12829 and dispatched to GOC shed/SR on 20.07.2013.

Next, DLW advised that 3 sets of MUI have been fitted in

Loco No. 40161/ Raipur/SECR, 12885/GOC/SR & Power

Pack No. G4-1133 to Gooty/SCR. Letter sent to the Rly on

30.12.2013 regarding performance. Performance received

from Gooty shed only & others sheds reply awaited. Letter

sent to firm on dated 07.03.14 for failure investigation. Firm

Vide letter dated 14.03.2014 advised that they are

investigating failures & the report will be sent. Once the

investigation is complete. Letter sent to firm on dated

07.04.2014 and 20.05.2014. Report awaited. DLW have

Inspection carried out. The firm was found to be not

prepared for inspection. As recorded in joint note dated

14.05.14, the firm will advise their preparedness for

inspection again. Firm’s advice is awaited. Firm has

indicated that they have made good the deficiencies for

conducting inspection. RDSO representative has been

deputed to visit the firm’s facilities from 26.08.2014 to verify
