Letter from Executive Director Dear Community, Thanks to all of you who have volunteered your me, your services, your finances and/or supported us through sharing who we are with your friends, your family and persons in need of our services; together we are making a difference for those suffering with a severe and persistent mental illness. Extended Housing staff have supported clients becoming housed and making behavioral changes in order for them to maintain their housing. Kudos to our partnering mental health agencies and for the landlords that partner with us on our housing voucher programs; because of your reless efforts Extended Housing connues to make a genuine and posive difference as we serve our clients and you, our community. Who We Serve

Programs require documentation by appropriate mental health professionals, such as psychologists, psychiatrists

or mental health nurse practitioners. Extended Housing serves individuals with severe and persistent mental

illness, including but not limited to, bipolar, schizophrenia, severe mental health disorders, and those who are

mentally ill and have drug and alcohol abuse disorders.

Program Highlights

102 people/93 households lived in Extended

Housing-owned Supportive Housing units in 17

properties throughout Lake County.

68 people received residential care services at

North Coast House.

142 families were able to live independently in other

rental housing units through EH’s Housing Voucher

Programs throughout Lake County. This includes a

specific program that housed and served 9 Veteran

Households consisting of 17 persons.

163 homeless households were enrolled in the

homeless outreach PATH Program, of those 125

people were linked with mental health services and

resources in the community. 36 households

accessed housing and were no longer homeless.

28 households received a one-time “no interest”

loan for rental assistance/security deposits.

6 Housing loans were paid off this year.

27 households successfully transferred off

Extended Housing assistance.

Success can be measured in many different ways

— such as getting the person to agree to resume

taking their medicine, seeing a doctor or even

changing their behavior.

Homeless Outreach Picnic

For over 10 years Extended

Housing’s PATH (Projects for

Assistance in Transition from

Homelessness) Program has

organized a homeless

outreach picnic at the

Painesville Recreation Park.

The picnic takes place in June and was staffed this past year by

Extended Housing employees Kelli Giambetro, Leah Houser and

Sandra Langenderfer as well as members of the PATH Advisory

Council. The event provides a fun get together and a way to provide

outreach to homeless individuals in Lake County. The picnic was

promoted by distributing flyers locally as well as partnering with other

service organizations such as the Salvation Army and Project Hope.

Last year over 50 homeless individuals attended the picnic and were

able to get a hot meal that featured, hamburgers, hotdogs, and other

cookout food sides. The PATH Advisory Council also paid for East

Coast Custard to bring an ice cream truck and hand out free dessert.

Attendees also were entered into a drawing to receive backpacks filled

with toiletries and other personal items. Both a men’s and a women’s

bicycle were raffle off at the event. At the picnic people are able to

signup for appointments with the homeless outreach team to receive

services to help them transition from being homeless.

We continue to serve those who are homeless within our community by

helping them to transition to a stable living environment.

hard to get out of that hole. Extended

Housing gave me hope (when they helped me

find a home) and the opportunity to be

independent and have goals.”
