Greetings Friends,

What an exciting and busy year 2018 was for ACHIEVE! We were elated to open our new, multi-generational adult social program, On the Pond ~ At Cutler, a unique program that allows adults of all ages to socialize and participate in a variety of activities of their choosing, enabling individuals to build knowledge, confidence and relationships through education, recreation and volunteerism. We were also honored to be reaccredited by the Council on Quality Leadership as a result of the ongoing high quality work our staff provide daily to promote person-centered planning and achieving positive outcomes. This prestigious 3 year accreditation ranks ACHIEVE among the premier provider of services in our region, and we are thankful and appreciative of the work that was done to help us maintain this recognition.

In addition, our leadership team and members of the Agency Board of Directors came together to develop a three year strategic plan for the agency. Two primary goals were identified with 11 underlying strategies, the first centering on creating opportunities that meet the identified needs of individuals with intellectual, developmental and other disabilities, their families, and the community at large, allowing us to continue emphasizing that we’re here to serve first. The second goal focuses on improved employee engagement and retention, ensuring that we have well-trained employees in place engaged in helping us maintain our standard of high quality services. The strategic plan provides our team with a laser focus on paths to meet the ever-changing needs of individuals served through staffing as well as program development and enhancement.

As we continue to work in partnership with NYS OPWDD on their Transformation Agenda, one particular challenge that we’re facing head-on involves linkage of the individuals we serve in our vocational and employment services to the business community through competitive employment experiences. We continue to dialogue with local business partners to assess workforce needs with the intent to offer vocational training and on site job supports to fill vacant positions.

Finally, we were proud to continue advocating as part of the statewide #bFair2DirectCare campaign, calling on New York State elected officials to include funding in the budget to pay our direct service workers a livable wage and address employee retention issues. We were fortunate to see this funding included in the budget and have our voices heard collectively, and will continue championing on our DSPs behalf!

As 2018 came to a close, ACHIEVE was honored and privileged to support more than 2,200 individuals in Broome, Chenango and Tioga Counties with a wide array of services that assist them in defining and attaining a fulfilling life. So much of this is because of the amazing outpouring of community support that our organization receives in so many forms, and we cannot begin to thank our partners on behalf of those that we serve.

On Election Day, two of ACHIEVE’s individuals, Joe and Gwyn, exercised their right to vote. Joe was transported to the voting poll station by his support staff and used his TobiDynavox Eye Gaze Speech Generated device to indicate his vote. The software program, Communicator 5, enables the programmer to create different category pages such as a page for voting. Joe looks at a button for one second, that button is activated, and then the typed information is spoken. After 30+ years of being non-verbal, he finally has a voice and was excited to use it.Gwyn Zimmerman, another individual from ACHIEVE, also had the opportunity to exercise her right to vote on Election Day. Gwyn headed to the polls to cast her vote with assistance from her staff. She is passionate about exercising her right to vote and stated that it’s important “so we can get the services that we need”.

Lance VanAlstineAn ACHIEVE Success Story

Lance VanAlstine began working at Valley View Nursing Home in May 2018. Lance holds the position of Receptionist and has done exceedingly well.

Lance has always held a strong desire to reside independently and obtain competitive employment within the local Norwich community. In 2015 Lance moved from family care into his own apartment. In September 2016, Lance was asked to train in our Plant 1 reception area to fill-in when our regular receptionist took breaks/lunch/vacation. Lance did exceptionally well with this opportunity, embracing the new responsibilities given to him, which included more independence, learning new job tasks, as well as following a specific dress-code. Lance surpassed our hopes in this position and in the latter part of 2017 was given added responsibilities at our dental clinic, scheduling/confirming appointments, typing up billing and answering incoming phone calls.

Lance decided to utilize Supported Employment Services, specifically ETP (Employment Training Program), via ACHIEVE at the beginning of 2017, while continuing to work at CWS. While enrolled in ACHIEVE’S ETP services, Lance initially began the “Discovery” process with his assigned Job Coach, followed by spending one-on-one time with his Job Coach in the community

and at home, completing career and interest assessments. Lance excelled in the Discovery process, displaying enthusiasm, determination, and never missing appointments - so it was time to find Lance a job. With the assistance of his Job Coach, Lance was able to develop a resume, participate in mock application completion/interviews and explore assorted job opportunities.

