Welcome to Ash HouseAsh House Rehabilitation and Resettlement Services is owned and operated by theKrinvest Care Group, a well-established care and support provider in the North West.Ash House provides specialist services for individuals with long term conditions requiring rehabilitation and reintegration back into their local community.We are strongly committed to ensuring individuals are presenting to their possible bestand in turn ensure and equip them for community based living.

Service Philosophy & ValuesWe believe strongly at Ash House that service users should be placed firmly at the centre of their own care. Whilst cognitive neurorehabilitation is a highly structured approach, it is based within a clearly enunciated person-centred tradition and we believe the quality of the relationship between staff and service users is paramount. We also believe the crucial role of the environment provided for all parties aides the facilitation of recovery based treatment. Indeed, as we have said “people aren’t disabled, but environments can be disabling”.

Who are we?We are a multi-disciplinary team who are based at Ash House in Warrington and are part of the Krinvest Group. Members of the team come from various backgrounds including occupational therapy psychology, psychiatry, psychotherapy and nursing. Our aim is to enable those people who experience difficulties as a result of the illness or injuries they may have suffered. To form a care pathway for themselves with their own goals of getting back into their community. We are a specialist service dealing with particularly challenging individuals with a broad staff team who are well experienced and compassionate in and around this particular individual group.

Our Aims• To view our service users as individuals who can make choices, and with the correct support, can collaborate fully in their rehabilitation and recovery journey.• To hold an honest and open dialogue at all levels, with service users, carers and the broader MDT to ensure goals are shared and ultimately achieved.• To create an environment of mutual respect and dignity at all times.• To foster and develop an environment which is homely and conducive to a relaxed and informal community, but also minimises the effects of service users cognitive difficulties.• To engage all individuals in intrinsically motivating functional activities.• To review all service users care plans to ensure a dynamic forward moving treatment programme that is geared to progressing individuals to conditions of least restriction.• To continue to develop the skills and expertise amongst our staff, ensuring evidenced based approaches at all levels of the MDT.• To continue our research and evidence base for a comprehensive care approach for individuals with compromised cognitive functioning and concomitant challenging behaviour.

We offer a range of services that will be matched to each individual’s needs:• Comprehensive assessment of all the factors that may contribute to an individual’s presenting problems. Assessments may also be conducted for reasons including pre-admission assessments and risk assessment.• Individual formulations to aid the understanding of the factors responsible for causing and maintaining the individual’s presenting problems.• Intervention programmes based upon the individual’s formulation.These include the following:• Individual and Group Psychological Therapy• Cognitive Rehabilitation Programmes• Environmental Management• Adapted Therapy Provision• Assessment and Consultancy• Award Winning Neurological and Brain Injury Awareness Training for staff both internally and externally.

Admission Criteria include but are not exclusive to:A broad spectrum of Neurological conditions including:• Traumatic Brain injury• Learning disability• Physical disability• Mental Health disorders• Early onset Dementia• Huntington’s disease• Korsakoff ’s syndrome• Picks diseaseAge group is from 18 and over with no upper age limit

SummaryAsh House rehabilitation service is committed to delivering the best possible service drawing from experience alongside the latest developments in behavioural, cognitive, neuropsychiatric and neuropsychological theories. We aim to ensure that individuals are placed at the centre of their own care and focus and build on the individuals strengths. We aim to “catch what individuals do well rather than focus on any relative deficit” We are proud to be one of the only units in the UK who adopt a relentlessly positive approach to service user care and support.

Service SpecificationAsh House will offer a clear pathway of reintegration back into the community, offering learning opportunities and support to achieve a positive discharge to independent or ongoing supported living within the community.

ReferralsReferrals will be accepted from local hospitals, secure setting as well as local CMHT’s which may include short stay relapse prevention placements.Each Referral will be assessed by members of our multidisciplinary team and care plans developed in partnership with the individual to meet their identified needs, ensuring a clear understanding and insight into their treatment pathway

Rehabilitation PathwayIndividual rehabilitation pathways will be developed in line with identified clinical need and length of stay projections will be reflected at point of assessment. Discharge planning commences at point of admission in collaboration with the service user, their family and local support teams.

Community IntegrationOur staff team has established links with many local community and educational facilities to offer individuals the best possible opportunities in working towards achieving their goals which can be continued long after discharge.

Care PathwayOur aim is to support each individual to move towards independence with discharge planning starting at time of admission. Progressing through our care pathway and will be continually assessed to ensure everyone is working towards their identified goals, developing the skills our clients will need to support independence upon discharge.

General ManagerAfter 6 years of living in the Shetland Isles Danny moved back to the North West and commenced a new career in health and social care. Danny started as a support worker and progressed through; Senior Support Work, Deputy Manager, Service Manager then Head of Service. Danny has worked predominantly with mental health / dual and forensic diagnosis. Danny has developed his knowledge through working with multiple social service providers and private secure settings throughout England. Danny now acts as Registered General Manager and ensures that the individuals we support both current and future receive the highest quality of service. Danny is committed to providing person-centred support for every individual and to ensure all staff providing this support have the necessary knowledge and skills to do this.

AccomodationAsh House is a purpose built facility with 18 ensuite rooms, all rooms are single occupancy and are spacious and well equipped, three of our disabled access rooms have been specifically designed to include assisted bathrooms to ensure we can meet the needs of our service users at any stage of their condition promoting quality of life throughout their stay.The house has been built to a high standard and the quality finish ensures a homely comfortable environment has been achieved.

Facilities included:• Spacious living areas• Communal lounge including a cinema screen area• Games room• Sensory Gardens• Therapy rooms (for individual and adapted therapy sessions• Fully Equipped OT Facilities• Treatment rooms (Including Art and creative Therapies)

StaffingWithin Ash House the structure for the staffing will be as follows:• Psychologist• General Manager• Occupational Therapist• Registered Mental Health Nurses• Social Worker/Advocate Support Workers • Administration Staff• Catering dept.

Recruitment & TrainingKrinvest have robust recruitment and training policies in place to ensure regulatory compliance in both these areas which in turn are audited by our governance team at Head Office.Additional training funds are available for identified service specific training to support individual continual personal development (CPD).

ActivitiesAsh House will offer a range of internal and external therapeutic activities developed in line with clinical need and best practice to help our individuals work towards and achieve agreed outcomes, many activities will be provided in collaboration with local community and educational facilities to support ongoing personal development and establishing links and integration back into the local community. Included appropriate activities here:• Warrington local Collegiate – offer specialist “well men” courses – possibly 6 or 12 week (small group courses specifically developed for this type of service user group)

• Warrington Centre for Independent living offer a number of courses for small groups including IT/Photography Group - possible voluntary opportunities

• Local Charity shops – Voluntary opportunities

• Warrington Headway Group offer – participation in local events and social opportunities – visits to the theatre etc.

Using staff experience and networking a number of other local facilities could be used: • Warrington Sports centre – activities such as swimming etc.
