Summer Programs 2010 Application Instructions Common Application for Summer Programs & Summer Study Abroad Dear Applicant, Please read all information carefully and follow application instructions. An application for admission to any program will not be considered until ALL materials have been received. We encourage students to contact our office to follow up on their application and to complete their application as soon as possible. While admission is on a rolling basis, early application is encouraged as spaces for these programs and classes are limited. IT IS THE STUDENT’S RESPONSIBILITY to ensure that the application is completed and parts have been received in our office (including application fee, current transcript, Form A, essays, recommendations B, C and D, etc.) > If you attended one of our on-campus programs LAST summer (2009) and wish to apply to the SAME on- campus program, submit Application Form A (including course selection sheet, if applicable), application fee, a current transcript showing at least one marking period from this academic year, and a graded English paper (essay/composition) from this academic year (no teacher or school recommendations are needed). If applying to a DIFFERENT program than you attended last year (2009), then submit all parts of the application. > If you have applied, or are in the process of applying, for the regular academic year at Choate, please contact us for instructions. You may not have to submit all parts of our summer application; we will advise you. We can now accept MasterCard and Visa credit cards for the application fee. Please call the office for instructions, (203) 697-2365.

> SUMMER STUDY ABROAD APPLICANTS ONLY – Please submit an essay describing your interests, activities, honors, and experiences and indicate reasons why you wish to attend the abroad program.

> PERSONAL STATEMENT (On-Campus Program Applicants Only) – Choose one topic and write (without help) a 200-300 word essay in English: What do you hope to accomplish this summer and why do you think Choate Summer Programs

FORM B (School Recommendation) Fill in your name, grade, and program name and give to your guidance counselor, adviser, principal, or school head for

completion.

> Have your school attach a CURRENT TRANSCRIPT that shows at least one 2009-2010 marking period. •Step 3:

FORM C (Teacher Recommendation) Fill in your name, grade, and program name and give to your current English teacher.

Exceptions: > If applying to the Math/Science Institute for Girls, give this form to your current science teacher. > If applying to the Summer Study Abroad, give this form to your current language teacher. •Step 4:

FORM D (Teacher Recommendation) Fill in your name, grade, and program name and give to a current academic teacher of your choice, but NOT the teacher

that fills out Form C.

International Students: Please have your school include an English translation of your transcript and your school’s grading system. If you have recently taken the TOEFL or any other standardized English test (i.e SLEP), please forward your scores with your application. If you are applying to any program other than the English Language Institute, a TOEFL score is required. To be admitted, your TOEFL score must be 550 or above on a paper-based test, 213 on a computer test, or 80 on an Internet based test.

A NOTE TO THOSE APPLYING FOR FINANCIAL AID:

Please call (203) 697-2365, fax (203) 697-2519, or visit www.choate.edu/summerprograms to obtain the 2010 Financial Aid Application. Financial Aid Application, in addition to this completed student application, must be completed and submitted with all supporting 2009 IRS documentation before the student’s application will be reviewed for an admission decision. Submitting your 2008 IRS/tax forms does NOT meet our requirement; the application would not be considered complete if the 2008 forms are submitted. Financial Aid funds are limited and awarded on a first-come, first served basis, so we encourage early application. Because of limited funds, Financial Aid is limited to U.S. residents filing an IRS Income Tax Return. Summer Study Abroad offers no Financial Aid to families.
