Payroll accounts that are inactive
for 6 months are automatically
reclassified.

In case the authorized representative loses the card or PIN
mailer:
The authorized signatory should immediately send a request
of cancellation for the card to the Relationship
Manager/Business Manager. A notarized affidavit of loss with
a signed application for Card Replacement should be
presented to UnionBank.
In case the employee loses the card or PIN mailer:
The employee should submit the ff. to the branch:
Notice of Loss

Completely filled out Card Replacement
And pay the Card Replacement Fee either through cash or
debit his/her account
