COLORADO HEALTH NETWORK, INC. 2015 ANNUAL REPORT
Dear friends and supporters:
A long-time client, Michael, recently came in to receive documents he needed related to his housing assistance. His case manager greeted him with a big smile and asked how he was doing. Michael beamed with pride as he shared how he now earns enough money to rent his own apartment! With assistance from our food bank, he’s able to make his own meals at home and for the first time in many years, his HIV viral load was at an undetectable level. He grinned and told his case manager, “You know, you can really tell when someone truly cares about you!”
There are many stories like Michael’s that remind us of why the work we do is very important. Whether a client is coming to us for oral health care, insurance, financial or housing assistance, counseling, or linkage to treatment and care, each of their stories are important.
Today, most of the people living with HIV that we serve are facing one or more barriers to treatment and care. Knowing this, our case managers assess and support clients in ways which allow them to make their health a priority. We also know preventing an HIV infection is equally as important. By investing time and resources into sexually transmitted infection (STI) testing, education, and our syringe access programs, we reduce our entire community’s risk of acquiring HIV and other disease.
In 2015, Colorado Health Network purchased a building on East Colfax Avenue in Denver to renovate for the consolidation of our Denver service locations. When the

project is complete late this summer, we’ll be a much more efficient organization and have the ability to provide a greater number of services to clients and patients all under one roof. We envision expanding to provide more general health, wellness, and preventative services to an even broader population. As the summer approaches, please watch for news and updates about the status of the building project and the additional services we’ll be providing at the new location.
We couldn’t do this important work without you! Thank you for your continued support and interest as we work toward reducing the impact of HIV, bringing healthier lives to persons affected by the disease, and laying the foundation for a new future serving clients and communities
Sincerely,

COLORADO HEALTH NETWORK, INC. 2015 ANNUAL REPORT
Colorado Health Network’s 2015 Annual Report illustrates the continuum of care, from start to
finish, that we provide to individuals who come into one of our offices looking for help.
Julian, Kyla, and Rick's stories reveal how this happens…
All data presented is from Fiscal Year 2015: October 1, 2014 - September 30, 2015.

Meet JulianJulian is a gay man living in Fort Collins and a current client of NCAP.
In 2012, after hearing about HeyDenver from a friend, Julian drove down to Denver to get tested for HIV. Up until this point, Julian had never been sure of his status and feared getting tested locally because he thought he’d run into someone he knew.
For Julian, the diagnosis that he was HIV-positive took him by surprise. He admits to feeling immediately overwhelmed. He recounts how his tester, Rick, was critical to helping him navigate next steps; ultimately connecting him with NCAP staff.
Currently Julian receives case management and insurance assistance from NCAP and is in his last year of college.
For people living with HIV, many factors can influence their ability to make
health a priority.
During 2015, CHN provided individualized support to 4,157 people living with HIV
and administered 7,309 free HIV, Hep C, and STI tests to those at risk.
The more aware people are of their status and are linked into care, the better poised
Colorado is to curb new infections.

Meet KylaKyla has been a Case Manager at NCAP for six years. She was also the staff member who guided Julian (pg. 7) through the support available for him.
Counseling services were huge for Julian in the beginning, dealing with his diagnosis and learning he wouldn't be alone in this. She recounts really seeing a shift in his outlook over the first six months of working with him.
Affordability of his medication was also something that concerned Julian. Kyla remembers how relieved he was when she told him we could supplement his out-of-pocket medical costs.
“We do such inspiring work here! You get a nice feeling when you see your client realize they have someone here to support them.”

Meet RickRick has been volunteering at HeyDenver for a few years. He originally was drawn to the organization after feeling that his current job didn't provide a sense of tangible connection to the community.
He still remembers meeting Julian (pg. 7) three years ago. While being the first person to tell someone that they’re HIV-positive is not an easy conversation, Rick found comfort knowing he's part of a network in providing supportive services, and that he was a resource for Julian in understanding more about his personal health.
“Testing sites can be off-putting for some. Our program is unique in that participants, like Julian, can come in and feel comfortable enough to have an open discussion about their sexual health.”
Volunteers are truly an integral part of CHN and contribute in all programs
of the organization.
Performing front desk duties, helping clients in the food bank, administering
HIV tests, conducting community outreach, and assisting with special events
are ways our volunteers give back.
In 2015, volunteers contributed 12,171 hours of service; translating to $312,551* saved on
staffing and professional services.
*Based on yearly earnings provided by the Bureau of Labor Statistics for the state of Colorado.
