so that they can better manage their health. The number of residents screened with follow-up by primary care doctors for those with abnormal results has increased. Meanwhile, Neighbours provide holistic support to those who have returned home but remain vulnerable and have complex medical and social care needs and has helped to decrease their number of hospital admissions. EHA has also built a strong primary care support network to reach out to patients who have returned home. Programmes such as GPFirst and Health Wellness

Eastern Health Alliance (EHA) celebrates its sixth anniversary this year as the regional healthcare system serving the needs of the eastern Singapore population.

Programme make care (including psycho-emotional support) more accessible to patients in the community.

Through telehealth, we are looking to bring care to patients within the comfort of their home. Our Health Management Unit (HMU) nurse telecarers have been instrumental in extending the personal touch through regular communication with our patients. They also equip patients with knowledge and skills to self-monitor and self-manage their conditions. Through this process, patients have reported increased confidence in taking care of their own health and found assurance in knowing that help is always there for them.

All these would not have been possible without the strong network of partners and communities that support our vision of ‘Towards Seamless Quality Care – Together’ and drive our mission of ‘Partners for Better Health’.

With the recent reorganisation of the public healthcare system, EHA will be integrating into SingHealth next year. I am confident that the services and programmes established will continue to grow from strength to strength as we tap on a larger pool of resources and extend the benefits to a wider group of recipients.

It has been an exciting six years since we launched EHA in 2011. The journey has been extremely fulfilling as we grew stronger together with our partners to provide holistic care for the eastern community. I would like to thank our staff and partners for their contributions and efforts and look forward to continuing to deliver care that matters to our patients and their caregivers as part of our enlarged SingHealth family.

GROUP CEO’S MESSAGE

This year, we continued to help our population keep well, get well and age well, in line with the Ministry of Health’s Beyond Healthcare 2020 Masterplan.

KEEP WELLAs part of the war on diabetes, our ECHO team extended its community health screening programme to all 17 constituencies in eastern Singapore and equipped participants with knowledge on healthy living. The follow-up rate by general practitioners for participants with abnormal results has

doubled since ECHO’s introduction in 2011. We also launched the Health Peers programme to help diabetics, or those at risk, make lifestyle modifications and achieve their health goals.

GET WELLOur Valued Care Programme reduced the length of stay for hip fracture patients by 27 per cent in CGH and 18 per cent in St. Andrew’s Community Hospital (SACH), saving costs for patients while increasing bed availability. We have doubled the proportion of hip fracture patients with surgery done within 48 hours, with nearly 90 per cent of patients mobilised within the first post-operative day. Similarly, redesigning the care model and infrastructure in CGH’s Integrated Building has resulted in improved mobility and functionality for our geriatric and rehabilitation patients.

AGE WELLOur Neighbours for Active Living programme has built strong links between health, social and community partners to create an enabling ecosystem for residents to age in place. This year, our community nurses were deployed as teams to specific geographical zones so that they could gain deeper understanding of the population’s needs in their respective areas and be more accessible to residents in providing services from

preventive care to chronic and end-of-life care. These services are complemented by personalised chronic disease education and support via telephone by our Health Management Unit. We also launched Careline, a 24/7 phone support service that takes a proactive and preventive approach to help residents stay safe and age well at home.

INNOVATIONOur Centre for Innovation (CFI) and Centre for Healthcare Assistive and Robotics Technology (CHART), continued to forge strong partnerships with healthcare providers, academia, research and economic agencies. Our strategic alliance with the Singapore University of Technology and Design yielded three patents from our first tranche of grants. We also developed the first ready-to-eat texture-modified Asian meals for elderly with dysphagia and the SEEK MyHealth Kiosk®, the first self-help health monitoring kiosk in the community. In partnership with Abbott, we established the Nutritional Health for Elderly Reference Centre to develop nutritional solutions that are tailored to Asian preferences and needs. CHART has played a key institutional and national role in technology road-mapping, providing user insights to co-develop healthcare solutions and test-bedding to validate effectiveness of solutions.

These achievements would not have been possible without our primary care, community, public and private sector partners. I especially thank our foundation partners, SACH, The Salvation Army Peacehaven Nursing Home, SingHealth Polyclinics, and Health Promotion Board, for their unwavering support and immense contributions.

The journey has been challenging but extremely rewarding. As we officially join the SingHealth family on 1 January 2018, we look forward to strengthening our existing partnerships and collaborating with new partners as we learn and grow together. Our focus remains clear: to deliver care that matters to our patients and community, and joy and meaning at work for our staff.

