ABOUTTHEPROGRAMTheAmericanPharmacistsAssociation(APhA)developedthePharmacy-BasedCardiovascularDiseaseRiskManagementCertificateTrainingProgram,aninnovativeandinteractiveexperiencethatexploresthepharmacist’sroleincardiovasculardiseaseriskmanagement.Thispractice-basedactivityisthefirststepforpharmacistsinterestedinlearningtheessentialskillstosuccessfullyassessrisk,promotecardiovasculardiseaseprevention,andencouragepatientadherencetotherapy.GOALSOFTHEPRACTICE-BASEDPROGRAM
• Educatepharmacistsoncurrentevidence-basedtreatmentgoalsandclinicalmanagementrecommendationsfordyslipidemiaandhypertension
• Familiarizepharmacistswithimportantconceptsrelatedtohealthfullifestylechangesthatfocusoncardiovasculardiseaseprevention
• Introducetechniquesandskillsforencouragingpatientadherencetoprescribedtherapies
• Ensurepharmacistproficiencyinbloodpressuremeasurementtechniqueandpoint-of-carelipidtesting
• Enhancepharmacistexperienceinapplyingelementsofmotivationalinterviewingwithrespecttomedicationadherenceandlifestylemodifications
TheAmericanPharmacistsAssociationisaccreditedbytheAccreditationCouncilforPharmacyEducationasaproviderofcontinuingpharmacyeducation.ThePharmacistandPatient-CenteredDiabetesCarecertificatetrainingprogramisapprovedforatotalof23.0contacthoursofcontinuingpharmacyeducation(CPE)credit(2.3CEUs).TheACPEUniversalActivityNumbers(UAN)forthisactivityarelistedbelow.
Thecertificatetrainingprogramhastwocomponents:Web-basedinteractiveself-studywithassessmentandtheliveseminarwithonlinefinalassessment.ACertificateofAchievementwillbeawardedtoparticipantswhosuccessfullycompleteallprogramcomponents,includinganevaluation.RELEASEDATE:SEPTEMBER1,2018EXPIRATIONDATE:SEPTEMBER1,2021SELF-STUDYACPE:UAN0202-9999-18-247-H01-PSuccessfulcompletionoftheself-studycomponentinvolvespassingtheself-studyassessmentwithagradeof70%orhigherandwillresultin12contacthoursofCPEcredit(1.2CEUs).LIVEACPE:UAN0202-9999-18-248-L01-PSuccessfulcompletionoftheliveseminarcomponent,whichinvolvesattendingthefullliveseminarandcompletingtheonlineassessmentandevaluation,willresultin8contacthoursofCPEcredit(0.8CEUs).Oncecreditisclaimed,StatementsofCreditwillbeavailableonlinewithin24hoursonparticipant’sCPEMonitorprofileatwww.nabp.net.TheCertificateofAchievementwillbeavailableonlineuponsuccessfulcompletionofthenecessaryactivityrequirementsontheparticipant’s“MyTraining”pageonwww.pharmacist.com.FEEANDREFUNDPOLICYSpaceislimited;reservationsfortheprogramcannotbehelduntiltheregistrationfeeisreceived.Cancellationsreceivedinwritingatleastsevendayspriortotheprogramwillreceiveafullrefundminusa$150cancellationfee.Wereservetherighttocanceltheprogram.Inthatevent,eachparticipantwillbenotifiedviaphoneandafullrefundwillbemade.Additionalprogramdetailsandaccesstoonlineself–studymaterialswillbeprovidedviaemailuponreceiptofregistrationfeeandasmadeavailablebyAPhA.