In April, Lance’s Job Coach was able to secure an interview with Valley View Nursing Home for a part-time receptionist. Shortly following the interview, the HR department contacted his Job Coach to offer Lance the position directly. Specifically, Valley View decided to forego ETP’s paid internship program and hire him on their payroll directly. Lance was ecstatic and quickly agreed to work for them. Although Lance has only held the position a few months, he’s exhibited exemplary service at his job. Valley View is very pleased with Lance’s work ethics and attitude and often will ask him if he can work additional unscheduled hours.

Lance has demonstrated extraordinary motivation and drive to succeed in living independently and working competitively in a job he is happy performing. His determination toward these goals has proven to be a real asset in making his dreams a reality.

Lauren Farwell has proven that with hard work and persistence, anything is possible.

Lauren suffered an Anoxic Brain Injury five years ago, and has since had to re-learn many basic skills and functions. Lauren had previously received a degree in culinary arts prior to her injury, and expressed to her job coach at their first meeting that she would really like to pursue a career in culinary again. After only two appointments with her job coach, Lauren was called in for an interview, and then hired by Tioga Downs to work in the buffet as a prep cook. Lauren has now been there since April of this year. She works sixteen hours a week, and makes more than

minimum wage. Keenan Brown, The executive chef, has been very understanding and continues to help her move forward with her career goals of working the hot food line, and becoming a line cook. He also loves that Lauren addresses him as “The Big Boss Applesauce.”

The kitchen manager, Ed, has also been very understanding. Ed has assisted Lauren with many of her tasks in the kitchen. Lauren spends her shifts preparing various foods; cutting up fruits and vegetables, prepping shrimp, and making a wide variety of desserts. Lauren also occasionally works the salad bar, where she maintains the pans of food and assists customers with their needs.

Lauren is a diligent worker and is working hard to move up to the position of line cook. She interacts with several kitchen staff and customers. Since beginning her employment with Tioga Downs, Lauren is happy to be working within the kitchen again and also to be making new friends with the people that she works with.

Lauren’s experience shows how ACHIEVE strives to bring out the best in the individuals that we serve, and also to assist them with continuing to maintain successful and meaningful employment.

Family Support ServicesFamily Empowerment ProgramThe Family Empowerment Program provides financial reimbursement for short-term respite services to families enrolled in the program. Families are empowered by the ability to choose their own provider, including family members provided they do not reside in the same household, and schedule them whenever there is a need or desire for respite. An annual allotment of respite hours is provided each year, with a set reimbursement rate established by ACHIEVE.

Community Habilitation ProgramCommunity Habilitation is a Medicaid waiver service that assists individuals who either live at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them. Individuals work on goals to build independent living skills in the following areas: personal hygiene, self-care (eating, grooming, etc.), general household chores, mobility training, personal health care, financial management, appropriate social skills, and life safety. Weekly schedules are established by the individual and ACHIEVE staff.

In addition, ACHIEVE also offers Community Habilitation with a Self-Directed option. Selecting a Self-Directed option allows the individual and his/her family to select their own provider with the option to choose someone who is currently involved in their life.

Community Behavior Supports ProgramThe Community Behavior Supports Program provides ongoing behavioral supports to individuals who live in the community and receive services through the Community Habilitation and Waiver Respite Programs. The Behavioral Specialist works with families on Behavioral Support planning, provides therapeutic support, education, and crisis management guidance.

Recreation ProgramThe Recreation Program provides organized and structured, community-based recreational activities and social opportunities for adults and teens living at home with their families while, at the same time, providing a brief period of respite to family members. These programs are available on Friday evenings and Saturdays at both the Cutler Pond Road facility in Binghamton as well as on Friday evenings at ACHIEVE’s Tioga facility in Owego.

In-Home Respite ProgramThe In-Home Respite Program offers families a brief respite by providing a care provider who is identified and provided by ACHIEVE. The care provider spends a predetermined number of hours with the individual and engages in activities in or out of the home. Respite is short-term, temporary care of family members with a disability, to provide families with some relief from the demands of routine care.

Children’s Summer Program ServicesSummer Program offers school aged children with developmental and other disabilities curriculum crucial to maintaining the learning they gained through the school year in a fun and challenging way. Clubs, including cooking, media, drama, science, and more, are offered. These clubs are designed to be fun and secretively educational. Students attending summer program also increase social skills, build confidence, increase independence skills, while being supported by a team of caring, compassionate counselors. A unique work study program is also offered to participants ages 14 – 21. This allows them to work for one hour a day, building vocational skills and earning a paycheck, and then they participate in normal program activities for the rest of their day.

