NORTH CAROLINA DEPARTMENT OF PUBLIC INSTRUCTION

In order to ensure confidentiality, and in accordance with standard security measures, I agree that I will

not discuss the content of the specific review of a Teacher’s ASW Evidence Collection with anyone other

than those specifically authorized by the North Carolina Department of Public Instruction.

I agree to destroy any notes used to evaluate or review the ASW Evidence Collection reviewed. I will not

retain any of the material reviewed or duplicate any of the material reviewed.

I understand that the material herein is the property of the North Carolina Department of Public

Instruction, the Local Educational Agency and the teacher submitting the material and as such will keep

all information regarding this review confidential.

If it does not follow the Strands and Standards Guidance Chart, then please put the appropriate Evidence Collection Level comment in the box: E07, E11, E13. Then continue to step #2.

If the evidence does not align with the chosen objective:

If the evidence is aligned, then examine the work samples, Context, and any Supporting Documents. Then continue to step #5.

For Whole Class Work Sample, this Timelapse Artifact’s evidence should be rated “Does Not Meet Expected Growth” and the appropriate Timelapse Artifact Level comment (A03 and/or A70) is written in the Comment box. Then continue to step #5.

For Student Work Sample, this evidence does not meet expected growth. Examine evidence #2 and #3 to check for similar misalignment. If 2 or more evidences are misaligned, this artifact should be rated “Does Not Meet Expected Growth” and the appropriate Timelapse Artifact Level comment (A03 and/or A70) is written in the Comment box. Then continue to step #5.

8 When finished rating all of the Timelapse Artifacts in an Evidence Collection, use the Collection Rating Form to assign an overall rating.

TIPS: 1. Make sure you have a place where you can work deliberately.

2. Review when you are well-rested and energized.

3. Take a break if you are tired or find yourself rushing. 4. When deciding between two ratings, review all materials carefully and remember calibration training.

5. Consistently follow the procedures in this document.
