Purpose
Uterine aspirators provide constant high-vacuum suction (at least 400 mm Hg) and high flow through a fine
curet to evacuate fluid and tissue from the uterus. Uterine aspiration is used primarily for dilation and evacuation
(D & E) therapy for terminating early pregnancies (up to 12 weeks), treatment of incomplete spontaneous
abortions, and removal of retained afterbirth.
Aspiration is also used in diagnostic dilation and curettage (D & C) procedures in hospitals and in endometrial biopsy procedures in physician offices to remove the outermost layers of the endometrium (uterus lining) so that
further microscopic examination can be performed.
Principles of operation
A basic uterine aspirator consists of a vacuum pump, a vacuum regulator and gauge, vacuum curets, a curet
handle, two collection bottles, and an optional bacterial filter. Plastic tubing connects these components,
completing an open-ended system that continuously draws tissue and fluid from the patient’s uterus into the
collection bottles. The gauge allows the user to set a safe limit for suctioning, to assess the performance of the
vacuum pump, and to detect leaks or blockages. Units are either portable or mounted on a stand or cart for
mobility.
Vacuum pumps
A single or double vacuum pump creates suction within the system.
Double-pump systems operate in synchrony, with both pumps
contributing to the suctioning capacity and increasing the airflow. The
three types of pumps used in uterine aspirators are rotary vane,
diaphragm, and cylinder piston.

the inside diameter of the pump housing (see Figure 1).
As the rotor turns, sliding vanes that fit into notches in
the rotor follow the contour of the housing. Air that is
trapped between a set of vanes is compressed and
expelled through the pressure outlet. As rotation
continues, the space between the vanes expands, and the
pressure of the air trapped between the vanes drops,
creating suction. Several rotary-vane models use graphite
vanes because of the friction between the vanes and the
chamber wall and the need to achieve airtight contact.
In a diaphragm pump, a mechanical drive flexes a
rubber diaphragm back and forth in a chamber,
alternately increasing and decreasing the chamber
volume (see Figure 2). As the chamber volume decreases,
air is compressed and directed out through the one-way exhaust valve; as it increases, a vacuum draws air into
the one-way intake valve.
Uterine aspirators that use cylinder pistons to create suction operate similarly to diaphragm pumps; however,
the pistons slide in a reciprocating motion within the pump cylinder to alternately increase and decrease chamber
volume and pressure. One-way valves respond to pressure differences to draw air from the patient end of the
tubing.
Vacuum curets
Uterine aspirators apply suction to the patient’s uterus through vacuum curets that are available in various
types, sizes, and tip designs. Rigid curets are made of plastic or metal (stainless steel or chrome plated) and are
either curved or straight (a curved shaft reduces the risk of perforating the uterus). Semirigid and flexible curets
are made of nylon or plastic. Plastic curets are often transparent to allow maximum visibility during suctioning. A
stylet can be placed inside a flexible curet to give it rigidity during insertion. Many manufacturers can also
custom-design curets for specialized procedures.Most curets are 15 to 31 cm (6 to 12 in) in length and 2 to 16 mm (0.078 to 0.63 in) in outer diameter. In some
uterine aspirators, the outer diameter of the curet varies within a specified range based on whether the curet is
rigid, semirigid, or flexible. The type of curet used
depends on the procedure, the size of the patient’s
cervix, and the type of material being aspirated.
Most curets have a single opening located at the top
or on the side of the shaft to allow the passage of
aspirate from the patient; some curets have a dual-
sided opening, while others have two separate
openings. Regardless of the type and size, all curets
have a tapered base that fits into a dedicatedhandle/hose assembly, which swivels to allow free
movement within the uterus during aspiration. In
addition to being controlled by the regulator, suction
can also be controlled through the use of a footswitch,
use of an on/off switch in the handle/hose assembly, or
placement of a finger over holes in the shaft of the curet
to increase suctioning power.

