SETTING UP THE NIGHT BEFORE

DAY/NIGHT BEFORE ELECTION

We encourage you to work with your polling location to arrange entry the day or night before the election to do those things you can, if at all possible. Those things that you can do are as follows:

• Position tables and chairs;• Put up postings and outside distance marker(s);• Set up the voting booths; and• Daisy chain the voting booths together.

REMEMBER: The ePollbook and the JBC must remain with the Judge at all times. The Judge will bring these items with him/her Election Morning. The JBC remains sealed overnight. Anything you need for setup is in the outside zipper pouch (e.g., caddy key).Connecting the voting booths: be careful when you connect the cables from booth to booth so that you do not bend any pins. Below are examples of what bent pins look like. Bent pins can cause a variety of problems including completely rendering the system inoperable. BE CAREFUL when making the cable connections booth to booth. Line up the connectors and take your time. Rushing or trying to “make it fit” will lead to bent pins.

Securing the voting booths: if you set up the night before, seal the booth lids with the yellow seals provided in the Judge’s Kit. Record seal numbers on the back of the JBC Reconciliation Log (JBC Flipchart Step 2).

ELECTION MORNING

ELECTION MORNINGElection Morning is very hectic. Expect it. Get to the polling location early (no later than 6:00 AM)

Do the following:

• Any Judge or Alternate Judge who has not already completed the statement of Officer and taken the Constitutional Oath of Office for this election must do so immediately (located in the Payroll folder);

• Give ALL Poll Workers the Oath (located on the payroll form);• Have each Poll Worker fill out the payroll form and sign;• Set up the ePollbook (turn on & log in), complete or begin equipment

set up (see ED Setup & Operations Guide for help);• Laptop, scanner, label printer turned on and operational; confirm

label printer is ON and working; • Fill out all forms and seal logs;• Attach the JBC to the string of voting booths and turn on;• DAU is last unit with a chair nearby, headphones plugged in and

volume turned up;• Booths open and connected, booth numbers assigned, JBC tapes verified,

ZERO Tape printed and signed, booth numbers hanging and privacy screens open;

• Open polls on the JBC (no one can vote until 7AM);• Postings (in the Postings folder in Judge’s Kit) hanging so voters can see

them, including all pages of Sample Ballot;• Distance markers outside at 100’, no campaign signs or candidates within distance markers;• All poll workers wearing name tag and no workers wearing campaign or party buttons, bracelets, etc.;• Judge cell phone(s) out, turned on, and volume turned up;• Statement of Residence Forms and pens ready for voter use;• Combination Form(s) completed on top, signed, dated and on table FACING voter with pens available;• Register of Election Day Voters (Label Sheet) at JBC;• eLookup Tablet and instructions are out and easily accessible if needed;• Ask Poll Workers to practice with tablet and review procedures before needed; and

• Confirm with Poll Workers that they did not park in voter parking.

The check-in table is the station where voters are qualified through eVote. At the very least you should have the ePollbook, ID Guide and Combination Form(s) on the check-in table. EVERY VOTER WITH A PRINTED LABEL SIGNS THE COMBINATION FORM. Depending upon the election type, your setup may be slightly different, as represented below:

The voter’s first label is placed on the Combination Form. The voter signs and, in the case of a Similar Name voter, initials. The voter’s signature indicates that he/she is the voter whose information is printed on the label. The voter’s initials, if initialing, indicate that the voter agrees to the Similar Name Affidavit located at the top of the Combination Form.

NOTE: In a Primary or Primary Runoff Election you will have two separate Combo Forms as shown below. You will have a Combination Form for each Party at the precinct, 1 Democratic (BLUE) and 1 Republican (WHITE). You will need to use the stamps provided in the kit to mark each Combo Form with the correct party.

NOTE: There will also be other Combination Forms in the Judge’s Kit as follows: GREEN Provisional, WHITE Omissions, PINK Curbside Voting. Always look at the top center of the form for the words “COMBINATION FORM” and verify the type of form as listed below the words “COMBINATION FORM.” example

FORMS-Label Sheets

eVote prints 2 labels for each voter. The voter takes the second label to the JBC Operator. The JBC Operator will use this second label to issue an Access Code from the JBC, either by scanning the barcode on the label or by using the information on the label to manually issue an Access Code, as instructed for the type of election. Voter DOES NOT sign the Label Sheet.