Guardianship ProgramThe Guardianship program serves as corporate guardian for individuals who do not have family members that are able to do so and need support with decision making. The program ensures that programmatic, medical, social and other needs are cared for just like a parent would. Through our parent agency, The Arc New York, Achieve may serve as Primary, Stand-by or Alternate Stand-by guardian for individuals. The Guardianship Program also provides information, referrals and technical assistance to individuals and parents on guardianship, supported decision making and other alternatives.

Party NightACHIEVE’s Party Night is an opportunity for adults with intellectual and other developmental disabilities to socialize with their peers. At least eight events are planned each year to give the individuals a chance to get together and have some fun.

Residential Services / Day Habilitation ServicesResidential Services ACHIEVE’s Residential Services Division offers three levels of support for individuals with intellectual and other developmental disabilities, whether in one of our 19 residential houses, or in supportive apartments within our communities.

Supervised Residences are supported 24 hours per day by caring staff that assist individuals with learning and growing residential skills, and accessing recreational, social and work opportunities in the community. Supervised Apartments is a unique program offering individuals an opportunity to live semi-independently in their own apartment within a larger complex. Staff is available 24 hours per day if needed. In Supportive Apartments, residents live in their own apartment in their community, with regular visits from staff, 2-3 times a week, allowing them maximum independence and integration.

Day Habilitation ServicesThe Day Habilitation Program provides a comprehensive spectrum of intensive rehabilitative and support services to assist individuals with moderate-to-severe levels of disability with developing and maintaining skills for community integration. Services include teaching activities for daily living, cognitive skill development, recreation, communication development, socialization opportunities, volunteer activities, speech education services, and comprehensive nursing services. Services are individualized and assist in developing, enhancing, and maintaining skills used while living, working or socializing in the community.

On the Pond ~ At Cutler, a unique multi-generational social program, opened its doors for services in October of 2018. Through volunteerism, recreation and education, individuals build knowledge, confidence and independence skills. Partnering with local businesses and organizations offers opportunities for individuals to broaden their experiences and facilitates integration in our community. Participants in On the Pond ~ At Cutler are given a wide array of activities to choose from, giving them ownership of their days and a real sense of independence.

Vocational & Employment ServicesPre-Vocational ServicesACHIEVE pre-vocational services are intended to explore people’s interests and the world of work, help them discover their capabilities, develop work-readiness skills, and merge these areas in a manner that will move the person forward in their employment endeavors. ACHIEVE offers Community-Based and Site-Based Prevocational Services. These services are exploratory in nature and are designed to teach a variety of skills necessary to be successful in an employment environment.

Supported EmploymentThe Supported Employment Program provides employment supports for individuals looking for community employment and for those already employed at local businesses. Services include assistance with finding and maintaining employment in the community, guidance with the application and interview process, travel training, on-site job support and coaching, as well as on-going support to the individual and business partner. The Supported Employment Program is certified and fiscally supported by ACCES-VR and OPWDD.

Pathway to EmploymentPathway to Employment is a twelve month pre-employment service implemented by OPWDD, designed for individuals who have an interest in becoming employed in the future and who are enrolled in the Home and Community Based Services (HCBS) waiver. Pathway to Employment provides comprehensive career and vocational exploration through a process of discovery encompassing community-based vocational explorations, small group work-readiness training and various soft skill building activities. Once an individual has completed the Pathway to Employment program, they will have a well-developed plan for achieving a person-centered vocational outcome.

CWSCWS Packaging, a subsidiary of ACHIEVE, serves as the core facility-based vocational program. CWS provides a variety of training and job opportunities through subcontracts with business and industry. The program is certified by the New York State Office for People with Developmental Disabilities (OPWDD), State and County Social Services Departments, and the United States Department of Labor. The discontinuation of contract manufacturing in Broome-Tioga Counties in the fall 2017 is reflected in the large decrease of individuals served under this program. Individuals previously supported in this domain are transitioning to alternate community-based services and/or competitive community employment. CWS is evolving into an Integrated Business and continues to support limited Site-Based vocational services.
