Why patients “screw up” in the ED

• You’re not clear why it's an EMERGENCY • You don’t disclose all of your medical problems • You don’t know what medication you take • You’re looking for a third or fourth opinion • You treat it like a primary care visit • You lie or hide the truth • You beat around the bush • You text, or are constantly on the phone • You don’t share when you're frustrated • You have consulted Dr Google and already know what

is wrong

The Four Stages of Competence

• Unconscious incompetence The individual does not understand or know how to do something and does not necessarily recognize the deficit (may lack insight). They may deny the usefulness of the skill. The individual must recognize their own incompetence, and the value of the new skill, before moving on to the next stage. The length of time an individual spends in this stage depends on the strength of the stimulus to learn.

• Conscious incompetence Though the individual does not understand or know how to do something, he or she does recognize the deficit, as well as the value of a new skill in addressing the deficit (insight). The making of mistakes can be integral to the learning process at this stage.

• Conscious competence The individual understands or knows how to do something. However, demonstrating the skill or knowledge requires concentration. It may be broken down into steps, and there is heavy conscious involvement in executing the new skill.

• Unconscious competence The individual has had so much practice with a skill that it has become "second nature" and can be performed easily. As a result, the skill can be performed while executing another task (multi-tasking). The individual may be able to teach it to others, depending upon how and when it was learned. Intuition. “Gut feeling”. Gestalt.

• Only come if it is a real emergency • Speak the same language as me • Have an acceptable level of (health) literacy and

intelligence • Listen to my questions and answer them

comprehensively (and promptly) • Arrive in an orderly and even-spaced fashion • See your GP (if appropriate), have some investigations

and bring a comprehensive (and legible) letter (and all your medications & x-rays)

• Don’t arrive with pre-conceived ideas about why they are there, what I can provide, what their diagnosis is

• Don’t arrive affected by alcohol and other drugs