AWARDS & RECOGNITION

CARING AWARDS 2016The award celebrates staff, patients and caregivers who have made a difference in the lives of others through their commitment, courage and compassion. 679 exceptional healthcare staff, patients and caregivers were recognised at the 5th iteration of the Caring Awards ceremony for their excellent care and service as well as their courage and determination in the face of adversity. The Guest of Honour was Mr Chee Hong Tat, the then-Minister of State for Communications and Information & Health.

COMMUNITY CHEST AWARD (SHARE PROGRAMME)CGH strongly supports the Community Chest programme, with 77 per cent of employees being active contributors to SHARE. In 2016, CGH received the Community Chest SHARE Platinum Award while EHA received the SHARE Bronze Award.

CUSTOMER SATISFACTION INDEX OF SINGAPORE (CSISG) 2016CGH ranked first, along with Khoo Teck Puat Hospital, among the public general hospital in the CSISG 2016, a key national survey which measures customer satisfaction across several industries. This was the third consecutive year that CGH topped the chart.

HEALTHCARE HUMANITY AWARDS 2016The Healthcare Humanity Awards salutes exemplary healthcare professionals who go beyond the call of duty to help others. PRESIDENT’S AWARD FOR NURSES 2016 Ms Hanijah Abdul Hamid, Assistant Director (Nursing Education) received the highest accolade in the nursing profession in recognition of her exemplary contributions to the nursing profession and the community.

She has been instrumental in the implementation of clinical guidelines in CGH to improve patient care and the development of evidence based training programmes. As an educator, she has continued to engage and inspire nursing students and junior nurses to grow in their nursing career.

Ms Hanijah is also involved in helping nursing homes in the east develop their capabilities to meet the Enhanced Nursing Home Standards and provide quality care.

HR EXCELLENCE AWARDS 2016EHA received the Gold Award for Excellence in Workplace Wellbeing.

NATIONAL DAY AWARD 2016Forty-two exceptional staff from EHA and CGH received the National Day Award 2016 for their commendable public service. A/Prof Low Cheng Ooi, Senior Consultant, Orthopaedic Surgery received the Public Administration Medal (Silver). Other winners include two Commendation Medal recipients, six Efficiency Medal recipients and 33 Long Service Medal recipients.

NURSES’ MERIT AWARD 2016Nine EHA staff received this accolade from the Ministry of Health for their outstanding performance and dedication to the nursing profession. The nurses from CGH are Senior Nurse Manager Jane Caroline Vander Straaten,

> A/Prof Tan Su-Ming, Senior Consultant and Director of Breast Centre, CGH received the PS21 Star Service Award for providing excellent patient care and achieving consistently high patient satisfaction ratings.

> The Neighbours for Active Living (Neighbours) programme was awarded the PS21 Best Practice Award (Inter-agency Collaboration).

> EHA Primary Care Integration team’s initiative to transform the primary care landscape in eastern Singapore through EHA’s partnership strategy led to them receiving the PS21 Best Practice Award (Stakeholder Communications and Engagement).

At the national level, she played an important role in developing and implementing the National Nursing Taskforce’s recommendation for Advanced Clinical Education.

PATIENT SATISFACTION SURVEY (PSS) 2015 The MOH PSS assesses patient satisfaction, and identifies areas for improvement in public healthcare institutions. CGH was the most improved hospital in the survey with a record score of 82.5 per cent. This is close to a 12 per cent increase from the score in 2014. More than eight out of 10 patients rated their experience at CGH as ‘Good’ or ‘Excellent’.

PS21 EXCELLENCE THROUGH CONTINUOUS ENTERPRISE AND LEARNING (ExCEL) AWARDS The PS21 ExCEL Awards Ceremony recognises teams and public officers who best exemplify the spirit of innovation and continuous learning. EHA and CGH won four awards in 2016:

> The Kaizen Everyday Engagement Programme (KEEP) was awarded the PS21 Best Practice Award (Organisational Design).

EHA and CGH won four awards in the 2016 PS21 ExCEL Awards Ceremony for their spirit of innovation and continuous learning.

EHA’s focus on preventive healthcare takes the form of health screening activities in the community through the ECHO programme, as well as strong partnerships forged with healthcare providers in the region.

KEEPING RESIDENTS WELL

the east to build a robust regional health system with a strong primary care base. Our GP partners appreciate being involved from the early stages of programme development resulting in good buy-in. Today, 83 per cent of GPs in the east have participated in at least one EHA programme.

