Intro
เปน AGN ทพบไดบอยทสดในประเทศไทย มกเกดตามหลงการตดเชอ group A beta-hemolytic
streptococcus เกดตามหลงการตดเชออนดวย (acute postinfectious
glomerulonephritis) เชน S. aureus, S. epidermidis, Mycoplasma sp., Leptospira sp., EBV, HIV, Toxoplasma
ระบาดวทยา ชาย>หญง พบบอยในเดกชายกอนวยเรยน และวยทเรมเขาโรงเรยน พบบอยชวงอาย 4-14 ป เฉลย 4.2 ป อาย < 2 ป พบ 5% อาย > 40 ป พบ 5-10% APSGN สวนหนงเปน isolated hematuria ดงนนจงไม
แสดงอาการ ทำาให incidence ตำากวาความเปนจรง Incidence ลดลงเรอย ค.ศ.1984 = 1 ใน 50,000 (สงคโปร)
สาเหต GBS ม antigen ทผนงเซลลของเชอ คอ M และ T
antigen ซงบางชนดเปน nephritogenic ag ทำาใหเกด APSGN ได M-type 12 พบบอยทสดจาก streptococcal pharyngitis M-type 49 พบบอยทสดจาก streptococcal skin
infection
พยาธกำาเนด
การเกดโรค เปนผลจาก Immune Complex (type 3) มปจจยตอการเกดโรค คอ ปจจยดานผปวย : genetic มผลตอ immune
response ตอเชอ โดย MHC antigen ทมผลตอเชอ ไดแก HLA-D, HLD-DR
ปจจยดานเชอ : ปรมาณและระยะเวลาทสมผสกบ antigen มความสำาคญตอการเกด AGN
พยาธสภาพ LM : ใน acute phase(<1 mo) จะพบ
glomerulus ใหญขน เพราะ มการเพมปรมาณของ Mesengial matrix และ inflamatory cell เชน Mesengial cell, endothelial cell, PMN, Eosinophil, Lymphocye, Phagocyte
Capillary lumen ตบแคบลง RBF↓, GFR↓ พบ crescent ได 10-15%; ถาพบ>60% prognosis ไมด Tubulointerstitium ปกต ยกเวนกลายเปน RPGN
IFM : พบการตดของ IgG,C3,C1q ตาม capillary loop และ mesengium ในลกษณะ granular staining
EM : พบ subepithelial electron-dense deposits ท capillary wall เรยกวา lump หรอ hump
>6 wk พยาธสภาพจะดขน แตบางรายอาจนานถง 2 ปแมไมมอาการแลว
พยาธสรรวทยา Inflammatory cell↑ และ GBM permeability↓ Glomerular filtration surface↓ และ rate↓ GFR↓ RBF↓ ; filtration fraction จงปกต Gromrular function ↓ urine volume↓ urine
Sp.Gr↑ Renin และ aldosterone ปกต Tubular function ปกต
อาการและอาการแสดง
บวม 85% ความดนสง 60-80% CHF 20% Gross hematuria 25-33% Nephrotic syndrome หรอ RPGN <1% เกดหลง pharingitis 1-2 wk เกดหลง skin infection 2-3 wk(อาจถง 6 wk)
อาการจะนอยหรอไมมอาการเลย
เกดจาก fluid overload
LAB
CBC : Hgb↓(fluid overload,RPGN), Plt↓(Plt survival time↓)
UA : Osm↑, Sp.Gr↑, proteinuria(mild-moderate), RBC (>10/μL),dysmorphic RBC, RBC cast, WBC, granular cast
RFT : BUN↑, Cr↑ (RBF↓,GFR↓) ***ถาผลเหมอน Prerenal cause จำาไววา ≠dehydration***
E’lytes : Na↓(free water retention) azotemia Na↓, K↑, acidosis
Tubular function ปกต ยกเวน interstitial inflammation หรอ severe glomerular involement
Immunologic features
Throat/skin swab culture (3/4 เพาะเชอไมขน) ThroatAntistreptolysin O (ASO) titer
พบวาม four-fold rising ได 80% False negative ใน skin infection (เพราะ streptolysin O จะจบกบ
ไขมนใน subcutaneous) หรอผปวยทไดรบ ATB มากอน SkinAnti Deoxyribonuclease-B (anti-Dnase B)
ม sensitivity และ specificity ดกวา, ตรวจเชอท skin infection ได ภาวะนเกดจาก alternative pathway เปนหลก
C3 ↓, C5 ↓, properdin ↓ C4 ปกต Complement ปกตใน 6 wk; ถามากกวานควรทำา renal biopsy