NOTE: In a Primary or Primary Runoff Election you will have two separate Label Sheets as shown to the right. You will have a Label Sheet for each Party at the precinct, 1 Democratic (PINK) and 1 Republican (WHITE). You will need to use the stamps provided in the kit to mark each Label Sheet with the correct Party.

The process at the JBC should be as follows:

1. Voter gives label to JBC Operator2a. In NON-Primary Election:

JBC Operator scans barcode on label to automatically issue an Access Code

2b. In Primary or Primary Runoff Election:JBC Operator uses info on label to manually issue Access Code for correct Party

3. JBC Operator checks Ballot Style/Party on Access Code against label:

a. If Ballot Style/Party matches give Access coCode to voter, or;b. If Ballot Style/Party DOES NOT match start over and issue correct Access Code

4. Place label in next open space on Label iiiiiiSheet

NOTE: In a Primary or Primary Runoff Election, where you have one sheet for each Party, if you put a label on the wrong sheet, mark the label out and reprint a label from eVote. Then put the reprinted label on the correct Party Label Sheet.

PRIMARY or PRIMARY RUNOFF Elections: These types of Elections are governed by the Election Code and have different rules and forms from Non-Primary Elections. The “Statement of Compensation and Oaths” is filled out by every Poll Worker at the site. Two separate forms should be maintained, one for each Party and labeled as such. Fill out all spaces provided including signature and Social Security number.

The $15.00 delivery fee for returning the supplies and JBC is for the entire precinct, not per person. It may be split, but the total of the amounts for all workers cannot be more than $15.00.

In addition to the payroll form, you will also find a Training Pay form. Neither the Parties, nor the State, pay for training in a Primary or Primary Runoff Election. This form will ensure that you are paid by Election Central after the election. Double check these forms for accuracy and completion.

Mistakes and omitted information will cause a delay in pay.

FORMS-Other Affidavits

OTHER AFFIDAVITS & FORMS

Affidavit of Deceased Voter: To be used when a voter (to the second degree of consanguinity (grandchild, sibling, grandparent)) informs the clerk that a relative has recently died.

NOTE: A good practice in this case is to look up the deceased relative in eVote or the eLookup tablet and see if they have a voter record. If you cannot find them in either, Election Central has already taken the necessary steps and this form is not required.

Affidavit of NON-Participation and Affiliation: To be used ONLY in a Primary Runoff Election. In a Primary Runoff, a voter must vote in the Party in which he voted in the previous Primary Election of the same year unless he did not vote in the Primary Election. If the voter believes the Party as indicated on the voter record is wrong, he must fill out this Affidavit to vote in a different Party. After the voter has filled out and signed this form, in eVote, go to the Judge’s menu, then to Cancel Party Affiliation, and follow the on-screen prompts.

NOTE: If the wrong label has already been printed for the voter, you will need to call Election Central to have this corrected before proceeding any further.

Precinct Problem Form: To be used in the event of any issue that happens at the polling location. Documentation of an issue is often the first thing looked at when investigating a problem and can help with determining a solution if one is needed.

Bilingual Questionnaire: To be used in every election to comply with Federal and State laws. This form MUST be filled out before returning to Election Central. All parts of this form are required, even if the answer is “0.”

Oath of Assistance and Interpreter: To be used if a voter requires assistance or an interpreter from someone other than an election official. *This form also tells you what assistance is and who is eligible to receive it. NOTE: If the purpose for using the form is that the voter has brought in a person to assist him, this person must take the oath before assisting the voter and must sign the Combination Form below the voter’s signature. Poll Workers need NOT complete this form as they are covered by the Oath on the payroll form. It is a requirement on Election Day that a voter be assisted by 2 Election Officials at the Polling Place.