ECHO SCREENING PROGRAMME EXTENDS ITS REACHECHO is a community-based chronic disease prevention programme aimed at early detection of risk factors, as well as intervention to prevent or delay the onset of disease. It offers screening at affordable rates and at easily-accessible locations, as well as activities to equip Singapore citizens and permanent residents with skills and knowledge to lead healthy lifestyles.

Partnership with the Pioneer Generation Office and other Grassroots Organisations enabled the ECHO team to increase participation in its screening programmes from 4,300 participants in 2015 to more than 6,500 participants in 2016. By the end of FY2016, the collaboration has been extended to all 17 constituencies in eastern Singapore.

In FY2016, several strides forward were made in fostering greater awareness of healthy living among residents, as well as promoting greater collaboration with general practitioners and other service providers.

MORE GENERAL PRACTITIONERS (GPS) PARTNER EHA Since 2011, the Primary Care Integration team has been actively engaging GPs in

MORE SUPPORT FOR HEALTH CARE PROFESSIONALS TO MANAGE MENTAL HEALTH IN THE COMMUNITY EHA enhanced its Health Wellness Programme to reach out to a wider group of patients with psycho-emotional conditions while growing its network of primary healthcare providers with specific interest in mental wellness. The team has conducted training workshops on basic counselling skills, sleep hygiene, relaxation techniques, and psychological reactions to physical illness for primary healthcare professionals and social service agencies managing mental health within the community. It has also published a guide entitled “Effective Psychological Interventions in Primary Care”, and has produced a quarterly e-newsletter on mental health topics for primary healthcare providers.

HEALTH PEERS PROGRAMME ON DIABETES PREVENTION AND MANAGEMENT LAUNCHEDEHA and South East Community Development Council jointly developed and launched the Health Peers Programme on 7 January 2017, supported by Dr Maliki Osman, Mayor of South East District. The programme equips community leaders and volunteers with skills and knowledge on diabetes prevention and management as part of

the War on Diabetes. The curriculum was developed with input from community leaders and expertise from our healthcare professionals including a sports medicine physician, dietitian and clinical psychologist.

Health Peers are trained to interact with residents who are at risk or who have been diagnosed with diabetes. They support them in making lifestyle modifications, as well as setting and achieving health goals. The first group of 50 Health Peers trained included ITE College East nursing students and grassroots leaders from Siglap Diabetes Warriors, resident committees, neighbourhood committees, and executive committees.

Health Peers are trained to interact with and support residents who are at risk or who have been diagnosed with diabetes.

EHA aims to transform care by enhancing services and creating seamless links between acute care in a restructured hospital, recovery and rehabilitation in a community hospital, and care in the community once a patient has returned home.

physiotherapy at the CHC to enable Tampines residents to access these services at a convenient location, with a GP referral.

WIDER RANGE OF EYE SERVICES WITH LAUNCH OF SNEC EYE CLINIC @ CGH CGH and the Singapore National Eye Centre (SNEC) launched the SNEC Eye Clinic @ CGH on 11 November 2016 to deliver more specialised eye care services, particularly for conditions such as cataracts, glaucoma, corneal diseases, and retina-related conditions such as diabetic retinopathy. The clinic serves to enhance eye care services that have already been provided at CGH by SNEC clinicians since 2002, with a particular focus on the ageing population in eastern Singapore. Patients at the Eye Clinic will also benefit from CGH’s capabilities in managing complex acute and chronic medical conditions. For example, patients with diabetic retinopathy can see a doctor for the condition at the SNEC Eye Clinic @ CGH, in addition to regular check-ups at CGH’s Diabetes Centre to ensure that their diabetes is well controlled.

ENHANCED NEUROLOGY AND NEUROSURGERY SERVICES AT CGHMore neurology and neurosurgery services are being offered with the opening of the NNI Neuroscience

Clinic @ CGH, which builds on the long-standing partnership between CGH and NNI. In addition to our patients, healthcare professionals will also benefit from this collaboration, as CGH and NNI will be enhancing the training of doctors, nurses, and allied health professionals, in neurosciences.

The tele-stroke service at CGH was also enhanced by streamlining workflows to improve outcomes of stroke patients. In FY2016, 343 tele-stroke conferences were done in the A&E, representing a 23 per cent increase over the previous year. 37 per cent of these patients were given rTPA, a drug that breaks down blood clots.