Collection bottles
Aspirate from the curet passes through the tubing and into the collection bottle. Most manufacturers have
moved away from use of reusable glass collection containers toward use of plastic disposable units, which save
cleaning time and prevent cross-contamination among patients and hospital personnel. If the aspirator utilizes a
glass bottle, it must be emptied, cleaned, and sterilized after use on each patient.
When large volumes of tissue and fluid are removed from the uterus, aspirate can collect within the bottles
quickly. As a result, most uterine aspirators use two collection bottles connected in series. The bottle closest to the
curet collects aspirate primarily for later microscopic examination or disposal. Gauze sacks can be attached within
this bottle to collect the specimens. The second bottle, called a “trap,” collects the overflow from the first bottle,
allowing the operator to postpone emptying it in response to the rising level of aspirate.
Most uterine aspirators have an overflow-protection assembly in
the cap of the second bottle to prevent large amounts of fluid from
overflowing into the pump and valves. Some units feature a third
bottle to collect overflow. Most uterine aspirators use a float
mechanism as overflow protection. As aspirate collects in the
second bottle, a ball or other mechanical device floats to the top,
covering the opening leading to the suction source and cutting off
suction, thereby alerting the user that the collection bottle is full.
This overflow mechanism cannot be totally relied upon to prevent
pump contamination; often, some fluid passes through the tubing
to the pump before the float blocks the opening. As a result, most
aspirators also include a disposable in-line hydrophobic filter
between the trap bottle and the pump. This filter prevents fluid or
aerosol contaminants from flowing back into the vacuum pump; it
can be reused several times, provided that no overflow has
occurred. Although most units have these overflow-protection
features, the user should check the collection bottles frequently and
not allow them to fill above the caution line.In some systems, a special filter/specimen container fitted
between the curet handle and the hose leading to the collection
bottle is used in place of a gauze sack to collect uterine samples for
microscopic examination. This container has a filter to trap solid aspirate (tissue fragments) yet allows fluid to
pass through the tubing and into the collection bottle. When an adequate sample has been obtained, the container
is removed and capped.
Reported problems
Perforation, hemorrhage, or infection of the uterus from aspiration can result in unanticipated surgery.
Perforations of the uterine wall—such as those caused by overinserting a curet or applying too much vacuum to
increase the airflow—are rare when trained personnel aspirate the patient and when an appropriately sized curet
is used. In addition, curets can rupture an artery, a vein, or a capillary, causing hemorrhaging. Unsterilized curets
can introduce pathogens into the uterus, thereby producing infections that complicate the recovery process.
Bacterial endometritis, a common inflammatory condition, can occur following aspiration. Failed abortion can
result if improper aspiration techniques are used or if the curet is too small.
Factors affecting pump operation can also alter aspiration. Aspirate that overflows beyond the second bottle
and filter can enter the pump and clog the valves; a clogged pump can jam and be difficult to restart. A pump
containing aspirated fluid can also be a source of contamination.

Purchase considerations
ECRI Institute recommendations
Included in the accompanying comparison chart are ECRI Institute’s recommendations for minimum
performance requirements for uterine aspirators. Units should have an adjustable vacuum range with at least a
400 mm Hg maximum vacuum level and a flow rate higher than 30 L/min at maximum vacuum, as well as some
type of vacuum-level regulator. Vacuum level should be displayed in mm Hg. Collection canisters should hold at
least 1.5 L of liquid and have overflow protection or another anticontamination mechanism. Because uterine
aspirators are used in potentially wet environments, they should be impervious to fluid spills. Manufacturers
should specify methods to clean and disinfect units.
Other considerations
The aspirator should be easy and intuitive to turn on, adjust, and operate. Emptying and exchanging reusable
canisters should be uncomplicated and without significant risk of instrument or operator contamination.
Discarding disposable canisters and their contents should be easy, with minimal risk of spills or contamination.
Some manufacturers offer disposable collection containers. The cost of disposables should be weighed againstthe cost, time, and infection risks associated with the disinfection of reusables. Most units with disposable
containers can accept alternative manufacturers’ containers. These standard containers may be more convenient
to order and offer the user a choice of suppliers. However, installed filters supplied with the canisters will differ;
some may severely limit airflow rates.
Some manufacturers have incorporated higher suction rates so that their units can also be used to perform
general-purpose suction. Some units are also being used for liposuction procedures.
Cost containment
Because uterine aspirators entail ongoing operational costs, the initial acquisition cost does not accurately
reflect the total cost of ownership. Therefore, a purchase decision should be based on issues such as life-cycle cost
(LCC), discount rates and non-price-related benefits offered by the supplier, and standardization with existingequipment in the department or hospital (i.e., purchasing all aspirators and/or collection containers from one
supplier).
An LCC analysis can be used to compare high-cost alternatives and/or to determine the positive or negative
economic value of a single alternative. Because it examines the cash-flow impact of initial acquisition costs and
operating costs over a period of time, LCC analysis is most useful for comparing alternatives with different cash
flows and for revealing the total costs of equipment ownership. One LCC technique—present value (PV)
analysis—is especially useful because it accounts for inflation and for the time value of money (i.e., money
received today is worth more than money received at a later date). Conducting a PV/LCC analysis often
demonstrates that the cost of ownership includes more than just the initial acquisition cost and that a small
increase in initial acquisition cost may produce significant savings in long-term operating costs. The PV is
calculated using the annual cash outflow, the dollar discount factor (the cost of capital), and the lifetime of theequipment (in years) in a mathematical equation.
The following represents a sample seven-year PV/LCC analysis for a uterine aspirator.

As illustrated by the above sample PV/LCC analysis, the initial acquisition cost is only a fraction of the total
cost of operation over seven years. Therefore, rather than making a purchase decision based solely on the
acquisition cost of a uterine aspirator, buyers should consider operating costs over the lifetime of the equipment.
Prices for disposable tubing and catheters vary greatly depending on a number of factors, including type, size,
material, and manufacturer, as well as bulk-rate discounts. Many of these disposables can be used with a number
of manufacturers’ uterine aspirators. Disposables are usually more relevant to an LCC analysis for aspirators than
is the device itself—many device models can be used with various manufacturers’ disposables.

Stage of development
Uterine aspiration is a mature and well-accepted technology. Advancements have included improved curet
design and ultrasound guidance to facilitate clearance and reduce injury to the uterine wall.
