Start application

Read carefully the information before you submit the application

Application Procedures (Part 1 - Personal information)

One e-form for each applicant per application: 1. Input name, candidate no.

and ID document no.

2. Upload ID document image

3. Download, complete the PDPO form and upload to the e-form

Application Procedures (Part 2 - Subject information and collection method)

4. Select the subject(s) for applying DAR

5. Select the collection method 6. Input contact number,

postal address or post office (if applicable)

Application Procedures (Part 3 - Final check and submission)

7. Check carefully the personal information, subject(s) requested and the collection method (can revert back to edit)

8. Read carefully and tick

the declaration check box

9. Submit the application

Application Procedures (Part 4 - Payment)

10. Candidates can settle the payment via Credit card (Visa/Master), or at 7-Eleven or Circle-K Convenience Stores

Payment Methods (A. Payment at convenience stores)

For payment at 7-Eleven or Circle-K convenience stores, candidates can download and print a hardcopy of the demand note for payment before the payment due date

Candidates will also receive an email with the demand note attached for record purpose

Candidates will receive a payment confirmation email after payment status is updated

Payment Methods (B. Payment by Credit Card)

For credit card payment, candidates will be re-directed to the visa/master card payment gateway to complete the payment process

Candidates will receive a payment confirmation email for record

If payment is unsuccessful, candidates will be re-directed back to the Payment page and he/she can try the credit card payment again or select the other payment method
