We chose a user-friendly web address so faculty can remember where to go to enter their information. This address is not public to ensure data comes only from faculty.

As faculty enter in their information, they can always click the “help” icon to get more specific instruction for each element of the web form.

Since we are a multi-campus institution, we needed a drop-down list so faculty can select their campus. The drop-down selection allows us to define specific input data and eliminate spelling errors.

Another drop-down list… this time it is to select the presentation format of the course.

What is nice about these drop-down lists is that you cannot go on to the next input option without first selecting something in the current drop-down list… here is one to select the academic division.

This drop-down is to select the academic discipline or program in which the course is taught.

And finally, this drop-down is to select which goal was assessed during this semester.

Click on “Next” to continue to the next section of the web form.

This is the second step of the web form. Here, faculty can enter their assessment details (such as assessment activity and criteria.)

The next two input boxes ask for the number of students assessed and the number of those students who were successful. Notice the help dialog when you click on the “help” icon.

As they scroll down on this page, faculty can submit action plans based on assessment results. The first is their own action plan, the second is for the Institution. Click on the “Next” button to continue through the form.

The third step of the web form is for review purposes only. Faculty members should review what they are about to submit to ensure everything is correct.

As you continue down the third page of the web form, you see more information to review before submission. When faculty are sure everything is correct, they must click on “Submit” to finalize their report.

This is the fourth page of the web form. At this point, the assessment information has been submitted and cannot be changed. This is simply a confirmation page.

This is the second half of the confirmation page. A confirmation email has been sent to the email address entered on the first page of the web form and assessment data has been submitted to the SQL database.
