Provide proof of CuRRent financial membership of an approved shooting club. This proof is to clearly show the name of the club, your name and the expiry date of your membership. (note: Applications by MinORS for the above mentioned reasons must also include a letter from a parent or guardian authorising the use of firearms. Contact details of the parent or guardian must be provided in the letter.)

Sports or target Club (Category C)Provide documentation signed by a doctor supporting the existence of a physical need for category C because of a medical condition or due to lack of strength or physical dexterity AnD documentation signed by your medical practitioner supporting lack of strength or physical dexterity AnD proof of current Clay target Club membership with an approved club. The proof is to clearly show the name of the club, your name and the expiry date of membership. (note: applications by MinORS for the above mentioned reasons must also include a letter from a parent or guardian authorising the use of firearms. Contact details of the parent or guardian must be provided in the letter.)

Sports or target Club (Category h)Provide current declaration (QP 0518) signed by the club representative, AnD a copy of your participation record for the period of your licence (applicable after 01-01-2004 only).(note: applications by MinORS for the above mentioned reasons must also include a letter from a parent or guardian authorising the use of firearms. Contact details of the parent or guardian must be provided in the letter.)

paintball Club (Category A only)Provide a letter from a paintball field official stating that you have permission to play paintball. (Club membership is not required).

Crossbow Club (Category M only) Provide proof of current membership of an approved Weapons Club for sports or target shooting. This proof is to include the name of the club, your name, membership number and membership expiry date.

if you are the holder of a current weapons Act licence of another type, you are not required to supply a recent photograph unLeSS —

1. A period of not more than 10 years has elapsed since you provided proof of identity and photographs; OR2. The current licence card does not reflect an accurate likeness of you (e.g. have you grown or removed a beard since

the issue of the licence card?)

1. Provide details of any current weapons licence held; OR2. Provide proof of the successful completion of an approved course in safety training for weapons—a ‘Statement of

Attainment’ issued within the last twelve months, detailing the date the certificate was awarded, the categories of theweapons in which you have been deemed competent, and the name of the registered training provider.