MORE HIP FRACTURE PATIENTS GET FASTER ACCESS TO SURGERYThe Valued Care Hip Fracture Programme saw an increase in the number of patients on the pathway, from 329 in the period December 2014 to November 2015, to 389 from December 2015 to November 2016. This multi-disciplinary programme has increased the percentage of hip fracture patients that receive surgery within 48 hours, while decreasing the total length of stay in the hospital.

care needs, who are at risk of frequent admissions to the hospital. The volunteers provide social support, and assist with simple but important tasks such as reminding clients to take their medication and attend medical appointments.

TRANSITIONAL CARE SERVICE SEES POSITIVE RESPONSEThe Transitional Care service at CGH enrolled 420 patients in FY2016 with positive feedback from patients and their families. The nurse-led service

aims to reduce readmissions to hospitals, re-attendance at emergency departments, and hospital length of stays and associated costs, by ensuring patients’ health and social needs are met in the first three months of their return home. Patients on the service demonstrate improved health outcomes, preference for being treated at home, reduced pain and anxiety, as well as psychological and social benefits of being treated in their own home.

INNOVATION,PARTNERSHIPS & RESEARCH

In January 2017, the team added hip fracture to the suite of chronic conditions that are covered under the service. These patients are jointly managed by the newly established Osteoporosis Liaison Service team, set up by the CGH Department of Endocrinology, to prevent secondary osteoporotic fractures. CGH is the first hospital to involve telecarers in improving osteoporosis management for its patients.

CENTRE FOR INNOVATIONThe EHA’s Centre for Innovation (CFI) is a healthcare innovation incubator that delivers patient-centric solutions by forging partnerships to supply

HEALTH MANAGEMENT UNIT Since 2010, the Health Management Unit (HMU) has been looking after patients in the east with long-term illness or chronic conditions upon discharge from the hospital. Telecarers ensure patients’ compliance to treatment plans and monitor their progress. They also educate patients on their condition to help build confidence in managing the disease independently.

Technology TransferIn FY2016, EHA out-licensed two technologies to local small and medium-sized enterprises (SME):

1. First ready-to-eat texture-modified Asian meals created for those with swallowing difficultiesPeople with swallowing difficulties (dysphagia) as well as the elderly who prefer soft meals can now enjoy the first ready-to-eat texture-modified meals in local flavours, created by CGH. Launched in September 2016, these meals fill a gap in the market to meet the dietary needs of those with dysphagia while providing meals that appeal to the local palate. They are available in three textures – finely minced, coarsely minced, and blended – to ease swallowing. The meals will be produced and marketed under a licensing agreement signed between CGH and local specialist nutrition company Health Food Matters (HFM).

2. Self Enabling and Empowering Kiosk (SEEK) MyHealth Kiosk® licensed to AIT TechnologiesThe SEEK MyHealth Kiosk® is a self-help kiosk for health monitoring created for use in community and polyclinic settings. It arose out of a collaboration

between EHA and SingHealth Polyclinics. In FY2016, the SEEK MyHealth Kiosk® was successfully licensed to interactive solutions company AIT Technologies Pte Ltd for upscaling and bringing the technology to market.

Collaborations and Partnerships

1. Partnership with SPRING to help SMEs develop healthcare solutionsEHA was awarded the Capability Development Scheme (Industry) Grant Partnership for Capability Transformation (PACT) by SPRING to bridge the needs of patients and healthcare professionals with potential solutions by industry experts. In 2016, there were three technology adoptions, five co-development projects and two test-beddings/ trials arising from the SPRING PACT programme.

2. Nutritional Health for the Elderly Reference Centre launchedEHA, CGH and Abbott Manufacturing Singapore Pte Ltd signed a Master Project Agreement on 1 March 2017 to establish a Nutritional Health for Elderly Reference Centre (NHERC). Funded by the Singapore Economic Development Board, NHERC aims to build knowledge in elderly nutrition in the Asian population. EHA, CGH and Abbott will also work with SingHealth

Polyclinics to conduct Asia’s largest clinical study of its kind to evaluate the effect of nutrition management in the elderly in Singapore. The study will be conducted at the newly set up centre located within CGH.

3. EHA renews partnership with the Singapore University of Technology and DesignEHA, CGH and the Singapore University of Technology and Design (SUTD) renewed their strategic alliance through a Memorandum of Understanding (MOU) to develop solutions for better healthcare delivery, nurture future generations of healthcare engineers, and cultivate healthcare innovation in Singapore. Under the first MOU, a total

of $2.25 million was committed to the CGH-SUTD HealthTech Innovation Fund in which 13 joint projects were awarded. These projects in turn yielded three patents – Body Fluid Drainage Device, Lung Simulation Model for Education and BWard Real-time Blood Sensing and Wound Monitoring Device.

