Do you need to enroll?If you are currently enrolled in HII benefits and do not take action during the Annual Enrollment period (April 20–May 1), most of your current benefit elections roll over starting July 1, with the EXCEPTIONS listed below.

Tobacco-use status If you do not use tobacco or have completed a tobacco cessation program, you MUST reset your tobacco-use status to “tobacco-free” during Annual Enrollment to receive the tobacco-free discount on your medical premium, saving you $660 a year. If you DO NOT update your status as tobacco-free, you will lose the tobacco-free discount starting July 1.

Flexible spending accounts (FSAs)

You MUST re-enroll in the health care and/or dependent care FSA and set your contribution amount each year if you want to continue participation. Your current FSA elections WILL NOT automatically roll over.

Voluntary benefits Review your voluntary benefit elections (group legal, hospital indemnity, accident insurance and/or critical illness insurance) each year to make sure you have coverage that is right for you and your family.

New benefit elections This is your opportunity to review your benefits to make sure you have the coverage you want for you and your family for the 2020-2021 plan year.

New! High option vision plan availableHII will offer three vision plan options: low, medium and high. The low and medium options are our current base plan and buy-up plan options. The new high option covers a second pair of glasses or contacts for each covered person every plan year. All vision plans provide an option to buy prescription safety glasses.

Changes to specialty prescription drug benefitsThe prescription drug plan is implementing several changes to its prescription drug coverage that will impact only a small number of people who take select specialty medications. CVS Caremark will reach out to you if these changes apply to you. Here is an overview of the changes:

• Costs for certain specialty medications will increase; however, you will have access to the CVS True Accumulation program that can help you enroll in copay assistance offered by the drug manufacturers of these medications. If you or a covered family member fill a specialty drug prescription on the True Accumulation drug list, a CVS Caremark Specialty representative will contact you regarding copay assistance to lower your deductible. If you participate, in most cases, the amount you pay for your medication will remain the same or may be even be lower, in most cases.

• Only the amount you actually pay for your prescriptions will be applied towards your deductible, or out-of-pocket maximum, when using a third-party (“manufacturer”) copay card program.

• While not all specialty prescriptions are eligible for copay assistance, many of the drugs used to treat multiple sclerosis, rheumatoid arthritis, anemia and other chronic conditions qualify for reimbursement through copay assistance programs.

What’s a specialty medication?A medication used to treat chronic, complex conditions like multiple sclerosis, hepatitis C and cancer, to name a few. Specialty medications can include oral solids, or can be injected, infused or inhaled and may require special handling, such as refrigeration.

New! Support for autism and pervasive developmental disordersAs part of HII’s healthy mind strategy, the Anthem medical plans include coverage for autism and pervasive developmental disorders treatment starting July 1. Pervasive development disorders include autism, Asperger syndrome and Rett syndrome.

New! Enhanced dental benefitsAll dental plans have enhanced benefits, including coverage for posterior composite fillings. Additionally, the dental care plus plan will no longer have a lifetime deductible for orthodontia.

Which medical plan option works for you?Here’s a quick look at how the two medical plan options work:

PPO CDHP with HRA account

Your payroll contributions are higher than the CDHP plan, but the PPO plan has lower deductibles and out-of-pocket maximums. Once those are met, you pay:

• Copayments for office visits and prescription drugs, or

• Coinsurance on certain services after you meet the deductible (20% for in-network care until you reach the out-of-pocket maximum). Once you reach the out-of-pocket maximum the plan pays 100%.

Your payroll contributions are lower than the PPO plan, but the CDHP plan has higher deductibles and out-of-pocket maximums.

• You are responsible for the cost of care until you reach the annual deductible; HOWEVER, HII’s annual contribution to your HRA account will cover 100% when funds are available. Plus, your HRA dollars roll over from year to year.

• If you use all the available funds in your HRA before your deductible is met and/or before the plan year is over, you are responsible for paying all incurred medical expenses and/or remaining deductible.

• Once your deductible is met, you pay coinsurance (20% for in-network care) until you reach the out-of-pocket maximum.

• Once you reach the out-of-pocket maximum, the plan pays 100%.