STATEMENT OF RESIDENCE

FORMS-Statement of Residence

The yellow Statement of Residence (SOR) Form MUST be filled out before a voter votes. There are two instances in which a voter may be required to do so: 1. the voter has a yellow background in eVote when looked up, or 2. the voter, upon being asked if he still lives at the address as displayed on the Address Display Monitor, responds that he has moved.In either case the recommendation from Election Central is to take the voter out of the line and have him complete the form and then bring the voter back and allow him or her to ‘cut’ in line.It is extremely important that the Statement of Residence Form be filled out completely and that the form is signed by the voter. Otherwise, in subsequent elections, the voter will be required to fill out the form again. This is frustrating to the voter and polling place personnel alike. Voter Registration will also have problems updating the voter’s record. A RESIDENCE ADDRESS IS REQUIRED.

ADDITIONAL NOTES:

Certificate #: if you have this #, perhaps the voter has his voter registration card, record the number here. VUID, found in eVote, could also be printed here.

Middle Name: while optional, if it appears on the ID the voter presents, the voter should include it on the form, as this will be the name that is entered in the voter record.

Residence Address: This box MUST be completed. If it is not, the voter will remain on suspense.

County: the voter must still reside in Montgomery County. In Non-Primary Elections, CALL ELECTION CENTRAL to verify Ballot Style.

Signature: the voter MUST sign the form.

DOUBLE-CHECK THE FORM FOR

COMPLETENESS

In eVote a blue background and the words “ATTENTION Ballot Mailed” mean that the voter requested and was sent a Ballot by Mail. This requires the voter to see the Judge to fill out the Request to Cancel Application for Ballot by Mail form. This form MUST be filled out before the voter is allowed to vote.The form itself is fairly simple. The voter either has the ballot in hand or does not.

Depending upon whether the voter has the ballot or not determines your next actions: • If the voter HAS the ballot in hand, the voter will vote as normal after giving it to you and

filling out the form. Follow the onscreen prompts in eVote by pressing the Surrender Ballot button.

• If the voter DOES NOT have the ballot in hand, the voter will have to vote a provisional ballot after filling out the form.

REQUEST TO CANCEL BALLOT BY MAIL

FORMS-Request to Cancel Ballot by Mail

In eVote a blue background and the words “ATTENTION Ballot Mailed” mean that the voter requested and was sent a Ballot by Mail. This requires the voter to see the Judge to fill out the Request to Cancel Application for Ballot by Mail form. This form MUST be filled out before the voter is allowed to vote.The form itself is fairly simple. The voter either has the ballot in hand or does not.

Depending upon whether the voter has the ballot or not determines your next actions: • If the voter HAS the ballot in hand, the voter will vote as normal after giving it to you and

filling out the form. Follow the onscreen prompts in eVote by pressing the Surrender Ballot button.

• If the voter DOES NOT have the ballot in hand, the voter will have to vote a provisional ballot after filling out the form.

At a minimum every voter must complete the top of the form including:

• Name• Election• Date of Election• Signature

IF the voter HAS the ballot the voter MUST sign the STATEMENT.

Election Day voters DO NOT fill out Affidavit A, B or C.

The bottom section is to be completed by the Election Day Judge and signed.

FORMS-Provisional Forms

PROVISIONAL COMBO FORM & AFFIDAVITThe provisional process has several steps that must be completed correctly. An instruction sheet for processing provisional voters is included with the provisional packets in the kit. The voter is always given the Notice to Provisional Voter and, in the case of a voter who does not have acceptable Photo ID or complete the RID procedure, it will include a label with the hours of operation for the “Cure Period” in which the voter may come to Election Central and provide to the Voter Registrar acceptable Photo ID or complete the RID procedure in order for the provisional vote to be counted. NOTE: The Provisional Affidavit Envelope is also an application for those voters NOT registered.

this envelope.After following the instructions in the JBC Flipchart to issue a Provisional Ballot Stub from the JBC, the signed portion of the JBC Tape is put in the green provisional envelope. IMPORTANT: Write

the Ballot Code from the JBC Tape on the front of the provisional envelope.

The final part of the process is to write the voter’s info on the “List of Provisional Voters” located on the orange provisional ballot bag and then to put the sealed provisional envelope inside the orange provisional bag. At the end of Election Day, after polls have closed, you will seal this bag for return to Election Central.

In a Primary each Party processes their own provisionals.