CENTRE FOR HEALTHCARE ASSISTIVE AND ROBOTICS TECHNOLOGY APPOINTED AS LEAD OF THE TECHNOLOGY ENABLING WORKGROUPThe Centre for Healthcare Assistive and Robotics Technology (CHART) was appointed by the Ministry of Health (MOH) in August 2016 to lead the Technology Enabling Workgroup consisting of hospital representatives across all public healthcare institutions (PHIs) in Singapore. The workgroup supports the implementation of technology within each institution to improve manpower productivity in healthcare delivery. Protoypes such as bed transporters and mobile hoists were shared with the wider healthcare sector through events such as the Healthcare Productivity Technology Showcase, organised by CHART and supported by MOH in February 2016. These devices would allow healthcare professionals to focus more on direct patient care related activities by relieving them of tasks that could be automated. Several

hospitals have committed to procure these devices.

EDUCATIONIn line with the move towards more inter-professional, collaborative and team-based approach to patient care, the Medical, Nursing and Allied Health Education Offices came together to introduce a programme to help enhance systems-based practice, develop communities of practice and help healthcare professionals learn to be more effective team members with other health professions.

The Nursing Education Office launched a new Graduate Nurse Integration Programme in FY2016 to enable newly graduated nurses to transit better from a classroom setting into a fast-paced ward environment. A clinical preceptorship framework was set up for experienced clinical facilitators to act as an immediate resource and provide support in their respective clinical areas for preceptees and other preceptors. Formal monitoring and documentation were introduced to track progress and the acquisition of clinical competencies and skills. A preceptee discussion forum was also introduced to facilitate a safe environment for confidential discussion of their challenges and concerns at work. A similar forum targeting perceptors was also organised to

achieve the same objectives. Through this approach, support for new nurses can be more focused and targeted to their specific needs, which in turn will help them to adapt and mature fast in a new environment.

Annual Scientific Conference Focuses on “Transitions of Care” The EHA Scientific Meeting 2016 addressed the increasing focus on integration of patient care in Singapore. More than 50 local and international speakers discussed the theme “Transitions of Care”, covering the full spectrum of healthcare setting from acute to primary, to intermediate and long-term care. A brand new track on healthcare education was introduced to look into building and growing capacity for clinical and scientific excellence.

RESEARCH

First Human Clinical Trials of ELO Water on DiabetesCGH signed an agreement with Hyflux on 15 February 2017 to conduct two clinical trials to establish the effectiveness of Hyflux’s ELO Water and ELO Gel on diabetes control and diabetic foot ulcer patients. The trials will study whether patients with diabetes can obtain better glycaemic control by drinking ELO Water in addition to their ongoing lifestyle

and medical treatments, and whether diabetic foot or ankle ulcers can be treated with ELO Water bath and ELO Gel.

Injury Prevention for SailorsIn May 2016, Changi Sports Medicine Centre (CSMC) published research that shed new light on sailing-related injuries and illnesses. The study was commissioned by World Sailing and is the biggest study conducted on elite sailors. The findings from the study showed a need to develop injury prevention programmes and devices. Currently, CSMC is collaborating with SUTD and Singapore Sailing to develop injury prevention devices for sailors.

Early Intervention for Trauma PatientsCGH’s Trauma Recovery and Corporate Solutions (TRaCS) team has been providing early psychological intervention to patients admitted to CGH since 2007. During the 6th Biennial Trauma Conference held in June 2016, the team shared data from eight years of screening which showed that only one in 16 patients (6 per cent) required follow-up treatment for post trauma mental health conditions such as post-traumatic stress disorder, depression, and anxiety disorders. In comparison,

internationally published research studies showed that this number was one in 10 patients globally. The findings suggest that early intervention contributes to reduced risk of developing post trauma mental health disorders.

COLLABORATIONS

Building Up Psychological Resilience for EmployeesCGH and the Singapore Business Federation, in partnership with Temasek Foundation Cares launched the Stay Prepared – Business Psychological Resilience Programme (B-PREP) on 7 September 2016. The aim of the programme, led by TRaCS, is to conceptualise, develop and implement a model for psychological resilience in the corporate environment. Companies in Singapore can incorporate the model into their business continuity plans to manage psychological and emotional impact of emergencies, threats and crises. This programme, supported by the Temasek Emergency Preparedness Fund, helps employees develop core skills and competencies as first responders by providing psychological support in crisis situations at the workplace.

