A Community Health Needs Assessment (CHNA) is a process involving the community, to identify and analyze community health needs and assets, in order to prioritize those needs, and develop a plan to act upon the significant unmet needs in the community. The CHNA along with the Implementation Plan are conducted every three years. The plan for 2014-2016 identified Access to Care, Mental Health & Substance Abuse, and Nutrition, Physical Activity and Weight, to be the primary areas of focus for community benefit funds. We have recently completed the CHNA and Implementation Plan for 2017-2019. These plans are always available on our website; ReidHealth.org