อยาลมหละวาตอง….Differential Diagnosis !!!
1. IgA nephropathy : gross hematuria เปนๆหายๆ/URI นำามากอนแค 1-2 วน(APSGN 1-2wk)/ไมคอยมาดวย acute nephritis/C3-C4ปกต/ biopsy=mesengial proliferation, มIgAเกาะ
2. Henoch-Schonlein purpura(HSP) : ควรมอาการทางผวหนง(palpable purpura ท extensor area ของขอ)/อาการGI = ปวดทอง ถายเปนเลอด/อาการไตเปน mixed nephitic-nephrotic syndrome/C3-C4ปกต
3. SLE : ใช criteria 4 ใน 11 ขอ/C3↓ C4↓4. Membranoproliferative glomerulonephritis
(MPGN) : อาการคลายกบ APSGN/แตมกพบในวยรน/ C3↓ C4ปกต/ แตระดบC3↓ นาน>6wk/พบไดนอยมากในประเทศไทย
5. Acute exacerbation of chronic glomerulo nephritis (CGN) : มระยะตดเชอท trigger ไมสมพนธกบอาการทางไต/ และพบอาการ CGN=ซด โตชา(ด growth curve)/ ม BUN และ Cr สงมาก ไมสมพนธกบอาการแสดง
6. Alport Syndrome : มประวตพนธกรรมไตวาย อาการรนแรงในเพศชาย/ถายทอด(XD80%,AR20%)/gross hematuria เปนๆหายๆ/การทำางานไต ↓เรอยๆ/มความผดปกตของ ตา(25%)และการไดยน(50%) รวมดวย
7. Cresentic glomerulonephritis หรอ Rapidly progressive glomerulonepritis(RPGN) : การทำางานไตเลวลงเรอยๆ ในเวลาสปดาห-เดอน/มกมอาการ anuria หรอ oliguria/ biopsy พบ crescent>50%/ อาจเปนผลจากจาก severe form ของ ASPGN หรอสาเหตอน
Entity Clinical Course Prognosis
Postinfection glomerulonephritis (GN). Onset occurs 10–14 d after acute illness, commonly streptococcal. Characteristics include acute onset, tea-colored urine, mild to severe renal insufficiency, and edema.
Acute phase is usually over in 2 wk. Complete resolution occurs in 95% of cases. Severity of renal failure and hypertension varies. Microhematuria may persist to 18 mo. Hypocomplementemia resolves in 1–30 d.
Excellent. Chronic disease is rare. Severe proteinuria, atypical presentation or course, or persistent hypocomplementemia suggest another entity.
Membranoproliferative GN. Presentation ranges from mild microhematuria to acute GN syndrome. Diagnosis is made by renal biopsy. Etiology is unknown. Types I and II are most common. Lesion is chronic.
Course can be mild to severe (rapid deterioration in renal function); may mimic postinfection GN. Proteinuria can be severe. Complement depression is intermittent to persistent. Hypertension is usually significant.
Type I may respond to corticosteroids. Type II (dense deposit disease) is less treatable; function decrease varies from immediate to as long as 15 y in 30–50% of untreated cases.
IgA nephropathy. Classic presentation is asymptomatic gross hematuria during acute unrelated illness, with microhematuria between episodes. Occasional instances of acute GN syndrome occur. Etiology is unknown. Diagnosis is made by biopsy.
90% of cases resolve in 1–5 y. Gross hematuric episodes resolve with recovery from acute illness. Severity of renal insufficiency and hypertension varies. Proteinuria occurs in more severe, atypical cases.
Generally good; a small percentage develops chronic renal failure. Proteinuria in the nephrotic range is a poor sign. There is no universally accepted medication. (Corticosteroids may be useful in severe cases.)

Entity Clinical Course Prognosis
Henoch–Schönlein purpura GN. Degree of renal involvement varies. Asymptomatic microhematuria is most common, but GN syndrome can occur. Renal biopsy is recommended in severe cases; it can provide prognostic information.
Presentation varies with severity of renal lesion. In rare cases, may progress rapidly to serious renal failure. Hypertension varies. Proteinuria in the nephrotic range and severe decline in function can occur.
Overall, prognosis is good. Patients presenting with > 50% reduction in function or proteinuria exceeding 1 g/24 h may develop chronic renal failure. Severity of renal biopsy picture can best guide approach in such cases. There is no universally accepted medication.
GN of systemic lupus erythematosus (SLE). Microhematuria and proteinuria are rarely first signs of this systemic disease. Renal involvement varies, but severe GN may ensue with remissions and exacerbations throughout the course.
Renal involvement is mild to severe. Clinical complexity depends on degree of renal insufficiency and other systems involved. Hypertension is significant. Manifestations of the severity of the renal lesion guide therapeutic intervention.
Renal involvement accounts for most significant morbidity in SLE. Control of hypertension affects renal prognosis. Medication is guided by symptoms, serology, and renal lesion. End-stage renal failure can occur.
Hereditary GN (eg, Alport syndrome). Transmission is autosomal-dominant/X-linked, with family history marked by end-stage renal failure, especially in young males. Deafness and eye abnormalities are associated.
There is no acute syndrome. Females are generally less affected but are carriers. Hypertension and increasing proteinuria occur with advancing renal failure. There is no known treatment.
Progressive proteinuria and hypertension occur early, with gradual decline in renal function in those most severely affected. Disease progresses to end-stage renal failure in most males.

