• Decrease mortality and morbidity rates of breast and cervical cancer through delivery of timely quality care – Decrease health disparities – Improve screening rates and increase early detection – Initiate health system interventions

• Well-woman exams including clinical breast exams and pap smears

• Screening Mammograms • Diagnostic Services for breast for both men and women • Diagnostic Services for abnormal pap smears • Patient Navigation along the continuum of care • Community Education and Outreach

• Enrollment of Patients- Consent Form, Eligibility • Preventative Health Visits with CBE and Pap • Completion of Transmittal Sheets-Data • Patient Navigation • Follow Reimbursement Guidelines

Health System Interventions

• Patient Reminders – Text messages, reminder cards

• Provider Reminders – EHR prompts

• Provider Feedback – QA/QI Review – Annual Feedback

• Collection of baseline data • Initiation of health system intervention • Collection of annual data
