Introduction
Haven Property Dam 1007 is located on Range Creek upstream of the Timber Gully junction. It is for the purpose of livestock watering, Homestead lawns and gardens, irrigation and any other purposes as per the water allocation agreement. The dam has a homogeneous earth fill embankment with a spillway on the left abutment. An auxiliary spillway is located on the right abutment. The crest length of the dam is approximately 170 meters.

Dam owner responsibilities
• Develop and maintain an emergency action plan (EAP).
• Respond in accordance with the approved EAP in all dam related emergencies.
• Review and seek approval for updated EAPs as required, to ensure it is always up to date and includes current contact details for relevant parties. (must review before 1 October each year)
• Distribution of current approved EAP to all parties listed in the distribution list.
• Regular on-site monitoring and visual inspection of the conditions at the dam. The dam owner is responsible for conducting a safety evaluation of the dam and to identify any deficiency in the dam’s safety. Where deficiencies exist, the dam owner is required to take appropriate steps to minimise the potential risk of dam failure from these deficiencies.
• Provide timely and accurate notifications to all relevant parties mentioned in the notification list in the event of a dam emergency. The dam owner shall be responsible for continuously reporting the status of the dam and the event in accordance with the EAP.
• Make appropriate dam safety related decisions based on advice from a consulting engineer where appropriate. The dam owner is also responsible for authorising immediate expenditure so that urgent repair work will not be delayed.
• Report incidents and failures at the dam to the dam safety regulator in accordance with the EAP.
• Activate the EAP and maintain an incident log when emergency condition is identified at the dam.
• Periodic testing of EAP.
• Prepare Emergency Event Report (EER) and submit to the dam safety regulator within 30 business days after the end of the emergency event.
• The dam owner, has nominated as the Standby Operator, who will undertake all the roles and responsibility of this EAP in the absence of

Emergency events
The emergency event is a dam failure hazard or downstream release hazard that has happened. The events that will initiate emergency conditions at the dam are floods, seismic, landslide, seepage etc. The following events are defined as emergency events:
• Dam flooding
o Significant rainfall in the dam catchment with storage level rising rapidly and abnormally high spillway discharge expected
• Embankment stability issues
o Signs of distress or abnormalities in embankment such as cracking or deformation / sliding or any structural damage due to an earthquake event.
• Seepage events
o Detection of new seepage or increase in observed seepage, not due to storage rise.
EAP activation
This EAP will be activated when an emergency condition happens at the dam. There may be no need to activate this EAP if minor incidents happen (cracking, small slips, sinkholes) which will not endanger the integrity of the dam. If the incidents deteriorate to a dam failure hazard the EAP is activated. However, such minor incidents must be investigated, monitored and adequate works completed to prevent further deterioration.

LEAN FORWARD
Spillway flows increasing but as yet are unlikely to impact on downstream PAR and infrastructure.
• Dam owner to continue dam inspections, monitoring and recording of water level of the dam

• Update notifications to those who received previous notifications plus notifications to those who might be impacted by increasing spillway discharges
• Update expected every 4 hour or when deemed appropriate
STAND-UP
Spillway discharge increasing with flows that are likely to impact on downstream PAR and infrastructure.
If flood activity continues a dam failure may occur due to overtopping.
Dam failure is initiated.
• Dam owner to continue dam inspections, monitoring and recording of water level of the dam if safe to do so
• Dam inspections and recording of water level at increased regular intervals
• Evacuate to high ground to avoid any impending flood / dam failure flows
• Notifications to all relevant entities including PAR, Western Downs Local Disaster Management Group and Dam Safety Regulator. Updates to be provided at least every 2 hours but also when ‘benchmark’ lake levels or discharge are being reached such as:
o discharges likely to close critical roads and other infrastructure
o record flood levels are occurring in the dam
o dam crest is being reached or is being overtopped
• Immediately notify PAR, Western Downs Local Disaster Management Group, Dam Safety Regulator and other relevant entities when the dam failure initiates

STAND-DOWN

• Updates expected every 4 hours or when deemed appropriate
STAND-UP
Increasing in cracking to a state where the safety of the dam is significantly impaired.
Increasing seepage discharge rate and clouding. Piping failure has initiated.
Progressive internal erosion of the embankment or foundation to form an open conduit or a pipe and the dam failure is likely.
• Continue monitoring the condition of the dam and the issue if safe to do so
• Continue remedial work or load reduction if safe to do so
• Evacuate to the high ground if dam failure is anticipated
• Notify downstream Population at Risk (PAR), Western Downs Local Disaster Management Group and the Dam Safety Regulator that the dam is likely to fail
• Updates expected every 2 hours or when deemed appropriate
STAND-DOWN
Remedial works completed and there’s no risk of failure or it is determined that the issue is
