How does a Health Home work?A Care Coordinator will ask you questions about your health and your life. You will talk about what things you need right away and what things you might need soon. You and your Care Coordinator will create a plan based on the goals you have for yourself. These goals can be big or small. The decision is yours.

You and your Care Coordinator will work together on an ongoing basis to make sure you are getting the supports you need to meet your goals. If you already have a care manager or case manager, that’s ok. Your Care Coordinator will work with your care team which may include your doctors, specialists, landlord, etc.

Who should enroll?You must have Medicaid.

You also need to have:• Two or more chronic conditions (i.e., asthma, diabetes, heart disease, a mental health condition, or substance use disorder) OR• A serious mental illness OR• HIV/AIDS.

You must also show that you need additional help in order to get healthier.