The JBC Reconciliation Log, aka the JBC Envelope, is located in the zipper pouch on the outside of the JBC Box. You will use it throughout the day. Keep it close by. In a Primary or Primary Runoff vote totals are split by Party. The most often missed items on the JBC Envelope are: numbers under CLOSING THE POLLS section, signatures, and putting it in the zipper pouch on the outside of the JBC Box when returning to Election Central on Election Night.Below is a simple guide on which boxes need to be filled out:

FORMS-Reasonable Impediment Declaration (RID)

Reasonable Impediment Declaration (RID)A voter who has NO ID and does not possess ID but is registered to vote may be given a regular ballot after filing out a Reasonable Impediment Declaration and showing a supporting form of ID. This could apply to a voter who has lost his or her ID or the ID has been destroyed or stolen. It may also apply to a voter who simply does not possess and cannot obtain an acceptable form of ID. Once this process has started you may not question the reason or the truthfulness of the reasonable impediment given on the RID Form.

Voter may present a supporting form of ID AND complete and sign a Reasonable Impediment Declaration (RID).Voter must show an original or copy of:• Valid Voter Registration Certificate• Certified Birth Certificate - Must be an original• Current utility bill – original or copy (you may not specify a

date)• Bank statement – original or copy• Government check – original or copy• Paycheck – original or copy• Copy of or original government document with name and

address (Must be original if it contains a photograph) Examples: Driver License from other state, ID card issued by federally recognized Native American tribe, DPS Receipt without a photo, expired Voter Registration Certificate, or expired TX DPS-issued Driver License or Personal ID Card (over 4 years).

CANNOT accept: Social Security Card (no address), public (not private) college or university ID without an address (Lone Star, Sam Houston, U of H, Texas A&M, UT, etc.), state/federal employee ID card without an address, library card without an address.

NOTE: The address on the Supporting Form of ID does NOT have to match the address in eVote. DO NOT vote a Provisional Ballot if the voter is registered in your precinct.

CLOSING POLLS & Return to Election Central

CLOSING POLLS & RETURNING TO ELECTION CENTRALClosing polls consists of the following 3 things: 1. Closing the Polling Location, 2. Closing Polls on the JBC, and 3. Closing eVote and the ePollbook case.

CLOSING THE POLLING LOCATIONALL voters in line at 7PM MUST be allowed the opportunity to be qualified and vote. It is recommended that you send a clerk to be the end of the line at 7PM. After the LAST voter has VOTED begin closing polls on the JBC.

CLOSING POLLS ON THE JBCRefer to the JBC Flipchart for precise instructions to close polls on the JBC. After closing polls on the JBC you may begin disconnecting the voting booths. The booths will be put back into the caddy. Print all reports from the JBC as specified by the JBC Flipchart and start filling out the JBC Reconciliation Log. You will need numbers from: the JBC Tally Tape, eVote Daily Totals, PUB Count on JBC, and all Combination Forms (Curbside, Provisional, Regular, etc.) used during Election Day.

CLOSING EVOTEYou will need the Daily Totals from eVote to write on the JBC Reconciliation Log. To get this click on Judges and then Daily Totals. The Daily Totals are now displayed in the green box on the right in eVote.Once you have the Daily Totals you can proceed with shutting down the laptop as instructed in the JBC Flipchart. DO NOT remove USB Encryption Key from laptop. Leave plugged in.

PACKING UP THE EVOTE EPOLLBOOKAfter the laptop has shut down you will put each device back in its designated spot. For a

complete diagram refer to the ED Setup & Operations Guide in the “Closing the Polls” section. It should look like this when everything is packed properly. The final step here is to put on the lid and secure the five latches. The 25ft power cord can be rolled up and placed on top of the palm rest.

PACKING UP THE JBC

The JBC scanner gun goes inside the JBC Box lid and is attached with

velcro.

NOTE: the scanner gun is removed in a Primary or Primary Runoff Election, so you would not have it to put back in JBC Box.

The ONLY item in the zipper pouch is the JBC

Reconciliation Log.

Use the Election Central Envelope to help you get everything in the right place and back to Election Central at the end of the night!

*In a Primary Election there will also be an envelope for the Judge of each Party.

Do not delay returning to Election Central for any reason.
