The DWS Reality •  Since 2009 our caseloads

have doubled, yet our workforce has decreased

•  We have turned to innovation and technology to maintain our level of customer service

•  We need your help to make myCase work for our mutual customers

3rd Party Access You are the missing piece!

Why use MyCase? •  It’s free! •  Easy to use! Only one

account is needed to access multiple customers

•  No need to wait on hold for eligibility

•  Notices can be read electronically by authorized representatives

•  Information is dynamic and immediate

Then click on myCase

This number will be given to your customers so they can add you as a representative

New Form To Help Our Customer! •  Some of our customers have problems granting

access to 3rd parties online due to various reasons (elderly, disabled, etc.)

•  The 114MC form has been created for our customers to complete and sign in order to grant their 3rd party access to view their case information through myCase

•  The Third Party/Provider will need to set up their own third party myCase account first and need to fill in their MC# on the 114MC form

•  When a customer completes and signs this form, selected DWS specialists will grant the third party access to myCase, based on what the customer listed on the 114mc form (View, Full, Notices, Verifications)

•  This form will be barcoded and available to the customer in eDocs and myCase

•  Customers will still have the ability to go into their own myCase account and grant a third party access

View access will grant the Third Party access to view case information only

Full Access will allow the Third Party access to view, update, alter, and make changes to the customers information

Notices will allow the Third Party to view notices the customer has received

Verifications will give access to the Third Party/provider to view and request for verifications that DWS has requested from the customer

Important information for the customer! Access will be granted within one (1) business day

We must have the Customer Signature

And the third party/provider

the customer with the third party/provider

Make sure the

date fields are complete

The customer will provide the 114mc to the Department of

Workforce Service

Once the 114mc is provided, DWS employees will give access to the Third

Party/Provider to view case information of the customer in myCase

Log on to: jobs.utah.gov

Then click on myCase

Go to 3rd Party/Auth Rep Sign-On

Sign on using your Screen Name and Password you created

How to View Case Information

You cannot enter a case number until the Terms & Conditions is agreed to

In order to view the customers information, he Third Party/Provider will need to log into myCase