Integral Role in the Sports Eco-SystemCSMC works closely with organisations such as the Football Association of Singapore, Singapore Sports School and Sport Singapore, to provide national athletes quality-assured and prompt sports medicine care. Our sports physicians have served as team physicians for Team Singapore in the Rio 2016 Olympics and Paralympics, as well as the 2017 Special Olympics World Winter Games. They also served as chief

medical officers for international sporting events such as the FORMULA 1™ Singapore Grand Prix and ONE Fighting Championship.

EHA was officially launched in 2011 as the Regional Health System for residents in eastern Singapore. Over the past six years, we have, through our programmes, partnerships and people, established a firm presence in our community to enable our residents to keep well, get well and age well.

Grace Corner, a 32-bed facility in The Salvation Army Peacehaven Nursing Home is set up to provide focused, lower-intensity rehabilitation to help clients with fractures and strokes regain maximum mobility to return home and live as independently as possible. It is the first transitional convalescent facility in a nursing home and is run by Peacehaven in partnership with CGH geriatric medicine doctors.

ECHO programme is piloted to offer health screening, coaching, and healthy lifestyle activities for residents in the Changi-Simei constituency.

EHA signs a Letter of Intent with CGH and Geisinger Health System to launch the Valued Care Programme, creating a partnership for a transformative process from a volume to a value-based healthcare delivery system

HPB officially joins the EHA family as the fifth foundation partner.

The Centre for Innovation (CFI) and Changi Simulation Institute (CSI) are officially launched. CFI provides a platform and resources for ideation, prototyping and partnerships for healthcare innovation. More than 132 GP clinics across eastern Singapore signed up for the pilot scheme. EHA and SingHealth Polyclinics

jointly launch Singapore’s first self-help kiosk, “Seek MyHealth Kiosk®” to help residents in the community monitor their health.

Senior Minister of State for Health Amy Khor launches HomeCare Enterprises, a social enterprise under EHA that provides affordable homecare products.

The Centre for Healthcare Assistive and Robotics Technology (CHART) is officially launched. CHART is a platform for healthcare professionals to work closely with academia, industry and research institutions to create technology-based healthcare solutions.

Neighbours is officially launched. Jointly developed by EHA and the South East Community Development Council, Neighbours combines healthcare, social expertise and resources to train volunteers to care for the elderly with high care needs in their own neighbourhoods.

ECHO and Neighbours programmes expand to all neighbourhoods in the East.

The 280 bed Integrated Building (IB), jointly run by CGH and SACH, is officially opened. The new model of care at the IB optimises rehabilitation for patients to help them achieve optimal functionality and recovery.

Changi General HospitalChangi General Hospital (CGH) is an award-winning public hospital with over 1,000 beds serving a community of 1.4 million people in eastern Singapore. Health Promotion Board The Health Promotion Board (HPB) was established as a statutory board under the Ministry of Health, Singapore, in 2001, with the vision of building ‘A Nation of Healthy People’.

HPB aims to empower the people of Singapore to attain optimal health, increase the quality and years of healthy life, and prevent illness, disability and premature death.

As the key driver of national health promotion and disease prevention programmes, HPB spearheads health education, promotion and prevention programmes, as well as creates a health-supportive environment in Singapore.

It develops and organises relevant health promotion and disease prevention programmes, reaching out to the healthy, the at-risk, and the unhealthy at all stages of life, from children to adults and older Singapore residents. St. Andrew’s Community Hospital St. Andrew’s Community Hospital (SACH) provides rehabilitative, sub-acute palliative, dementia and continuing care for adult and paediatric patients after their hospitalisation for acute care at a general hospital. SACH aims to help patients recover and regain daily living functions before they are discharged back into the community.

The Salvation Army Peacehaven Nursing HomeThe Salvation Army Peacehaven Nursing Home provides compassionate, individualised,

holistic care to all without discrimination. Established in 2000, Peacehaven has a capacity of 401 beds, and is run by The Salvation Army, an international Christian movement. Peacehaven offers a comprehensive gamut of services to care for the sick, frail and elderly who have physical or mental disabilities with minimal or no appropriate caregiver support at home. Its residents have varying degrees of mental or physical disability, and the majority are either from destitute or low-income families. The needs of the residents are met in a holistic manner, with services ranging from clinical care (such as medication management, wound dressing, and colostomy care) to therapy (from occupational and recreational therapists) advice from allied health professionals (such as dietitians, geriatricians and pharmacists).