anti-Dnase B ไดผลลบ/C3ไมตำาอาการ/อาการแสดงในระยะแรก
มภาวะ anuria หรอ neprotic syndrome มภาวะ azotemia ไมสมพนธกบอาการ/อาการแสดง
สาเหตอนๆทเกยวของ มอาการเมออาย <2ป หรอ >12ป มประวตโรคไตในอดต มการเตบโตชากวาปกต มประวตครอบครวเปน nephritis มอาการในระบบอนๆรวมดวย
ขอบงชในการทำา renal biopsy2 . มอาการ/อาการแสดง/lab หายชากวาปกต (persistent)
ระยะแรกของโรค oliguria หรอ azotemia >2wk ความดนโลหต↑ >3wk Gross hemautia>3wk C3↓ >6-8wkระยะทายของโรค Proteinuria และ microscopic hematuria >6 mo Proteinuria >6 mo microscopic hematuria >12 mo
การรกษา Fluid overload บวม,ความดนสง
Bed rest Restrict fluid = IWL +UO(ปสสาวะปกตของผปวยกอนให
furosemide) Restrict Salt 400mg/day furosemide 1-2 mg/kg/dose IV or PO
หากคม HTN ไมไดใชยาลดความดนกลมอน เชน Vasodilator=hydralazine 0.15mg/kg/dose IV or IM *
q 30-90 min max.dose=1.7-3.6mg/kg/day*IV จะออกฤทธทนท แต IM จะใชเวลา 15-30 นาท
-adrenergic bloackade = propanolol 1-5mg/kg/day PO ACEI กไดผลด แตเสยงตอ hyperK จงไมใช 1st line drug
IWL = 300-400 ml/m2/dayBSA= [4xBW(kg)]+7 / BW(kg)+90]
การรกษา สำาหรบผปวย hypertensive encephalopathy หรอความดน
โลหตสงมากจะให nitroprusside 0.5-2 mcg/kg/min IV ถา fluid overload
มาก/รกษาขนตนยงไมดขน/มoliguria,anuria จาก RPGN/ แนะนำาใหทำา acute dialysis/หามให digitalis
ถาม complication อยาง azotemia ควรจำากด Potassium และ Phosphorus Hyperkalemia restrict K 0.5mEq/kg/day &monitor K
K=5-6mg/dL restrict K + furosemide K=6-6.5mg/dL NaHCO3 + Kayexalate K>6.5mg/dL NaHCO3 + glucose + insulin acute dialysis
การรกษา การให ATB เพอรกษา streptococcal infection
พบวา ไมมผลปองกนการกอโรค และ ผลการดำาเนนโรค แตลดการแพรกระจายของโรคได
แนะนำาใหแก ผปวย และผใกลชดทสมผสโรค Penicillin or Erythromycin PO for 10 day เพอมนใจวากำาจดเชอไดหมด

Further Inpatient Care Only a small percentage of patients with acute
glomerulonephritis (AGN) require initial hospitalization, and most of those are ready for discharge in 2-4 days.
As soon as the blood pressure (BP) is under relatively good control and diuresis has begun, most children can be discharged and monitored as outpatients.

การดำาเนนโรคและผลการรกษา สวนนอย โดยเฉพาะใน ผใหญ จะม hypertension,
recurrent proteinuria และ renal insufficiency นานถง 10-40 ปหลงเรมเปนโรค เพราะ
เกด glomeruloscerlosis บางสวน glomerular ทเหลอจง pressure↑ และ size↑ nonimmunologic glomerular injury progressive renal dysfunction
การ recurrence ของโรคพบไดนอยมาก และอาจเปนผลมาจากม antibody ตอ nephritogenic streptococcal anitgen คงอยในเลอดเปนเวลานาน
สรป APSGN
Present 1-3 wk after Group A ß-hemolytic streptococcal infection of throat and skin
Occurs in all age group, most 5-15 yr Features: tea-colored urine, edema, HTN, mild to
moderate impairment of renal function If symptoms of nephritis are subclinical or missed,
may present with isolated heamaturia Antibiotic tx will not prevent development of
nephritis but should be prescribe for a patient and relatives (Penicillin/Erythromycin PO 10 days)
สรป APSGN
ASO titer/Anti Dnase-B ↑ initially; serum C3 ↓ but returns to normal within 6-8 wk
If C3 does not normalized by 6-8 wk, consider renal biopsy by establish diagnosis
Microscopic hematuria generalized resolve within 6-12 mo after onset of nephritis; may persisit up to 2 yr
Treatment in acute phase: bed rest, fluid and salt restriction, diuretics, antihypertensive
Excellent prognosis
