/ 15. RADIATION PROTECTION PROGRAM

A - SYSTEM SKETCH

1) The enclosed sketch gives the specifics of theinstallation. All equipment will be located andmounted in accordance with the recommendation of themanufacturer.

B - RADIATION SURVEY, SERVICING, MAINTENANCE, ETC.

1) Initial radiation survey, servicing, maintenance,relocation, and repair of the source holder willbe performed by "Endress & Hauser Inc." 2350Endress Place, Greenwood, Indiana 46142.

C - CONTROL MEASURES

1) If maintenance is required inside the cupola, alockout procedure will be employed to preventpersonnel access with the source in the ' measure'position.

2)The radiation p' man-hoist" employed for lowering

rotection officer will have theonly key to apersonnel into the cupola vessel for maintenancework. In addition, the radiation protectionofficer will lock the source head in the storeposition before allowing maintenance personnelto be lowered into the cupola vessel.

D - LEAK TESTING

1) The OHMART Corporation will provide " leak testing''services.

E - PROCEDURE TO BE FOLLOWED IN EVENT OF DAMAGE TO SOURCEHOUSING

1) The following procedure will be followed in theevent of damage to the source housing due to suchaction as fire, ete:

a) Immediately rope off the area around thesource holder to a ndnimum of 10 feet indiameter.

b) Inform plant radiation safety officer of thesituation.

c) Inform by phone or telegram the Region IIINRC Office of the accident.

| d) Limit access to source head until a radiationsurvey and source wipe can be performed by aqualified person. v

March, 1978 to Anril, 1979:Employed as the Manager of Safety at Golden Operations,CWC Division of Textron in Columbus, Indiana.

Golden Operations is a grey iron foundry producingcastings for the diesel truck engine industry.At Golden, I developed a Safety Policy and ProcedureManual that served as a base for an effective andcomprehensive loss control program which resulted ina substantial decrease in costs related to both injuryaccidents and property damage accidents.I was responsible for the direction of the 3 manOccupational Health Department, the 12 man securityforce, and all correspondence between the De'partmentof Labor and the company in all matters pertaining tothe 33 member Maintenance App'renticeship Program.I did all in-house industrial hygiene sampling, sentthe results to NIOSH approved labs for analyzation,and took appropriate action upon receipt of the analyzedresults.

March, 1976 to March,1978:Assistant Safety Director at the Marion Power Shovel Co.in Marion, Ohio. Major product manufactured was heavyequipment for the surface mining industry. I had safetyresponsibility for the set-up and finish welding operation,a burning and fabricating operation, assembly operation,painting, shipping, and receiving departments, partswarehouses, and a steel foundry.