How to be a smart health care consumerBetter care doesn’t mean paying more. It means making informed decisions about which providers to use, what care to receive when, and how you receive care. When you make good decisions in these areas, you’ll find the payoff is more than just money—it’s good health, too.

Here are a few tips for getting the best deal on healthcare.

Use in-network providers

With all HII medical plans, you pay less when you use in-network providers.

Avoid unnecessary ER visits

Emergency rooms generally cost three times as much as primary or urgent care services, according to the Urgent Care Association of America. That said, visit the ER if you have life-threatening symptoms. For non-emergencies, try Teladoc, the HII Family Health Center, your personal provider or an urgent care center.

Get your preventive checkup and all recommended screenings

Preventive care is 100% covered by all HII medical plans. So take advantage of your benefit and go to the doctor for a physical each year. Your health plan also has a list of recommended screenings by age and gender. Use that as a guide to stay on top of your health.

Know Your Numbers ProgramWhen you enroll in an Anthem PPO or CDHP plan, you can pay up to $600 less on your annual medical premium by completing HII’s Know Your Numbers program. The program is administered by QuadMed and designed to help you learn more about your health and potentially catch disease before it starts.

Program requirementsTo successfully complete the Know Your Numbers program, you must:

1 Complete a health screening (either through QuadMed’s BeWell for Life program or through your primary care provider).

2 Complete your wellness profile at myquadmedical.com/hii.

How the $600 discount worksYour medical premium for the upcoming plan year reflects your status in the Know Your Numbers program:

If you completed the program by Sept. 30, 2019

If you completed the program after Sept. 30, 2019 but before March 31, 2020

If you have not completed the program

Good news! Your 2020-2021 medical premium already reflects

the $600 discount. You will automatically receive this discount

until June 30, 2021.

Enroll in an FSA and start savingFlexible spending accounts (FSAs) help you save money on taxes by paying for eligible out-of-pocket health care and dependent care expenses with pre-tax dollars. You choose the amount you want to contribute each paycheck and it is deposited directly into your FSA. You must elect these accounts each year.

If you are currently enrolled in one or both FSAs, you must re-enroll to continue participating for the 2020-2021 plan year.

Health care FSAUse pre-tax dollars on copays, deductibles, prescriptions and more to save money on your taxes. This year, you can contribute up to $2,750 (currently $2,700). The full amount you elect to contribute to your health care FSA is available in your account on the first day of the plan year. Your contributions will be deducted from your paycheck evenly over the plan year.

Dependent care FSAPay for dependent day care expenses on a pre-tax basis and pay yourself back tax free for expenses throughout the year. You can contribute up to $5,000 each year to the dependent care FSA if both you and your spouse work, your spouse goes to school full-time or your spouse isn’t able to provide self-care. You may receive reimbursement for eligible expenses up to the amount in your account at the time you submit your claim.

Voluntary benefits

HII encourages you to review your voluntary benefit elections each year to ensure you have the coverage you want.

The following voluntary benefits are available only to employees located in the United States.

Group legalFor a modest premium, group legal coverage gives you a place to turn for help with a wide range of legal matters, such as buying a home or creating a will, as well as unexpected issues, like a dispute with a home contractor or a sudden illness. When you enroll you get coverage for yourself, your spouse and any dependent children.

Accident insuranceAccidental injury insurance can provide financial assistance when you suffer a covered accident. The financial benefits can help you cover out-of-pocket costs not covered by your medical plan or help pay for living expenses that you may not otherwise be able to afford. You can also extend coverage to your spouse and/or children.

Critical illness insurance Critical illness insurance gives you a lump sum payment if you’re diagnosed with a covered illness or disease. Use this lump sum to pay for whatever you need, such as medical bills not covered by your health plan, mortgage and rent payments, child care and more. You can also extend coverage to your spouse and/or children.

Hospital indemnityHospital indemnity provides financial assistance by paying you a daily benefit when you have a covered hospital stay. You can use the money to cover out-of-pocket costs not covered by your medical plan or to help pay for living expenses that you may not otherwise be able to afford. You can also extend coverage to your spouse and/or children.

Important note: HII does not contribute to the voluntary benefits listed above and has minimal involvement in these plans. If an employee enrolls in one or more of the plans above, he/she will pay the full premium for coverage.
