The Ten Steps to Successful Breastfeeding

1. Have a written breastfeeding policy that is routinely communicated to all heath care staff.

2. Train all health care staff in the skills necessary to implement this policy.

3. Inform all pregnant women about the benefits and management of breastfeeding.

4. Help mothers initiate breastfeeding within one hour of birth.

5. Show mothers how to breastfeed and how to maintain lactation, even if they are separated from their infants.

6. Give infants no food or drink other than breast-milk, unless medically indicated.

7. Practice rooming in – allow mothers and infants to remain together 24 hours a day.

8. Encourage breastfeeding on demand.9. Give no pacifiers or artificial nipples to

breastfeeding infants.10.Foster the establishment of breastfeeding

support groups and refer mothers to them on discharge from the hospital or birth center.

• Nationwide survey administered by the CDC in 2007, 2009, 2011, 2013 then every 2 years

• All maternity facilities in US were invited to participate • To be completed by person most knowledgeable about the

issues• 89% of the 28 eligible hospitals and birth centers in WV

In recognition of the devastating effect of formula and bottle marketing, the World Health Organization and UNICEF adopted the “WHO-CODE” in 1981. The “WHO-CODE” is short for the World Health Organization’s

International Code of the Marketing of Breastmilk Substitutes.• No advertising of breast milk substitutes to families. • No free samples or supplies in the health care system.• No promotion of products through health care facilities, including no free or low-cost formula.• No contact between marketing personnel and mothers.• No gifts or personal samples to health workers.• No words or pictures idealizing artificial feeding, including pictures of infants, on the labels or the

product.• Information to health workers should be scientific and factual only.• All information on artificial feeding, including labels, should explain the benefits

Free formula samples, coupons and gift bags given by many hospitals and healthcare provider offices can greatly influence parental choice of infant feeding methods.

Importance of Policy1. Safety: A well-researched and reviewed policy establishes clear guidance about those practices that are most effective at reducing and managing risk.2. Effectiveness: Developing policy through a process of thorough review of the current body of scientific knowledge ensures that policy is evidence-based and that the most effective strategies are consistently implemented. Establishing and communicating this type of written policy provides a framework for a sanctioned course of action that all staff should be expected to follow. The standards laid out in the policy provide a “road map” for carrying out and evaluating best practices in patient care.3. Patient-centeredness: Implementation of an evidence-based policy establishes a framework for providing coordinated, respectful, responsive care that can be tailored to individual patient preferences, values and needs.4. Timeliness: A consistently communicated policy assures that appropriate support will be delivered throughout the patient’s stay and that continuity of care will be maximized.5. Efficiency: When all staff are trained in and consistently adhere to the same evidence-based standards of care, cross-purpose efforts are avoided, reducing waste on many levels.6. Equity: Establishment and clear communication of policy sets an explicit standard for consistent delivery of care. When the Ten Steps are consistently implemented, disparities in outcomes are reduced.

Academy of Breastfeeding Medicine’s Clinical Protocol #7: Model Hospital Policy

• Create a breastfeeding policy that promotes patient-centered care• Policy must require staff to implement the Ten Steps and the Code• Train staff on the policy within six months of hire and provide ongoing

continuing education on the principles of the policy. See also Step 2• Display the policy and/or a printed patient guide in all patient care areas

Joint Commission Definition of Exclusive Breastfeeding

A newborn receiving only breast milk and no other liquids or solids except for drops or syrups consisting of multivitamins, minerals, or medicines during the entirehospital stay. Pumped and donor breast milk are considered “exclusive”.

9. Give no artificial nipples or pacifiers to breastfeeding

infants

Risks of Early Pacifier Use• Interfere with demand feeding• Reduce time suckling at the breast• Alter oral dynamics• May lead to “nipple confusion”• May indicate a breastfeeding problem

The AAP has clarified its position on giving pacifiers to breastfeeding infants, stating that it:“does not support a categorical ban on pacifiers…[but] pacifier use in the hospital in the neonatal period should be limited to specific medical indications…Mothers of healthy term breastfed infants should be instructed to delay pacifier use until breastfeeding is well-established, usually about 3 to 4 weeks after birth.”

• Pacifier use in first 2 weeks postpartum=increased risks for stopping exclusive breastfeeding

• No increased problems noted with pacifier use after the first 2 weeks postpartum

Baby-Friendly Hospital Initiative

• The Baby-Friendly Hospital Initiative (BFHI) was launched by the World Health Organization (WHO) and United Nations Children's Fund (UNICEF) in 1991 to ensure that all birthing hospitals and centers become "centers of breastfeeding support."

• In order to receive a Baby-Friendly designation, birthing facilities must implement the Ten Steps to Successful Breastfeeding and comply with the International Code of Marketing of Breast-Milk Substitutes.

• To date, more than 20,000 maternity facilities in 150 countries around the world have earned the Baby-Friendly designation.
